Confirm the Refund Was Actually Sent
Before starting a trace, check Where’s My Refund at irs.gov or through the IRS2Go app. The tool updates 24 hours after the IRS accepts an e-filed return and three days after a prior-year e-filed return. If it shows the refund was issued but nothing arrived, that is your cue to start a trace. If it still shows the return as processing, the refund hasn’t been sent yet and no trace is possible.
How Long You Have to Wait Before Starting a Trace

- Direct deposit: at least 5 days from the scheduled deposit date.
- Paper check: at least 4 weeks from the mailing date.
- Paper check with a recent change of address on file with the Postal Service: at least 6 weeks.
- Foreign address: at least 9 weeks from the mailing date.
Mark the date and use the wait to gather your documents.
Who Needs the Paper Form

- Go to Where’s My Refund at irs.gov or open the IRS2Go app and follow the prompts to request a trace.

What to Pull From Your Return First
Have your original return in front of you. You’ll need:
- Social Security numbers for you and your spouse if you filed jointly.
- Filing status as shown on the return.
- Tax year of the return that generated the missing refund.
- The exact refund amount from the return, not a rounded estimate.
- If you asked for direct deposit, the bank name, routing number, and account number you provided.

Filling Out the Form
Section I: Personal Information
Enter your current name, taxpayer identification number (Social Security number or ITIN for individuals), and mailing address with ZIP code. For a joint return, enter information for both spouses on lines 1 and 2.4Internal Revenue Service. Verify every digit. Transposed routing or account numbers are one of the most common reasons a trace stalls.

- Box 8 if you never received the refund, or you received a check that was lost, stolen, or destroyed.
- Box 9 if you received the check and endorsed it, but someone else cashed it.
Read the warning next to Box 9 carefully. The IRS will not issue a replacement if you endorsed the check and someone else then cashed it, because your signature is treated as valid. Section III: Signature
Sign and date under penalty of perjury. On a joint return, both spouses must sign or the IRS will not open the trace.4Internal Revenue Service. Form 3911, Taxpayer Statement Regarding Refund Missing signatures are the most common reason these forms get returned.

Mail or fax the completed form to the IRS Refund Inquiry Unit assigned to the state where you filed. Those are fax numbers, not voice lines. States occasionally get reassigned between offices, so check the IRS page before sending.
What Happens After You File
The outcome depends on what the trace uncovers.
If a direct deposit went to the wrong account, the IRS works with the receiving bank to recover the funds. Banks have up to 90 days from the trace start date to respond, and full resolution can take up to 120 days.
If a paper check was never cashed, Treasury voids the original and issues a replacement. That is usually the fastest scenario to resolve.
If the check was cashed by someone else, you’ll receive a claim package within about six weeks. Complete and return it to the Bureau of the Fiscal Service, which investigates the forgery and issues a replacement if the endorsement is found to be forged.1Taxpayer Advocate Service. When Form 3911 Is the Wrong Tool
Refund Offsets
A refund that came up short, or didn’t arrive at all because the government applied it to a debt, hasn’t been lost. It’s been offset. The Treasury Offset Program can redirect a refund toward past-due child support, federal agency debts, state income tax obligations, and certain unemployment compensation debts.5Internal Revenue Service. The Bureau of the Fiscal Service sends a notice listing the original refund amount, the offset amount, and the agency that received the money. Dispute the debt with that agency, not the IRS. Tax-Related Identity Theft
If someone filed a fraudulent return using your Social Security number, Form 3911 will not resolve it. Use Form 14039, Identity Theft Affidavit. Signs pointing to identity theft rather than a lost check include being unable to e-file because a return was already submitted under your SSN, receiving a tax transcript you didn’t request, or getting IRS notices about income from an employer you never worked for. Expired Refund Checks
Federal refund checks expire one year after the issue date. A bank won’t accept one past that point. Contact the IRS for instructions on returning the expired check and requesting a reissue.4Internal Revenue Service. Form 3911, Taxpayer Statement Regarding Refund
The Deadline for Claiming a Missing Refund
There is a hard cutoff. The Refund Statute Expiration Date is generally the later of three years from the date you filed your return or two years from the date you paid the tax.7Internal Revenue Service. Time You Can Claim a Credit or Refund A return filed before the due date is treated as filed on the due date for this calculation. Once the date passes, the refund is gone. If you think a refund went missing more than a year or two ago, don’t sit on it.
