How to Fill Out and Submit the 32BJ 401(k) Contribution Form

The 32BJ 401(k) contribution form is a one-page authorization you complete and hand to your employer’s payroll office to start, change, or stop pre-tax deductions into your Supplemental Retirement Savings Plan (SRSP) account. You do not send it to the 32BJ Benefit Funds and you do not send it to John Hancock. Payroll signs off on the bottom of the form, then begins withholding the flat dollar amount you wrote in.

Confirm You’re Eligible Before You Fill It Out

Only SEIU Local 32BJ members whose collective bargaining agreement specifically includes the SRSP benefit can enroll. Some 32BJ contracts cover only pension or health fund benefits, so check your contract language or use the “What Are My Benefits” tool on the 32BJ Funds website if you’re not sure.132BJ Funds. The SRSP is separate from the 32BJ Health Fund and from the defined-benefit pension programs. Enrolling in one does not enroll you in the others.

Where to Get the Form

The form is a single page, available in English and Spanish, and can be downloaded from the 32BJ Benefit Funds website or picked up through your union local.332BJ Benefit Funds. 32BJ 401(k) Contribution Form Grab a recent paystub before you sit down with it. You’ll need your employee number, employer name, and date of hire.

Filling Out the Participant Information

Enter your full legal name, the last four digits of your Social Security number, your mailing address, email address, employee number, date of hire, and date of birth. Use the name and address that match your payroll records exactly. A mismatch, like a maiden name on the form and a married name on your paystub, can delay processing.

Choosing Your Contribution Amount

The form gives you two options:

  • Section A is for starting new pre-tax contributions or changing an existing deferral amount. Write in the dollar amount you want deducted from each paycheck. The minimum is $10 per paycheck.2Building Service 32BJ Funds. Supplemental Retirement Saving Plan Notice to Participating Employers
  • Section B is for stopping contributions. Check this box to halt future pre-tax deductions. Your existing balance stays invested with John Hancock; only new deductions stop.

The election is a flat dollar amount per paycheck, not a percentage of wages. If you’re aiming at an annual savings target, divide it by the number of pay periods in your year. A weekly-paid worker aiming for roughly $5,200 a year would write $100.

Contributions to the SRSP are pre-tax. The plan’s current materials do not describe a Roth (after-tax) option.132BJ Funds. Supplemental Retirement Savings Plan 401(k)

2026 IRS Limits to Stay Under

Whatever dollar figure you write in, your total elective deferrals across every 401(k) you participate in during the year cannot exceed the IRS cap. For 2026:

If you have a second job with a 401(k), the ceiling applies to your combined deferrals. Exceeding it triggers extra taxes.5Internal Revenue Service. Consequences to a Participant Who Makes Excess Annual Salary Deferrals

The Employer Approval Section

The bottom of the form is for your employer’s payroll office to mark the election approved or not approved and to record the date they received it. Leave that section blank. Just make sure it’s visible and not covered when you hand the form over.

When Changes Take Effect

You can submit the form when you first become eligible, and again each December to start or change your deferral. Changes submitted during the December window take effect in the first pay period of January.2Building Service 32BJ Funds. Supplemental Retirement Saving Plan Notice to Participating Employers

Where to Submit It

Return the completed form to your employer’s payroll office. Not to 32BJ Benefit Funds. Not to John Hancock. The form itself directs you to payroll, and the employer approval line at the bottom is designed for your payroll department to sign before contributions begin. If you don’t know who handles payroll at your building or worksite, ask your shop steward or building manager.

Keep a copy of the signed form. Deductions don’t start the day you turn it in; your employer needs time to process the paperwork and align it with the pay cycle. If you don’t see the deduction on your paystub within two to three pay periods, follow up with payroll directly. For questions about the plan itself, 32BJ Member Services is at 800-551-3225 (Monday through Friday, 8:30 a.m. to 8:00 p.m.; Saturday, 9:00 a.m. to 5:00 p.m.).632BJ Health Fund. 32BJ Health Fund – Member Services

After the Form: Managing Your Account

Once contributions start flowing in, John Hancock handles the investment side. You choose how your money is allocated among the funds available in the plan. Beneficiary designations are handled through John Hancock’s portal, not the contribution form. Log in to add or update primary and contingent beneficiaries, make sure the percentages at each tier add up to exactly 100, and revisit your choices after major life events like marriage, divorce, or the birth of a child.
