How to Fill Out and Submit PBGC Form 700: Documents and Filing

PBGC Form 700 is the application you file with the Pension Benefit Guaranty Corporation to start monthly payments from a pension plan the agency has taken over. The form is not posted as a public download. You get it one of two ways: apply online through the PBGC’s MyPBA portal, or call the Customer Contact Center at 1-800-400-7242 to have a paper application mailed to you.1Pension Benefit Guaranty Corporation. Who Can File Form 700

You can file if your former employer’s defined benefit pension plan has been terminated and trusteed by the PBGC, and you are eligible (or will be within the next 180 days) to begin benefits under your plan’s rules.2Pension Benefit Guaranty Corporation. Apply for Your Benefits

You also need to be vested. If your employment ended after January 1, 1984, that generally means at least five years of service; before that date, ten years.3Pension Benefit Guaranty Corporation. Request a Benefit Estimate Before You Apply

If you plan to start your pension within the next four months, request a benefit estimate first. This step is required, and skipping it stalls the application.5Pension Benefit Guaranty Corporation. The estimate arrives by mail in 15 to 45 days and lists the actual monthly dollar amount for each annuity option your plan offers: straight-life, joint-and-survivor at different survivor percentages, and any other forms available. With those numbers in hand, the payment-option question on Form 700 becomes a real comparison rather than a guess.

Documents to Gather

The PBGC will contact you for anything missing after you file, but collecting these first avoids weeks of back-and-forth.2Pension Benefit Guaranty Corporation. Apply for Your Benefits

  • Proof of age. A birth certificate or passport works; a driver’s license does not. You need proof of age for any beneficiary you name as well.
  • Social Security number, plus Form SSA-L99-C1 if Social Security has sent you one. Without it, you may need detailed earnings information from the Social Security Administration.6Pension Benefit Guaranty Corporation. What to Do Before Contacting PBGC
  • The exact legal name of your former employer’s pension plan and the PBGC case number. Both appear on prior PBGC correspondence.
  • Marriage certificate, if you want a joint-and-survivor annuity or if your spouse may be entitled to benefits.
  • For a prior marriage, a Qualified Domestic Relations Order the PBGC has approved. Names on the application should match your legal documents exactly. A mismatch between a birth certificate and the PBGC’s records is one of the most common reasons applications sit.

How to Submit the Application

Online Through MyPBA

MyPBA is the faster route. You apply, upload birth certificates and marriage licenses, and track status inside the portal.8Pension Benefit Guaranty Corporation. By Mail

Write your full name and customer ID (or the last four digits of your Social Security number) at the top of each page you send. If you are comfortable online, MyPBA cuts out both lost mail and manual data-entry errors.

Choose Your Annuity Option

Form 700 makes you pick the payment form that will govern your pension for life. The choice is binding once payments begin, so use the dollar figures from your benefit estimate to compare.

  • Straight-life annuity. Highest monthly amount, but payments stop when you die. Nothing continues to a spouse or beneficiary.
  • Joint-and-survivor annuity. Reduced monthly amount during your lifetime, then 50%, 75%, or 100% of that amount continues to your beneficiary after you die. The higher the survivor percentage, the lower your own monthly check.10Pension Benefit Guaranty Corporation. Benefit Options

If you are married and select the straight-life option (or any form leaving nothing to your spouse), your spouse may need to consent in writing. The joint-and-survivor form exists to protect a surviving spouse, and both of you should understand what waiving it means.

If the total value of your benefit is $7,000 or less for a plan ending after January 1, 2024, the PBGC may pay a lump sum instead of monthly installments.

Federal Tax Withholding

PBGC pension payments are taxable, and the agency withholds federal income tax from each check. With Form 700, submit IRS Form W-4P to set your filing status and any adjustments.

If you don’t submit a W-4P, the PBGC defaults to withholding as single with no adjustments, which usually withholds more than needed.11Internal Revenue Service. The PBGC does not withhold state income tax. If your state taxes pension income, plan for estimated payments or withhold from another source.

Set Up Direct Deposit

Direct deposit is the most reliable way to be paid. Set it up with PBGC Form 710 (Application for Electronic Direct Deposit) or through MyPBA. You will need your bank’s nine-digit routing number, your account number, and whether the account is checking or savings.13Pension Benefit Guaranty Corporation. Direct deposits post no later than the first business day of the month; when the first falls on a weekend or holiday, the deposit usually lands before it. Paper checks are mailed before the first, but the PBGC does not guarantee a delivery date.14Pension Benefit Guaranty Corporation. PBGC Payment Dates

What Happens After You File

The PBGC aims to process fully completed applications within 45 days. Incomplete files, whether missing proof of age, an unsigned form, or a required spousal document, take longer because the agency has to write to you and wait. When processing finishes, you receive a benefit determination letter with your final monthly amount and payment start date.

If your address, bank account, or withholding preference changes while your application is pending or after payments start, update it through MyPBA. For a mailed change of address, use PBGC Form COA with your full name, Social Security number or customer ID, and a signed date.15Pension Benefit Guaranty Corporation. Change of Home Address

If the Determination Looks Wrong

You have 45 calendar days from the date of your benefit determination to file a written appeal with the PBGC Appeals Board. If day 45 falls on a weekend or federal holiday, the deadline moves to the next business day.16Pension Benefit Guaranty Corporation. Your Right to Appeal

The appeal should explain why you believe the determination is wrong, state what you want changed, and include supporting documents. PBGC Form 724 is an optional template. Send the appeal by mail to PBGC, ATTN: Appeals Board, PO Box 151750, Alexandria, VA 22315-1750, by fax to 202-229-4095, or by email to appeals@pbgc.gov.
