How to Fill Out and Submit the Allegiant Air Service Animal Form

To fly with your service dog on Allegiant, complete the U.S. Department of Transportation Service Animal Air Transportation Form and send it to Allegiant Air at least 48 hours before departure if you booked that far ahead. The service animal form Allegiant Air requires is the same one-page federal document every U.S. airline uses, and only dogs qualify, regardless of breed or size.

What the Form Asks You to Fill In

The DOT form has five short sections. Every checkbox and blank has to be filled; a partial form gives the airline grounds to reject it.

Section A: Your Contact Information

Full name, phone number, and email. You check a box attesting that a service animal is needed in the cabin, either for you or for the passenger with a disability you’re traveling with.1U.S. Department of Transportation. U.S. Department of Transportation Service Animal Air Transportation Form

Section B: Your Dog and Its Health

Enter the dog’s name, a physical description with weight and color, and your veterinarian’s name and phone number. You attest that the dog is free of fleas, ticks, and communicable diseases, and that it is vaccinated against rabies. The form asks for the rabies vaccination expiration date, so pull the record before you sign; a lapsed vaccination makes the attestation false.1U.S. Department of Transportation. Section C: Task Training

You attest that the dog has been individually trained to perform a specific task that assists with your disability, and you list the name and phone number of the trainer or organization. If you trained the dog yourself, the form’s instructions say you may enter your own name and contact information.1U.S. Department of Transportation. Section D: Behavior Training

This is the section with the most legal weight. You attest that the dog has been trained to behave in public and you list the behavior trainer or organization; again, self-trainers can enter their own information. You confirm you understand what “properly trained” means here: no biting, barking, jumping, lunging, injuring people or animals, or relieving itself on the aircraft or in the gate area. You then separately attest that, to the best of your knowledge, the dog has not behaved aggressively or caused serious injury. If you cannot make that attestation, there is a space to explain.1U.S. Department of Transportation. U.S. Department of Transportation Service Animal Air Transportation Form

Section E: Final Assurances and Signature

You confirm three things: the dog will be harnessed, leashed, or tethered at all times in the airport and on the aircraft; the airline can bill you for damage the dog causes; and you are signing an official federal document. Then you sign and date. The date must be on or after the day you purchased your ticket. An old form from a previous trip will not be accepted.2eCFR. 14 CFR 382.75 – May a Carrier Require Documentation From Passengers With Disabilities Seeking to Travel With a Service Animal

How and When to Send the Form to Allegiant

If you booked more than 48 hours before departure, Allegiant can require the completed form up to 48 hours in advance. If you bought your ticket inside that window, the airline cannot require advance submission; you bring the completed form to the gate on the day of travel.3Allegiant Air. U.S. Department of Transportation Service Animal Air Transportation Form

Federal rules require the airline to accept either an electronic or a paper form.2eCFR. 14 CFR 382.75 – May a Carrier Require Documentation From Passengers With Disabilities Seeking to Travel With a Service Animal To submit electronically, email the signed form to Allegiant’s accessibility team at Assistive.Devices@allegiantair.com. If you need to reach the airline by phone, Allegiant Customer Care is at (702) 505-8888.

Bring a copy with you even after emailing it. Gate agents or the flight crew may ask to see the form during boarding. Allegiant is also required to keep blank DOT forms available at every airport it serves, so if your submission gets lost, you can request one at the gate and fill it out on the spot.2eCFR. 14 CFR 382.75 – May a Carrier Require Documentation From Passengers With Disabilities Seeking to Travel With a Service Animal

Rules at the Airport and in the Cabin

Your dog must be harnessed, leashed, or tethered at all times, both in the airport and on the plane.1U.S. Department of Transportation. U.S. Department of Transportation Service Animal Air Transportation Form The DOT requires airlines to let your service dog ride in the space under the seat in front of you. It cannot block an aisle or an emergency exit, so you will not be seated in an exit row.4US Department of Transportation. Service Animals

A small service dog may be allowed on your lap if it can be done safely. A larger dog that will not fit in the floor space may require a reassignment to a row with more room, if one is available. The dog cannot occupy a passenger seat.4US Department of Transportation. Service Animals

What Happens if the Dog Misbehaves or Causes Damage

If the dog shows it hasn’t been trained to behave in public by biting, repeated barking, lunging, or relieving itself in the cabin or gate area, the airline has two options. It can reclassify the dog as a pet, in which case you would owe a pet fee and the dog would need to travel in a carrier, or it can deny transport entirely.1U.S. Department of Transportation. U.S. Department of Transportation Service Animal Air Transportation Form Allegiant’s pet fee is $50 per flight segment.5Allegiant Air. Traveling with Pets

If the dog damages the aircraft, Allegiant can bill you for repairs, provided it would charge any other passenger for the same kind of damage.1U.S. Department of Transportation. U.S. Department of Transportation Service Animal Air Transportation Form

The Federal Warning on the Form

The top of the DOT form carries a federal warning: knowingly making materially false, fictitious, or fraudulent statements to obtain disability accommodations is a federal crime under 18 U.S.C. ยง 1001. That covers claiming a pet is a trained service animal, fabricating vaccination dates, or inventing trainer information. Beyond criminal exposure, the airline can deny boarding and flag your record for future travel.1U.S. Department of Transportation. Animals the Form Does Not Cover

Only dogs qualify as service animals under current DOT rules. Emotional support animals, comfort animals, companionship animals, and service animals in training are not covered.4US Department of Transportation. Service Animals A cat, a non-service dog, or any other domestic animal falls under Allegiant’s pet policy, which charges $50 per segment and limits pets to domestic dogs and cats in an under-seat carrier.5Allegiant Air. Psychiatric service dogs are covered on the same terms as any other service animal and use the same form.6U.S. Department of Transportation. Traveling by Air with Service Animals Final Rule

The Second Form for Flights Eight Hours or Longer

For any flight segment scheduled at eight hours or longer, airlines may require a second document, the DOT Service Animal Relief Attestation, on which you confirm the dog either will not need to relieve itself during the flight or can do so sanitarily, for example with a dog diaper.2eCFR. 14 CFR 382.75 – May a Carrier Require Documentation From Passengers With Disabilities Seeking to Travel With a Service Animal Most Allegiant routes are well under that threshold, so this form rarely comes up. If you have a connecting itinerary with an unusually long segment, contact Allegiant’s accessibility team to confirm whether it’s needed.
