When You Actually Need the 97CI
Not every update requires this form. If you just want to change your address, phone number, or email, sign in to online banking or the mobile app, open your profile, and save the new details on screen.2Navy Federal Credit Union. Digital Banking Video Tutorials No PDF, no signature.
The 97CI is the right form when the change needs documentation or a signed disclosure on file:
- A legal name change after marriage, divorce, or a court order.
- A new or corrected Social Security Number or ITIN.
- Employment details: employer, job title or rank, employment category, re-enlistment or EAOS date, or a switch to retired status.
- A correction to your date of birth.

Filling Out the Form, Section by Section
The 97CI is available under “Frequently Requested Forms” on navyfederal.org/forms.html.4Navy Federal Credit Union. Forms and Brochures It has five sections, A through E. Only Sections A and E are needed on every submission; complete the others only if they apply.
Section A: Current Information
This is how Navy Federal matches the form to your account. Enter your full legal name exactly as it appears on the account today, your access number, and your Social Security Number. Every submission needs this section filled in.
Section B: New Information
Fill in only what you’re changing. Fields cover your name (with an optional title prefix), home address, a separate mailing address, date of birth, SSN or ITIN, mobile number, another contact number, and email address.
One thing that trips people up: the home address field cannot be a P.O. Box. If your mail goes to a P.O. Box, put the box on the mailing address line and your physical street address on the home address line.
If you’re stationed overseas, use your APO, FPO, or DPO address on the mailing line, formatted with your name, rank, unit, military address, and ZIP.
Section C: Employment Information
Section C captures your employer, job title or rank, employment category (DoD Military, Civilian DoD Employee, or Non-DoD Civilian Employee), and re-enlistment or EAOS date if applicable. Three checkboxes cover retirement and non-wage-earner status. Keeping this current matters if you plan to apply for a loan, since Navy Federal uses employment data in underwriting.
Section D: Disclosure Agreement
Section D is Navy Federal’s standard disclosures covering consumer reports, escheatment, identification, and statutory lien rights. Read it before you sign. Signing in Section E is your acknowledgment of these terms.
Section E: Signature and Tax Certification
Sign and date the form. The tax certification includes a checkbox for members who are not U.S. citizens or permanent residents. No notary is required; your signature alone is sufficient, even for a name change submitted by mail.1Navy Federal Credit Union. Change of Information
Documents to Attach for a Name Change
Name changes are the most document-heavy use of the 97CI. Navy Federal treats a last-name-only change differently from a full name change.
For a last-name-only change, send two documents:
- Proof of your previous last name: marriage license, divorce decree, or court-ordered name change document.
- Proof of your new last name: driver’s license or government-issued photo ID showing the new name.
For a full name change (first and last), send three:
- A driver’s license or government-issued photo ID showing the new name.
- A second document with the new name, such as a court document or updated Social Security card.
- Proof of your previous name: marriage license, divorce decree, or court document.
Send photocopies, not originals. If you’re using the online message center, scan or photograph the documents and attach them along with the completed form.1Navy Federal Credit Union. Change of Information
How to Submit the 97CI
Navy Federal accepts the completed form four ways:
- Fax to 703-206-4600. Mark the cover sheet “ATTN: Membership Administration.”
- Mail the form and photocopies to Navy Federal Credit Union, PO Box 3002, Merrifield, VA 22116-9887.
- Online banking. Sign in, open the Message Center, click Create New Message, choose General from the topic dropdown, pick a reason, write a short note, and attach the form and documents.
- In person at any Navy Federal branch, where a representative can verify your ID and process the change while you wait.
Watch the mailing address. The forms box (PO Box 3002, Merrifield, VA 22116-9887) is not the same as Navy Federal’s general correspondence box (PO Box 3000, Merrifield, VA 22119-3000). Sending the 97CI to the wrong one will delay processing. If you have questions first, member service is available 24/7 at 1-888-842-6328.5Navy Federal Credit Union. Contact Us
After You Submit
Membership Administration reviews the form and verifies your documents. In-branch submissions are often processed the same day. Fax, mail, and message-center submissions typically take a few business days; Navy Federal does not publish a guaranteed processing window. If a document is missing or a photocopy is unreadable, a representative may contact you before making the change.
Once it’s done, log in and check your profile to confirm every field you changed. For a name change, verify that your debit card and any linked accounts show the new name.
Credit Report Follow-Up
Navy Federal reports account data to the major credit bureaus on a monthly cycle, so a name or address change will flow through to your credit reports, but not instantly. If your report still shows the old information after a billing cycle or two, you can dispute it with the bureau directly; furnishers who receive a dispute have 30 days to investigate and respond.6Consumer Financial Protection Bureau. Tax Reporting
The name and SSN on your Navy Federal account also drive your Form 1099-INT. If the name or number on file doesn’t match IRS records, the IRS can send Navy Federal a CP2100 or CP2100A notice, which can put your interest income into backup withholding until the mismatch is fixed.7Internal Revenue Service. Backup Withholding “B” Program Filing the 97CI proactively after a name change or SSN correction keeps that from happening.
