Confirm You Are Eligible Before You Start
U.S. citizenship is mandatory for every program. 10 U.S.C. ยง532 makes it a legal prerequisite for holding a commission, and Block 6 of the form is where you document it.3MyNavyHR. Enlisted to Officer Commissioning Programs Playbook
Beyond that, criteria vary sharply by program:
- Officer Candidate School requires a bachelor’s degree (or completion by commissioning) and a minimum Officer Aptitude Rating score on the ASTB-E. Candidates must be between 19 and 35 years old at commissioning. Age waivers are case-by-case.
- Seaman to Admiral-21 is a degree-completion program, so a bachelor’s degree is not required. You need a minimum cumulative GPA of 2.5 on your high school or college transcript and at least 12 semester hours if you are using college credits.4Naval Service Training Command. You can apply to more than one program in the same package. Block 2 lets you check every program you want to be considered for. Some programs use different selection boards, so you may need to send duplicate packages to different addresses; each program’s NAVADMIN spells out the routing.2Department of the Navy. Documents to Gather Before You Open the Form
The form itself is not the bottleneck. Chasing records is. Start pulling these together weeks before your board deadline:
- Your Joint Service Transcript and Sailor/Marine American Council on Education Registry Transcript to verify military training credits, plus one certified official transcript from every college you have attended. For STA-21 and other degree-completion tracks, include high school transcripts, or a GED certificate if you did not graduate.2Department of the Navy. - Your Official Military Personnel File, reviewed for completeness. Every evaluation, award, and fitness report should be current. Missing evaluations are one of the easiest ways to get a package kicked back.
- A commissioning physical using DD Form 2807-1 (Report of Medical History) and DD Form 2808 (Report of Medical Examination), plus a dental clearance signature. - Proof of citizenship: birth certificate, valid passport, or naturalization certificate.
- ASTB-E or OAR scores for OCS and aviation candidates. The Officer Aptitude Rating floor is 35, but competitive programs select well above that. Aviation tracks add sub-score minimums for the AQR, PFAR, and FOFAR sections. Your program’s NAVADMIN sets the specific requirement.
- Results from your last three consecutive official PFAs, including cardio event and time, forearm plank time, push-up count, and overall score category.
Filling Out the OPNAV 1420/1 Block by Block

The instruction warns that illegible entries can be misinterpreted by a selection board and hurt your chances.2Department of the Navy. Biographical and Service Blocks
The opening blocks collect your legal name, any prior names used (important if transcripts show a different name), the programs you are applying for, your desired officer community and designator, and the usual identifying data. Blocks 7 through 18 cover service history: dates of enlistment, time in rate, duty stations, and total active service. Every entry must match your official records. A one-digit discrepancy in a date can trigger an administrative review that delays the whole package.
Block 19: Physical Fitness Assessment
Your Command Fitness Leader completes Block 19, not you. It captures the last three consecutive official PFAs: cardio event type (run, row, swim, or bike) and time, forearm plank time, push-up count, height, weight, and Body Composition Assessment result.2Department of the Navy. Education Blocks
Block 21 covers high school and GED information. OCS applicants can skip it; the block is aimed at applicants who did not graduate from high school. Block 22 captures college attendance: school name, dates attended, credit hours, and degree status. If you are applying for a degree-completion program and have not finished a bachelor’s, enter the total fully transferable credits you hold. Block 23 asks for your degree preference. Writing the Personal Statement
This is your one chance to speak to board members in your own voice. Focus on leadership experiences, why you want a commission, and how your enlisted career has prepared you for officer responsibilities. Keep it to a single page with standard margins and a readable font. Boards read hundreds of these. A specific statement that shows self-awareness lands better than generic patriotic language or a list of accomplishments the board can already see in your record.
Interview Appraisals
Attached to the main form are Interview Appraisal Sheets, where a panel of officers rates you on professional knowledge, communication, and leadership potential. Schedule these interviews early. Officers at your command have their own operational demands, and trying to collect five appraisals in the final week before a deadline produces rushed, generic write-ups. Give each interviewer your resume and personal statement in advance so their assessments contain specific observations instead of boilerplate.
The Commanding Officer Endorsement
Your CO’s endorsement is not optional. Every applicant must have CO support to be eligible.2Department of the Navy. Without the CO’s signature, the application is incomplete and will not reach a selection board.
A lukewarm endorsement (“recommend with reservations”) is often worse than not applying, because boards read between the lines. If your CO is new and does not know you well, give them a summary of your accomplishments and copies of your recent evaluations so the recommendation can be strong and specific.
Submitting the Package
Submission routing depends on the program. Most OCS and officer community packages are emailed to Navy Recruiting Command as two separate PDFs, one for the application and one for the medical documentation, each under 10 MB.1MyNavy HR. Submit early. The FY26 board schedule states plainly that any application arriving on the due date that needs corrections or a waiver rolls to the following board.8MyNavyHR. If you receive an Exception to Policy to submit late, the exception must be cleared at least two weeks before the board convenes.
After sending, confirm receipt with the program manager listed in the NAVADMIN or through the MyNavy HR Commissioning Programs page.1MyNavy HR. Commissioning Programs File-size limits and formatting issues can silently block emailed submissions, so do not assume silence means success.
Deadlines to Watch
There is no single annual deadline. Each program runs its own selection board calendar, and some communities hold multiple boards per year. Two program-specific dates to know for 2026:
- STA-21 (FY27 cycle): applications due 1 July 2026, board scheduled for August 2026.
