Form SSA-372 is the one-page request you sign to ask Social Security to restart your Supplemental Security Income (SSI) disability payments after they ended because you went back to work. Filing it starts the Expedited Reinstatement (EXR) process, which lets you skip a brand-new SSI application and can provide up to six months of provisional cash payments and Medicaid while SSA reviews your medical condition. Who Can File
You can use SSA-372 if you previously received SSI based on disability or blindness, your payments ended specifically because your earnings crossed the substantial gainful activity (SGA) threshold, and you now cannot work at that level. - Your SSI eligibility ended because of work, not for any other reason.
- The impairment stopping you from working now is the same one that originally qualified you, or a medically related condition.
- In the month you file or the month after, your earnings are below SGA.
- You file no later than 60 months after the month your SSI eligibility ended.
For 2026, the monthly SGA limit is $1,690 for non-blind individuals and $2,830 for people who are statutorily blind. SSA subtracts impairment-related work expenses before comparing your earnings to the limit, so some gross wages above these figures may still fall under. The blind SGA figure applies to SSDI, not SSI, so an SSI recipient who is blind is measured against the non-blind number when SSA looks at whether past earnings were SGA.3Social Security Administration. One boundary worth knowing: if your prior SSI was awarded as a closed period of disability (SSA found you disabled only for a defined past window), expedited reinstatement is not available to you.2Social Security Administration. What the Form Itself Contains
SSA-372 is shorter than most people expect. It is a signed declaration, not an application. The form asks for:4Social Security Administration. Request for Reinstatement – Title XVI
- Your name, Social Security number, daytime phone, and mailing address, plus the same for an eligible spouse if you have one.
- Declarations you attest to by signing: that you are blind or disabled, that your impairment is the same as or related to the one that originally qualified you, that you meet SSI’s non-medical requirements, and that you are not currently performing SGA.
- Your signature and the date. If you sign with a mark, two witnesses must sign and provide their addresses.
- A page-two section a representative payee completes only if provisional payments would go to one.
The detailed questions about your work history, earnings, treating providers, and current symptoms come during the SSA interview, not on the paper form. Field office staff often complete and attest the form electronically during that interview, so you may not need to hand in a paper copy at all.5Social Security Administration. How to File
There is no online option. SSA directs you to either call or go in person:6Social Security Administration. Get Disability Back if Your Benefit Ended
- Call 1-800-772-1213 (TTY 1-800-325-0778) and tell the representative you want to file for expedited reinstatement of your SSI. They will schedule an interview or start the process on the call.
- Visit your local Social Security field office. Bringing your documents to the appointment cuts down on follow-up trips.
What to Have Ready for the Interview
The form is brief, but the case around it takes some preparation. Come to the interview with:
- The dates you worked and how much you earned each month. SSA will pull its own wage records, but organized numbers keep the conversation moving.
- Names, addresses, phone numbers, and fax numbers for every doctor, therapist, hospital, or clinic that has treated you since your SSI ended.
- A plain description of what your condition prevents you from doing now, including how it affects daily activities and the kinds of work you cannot sustain.
- Any vocational rehabilitation history from the period you were working, including who provided the services and when.
You will also sign Form SSA-827, which authorizes SSA and your state disability determination services to obtain your medical records. Provisional Payments and Medicaid During the Review
Once SSA accepts your request, you can receive provisional cash payments and Medicaid coverage for up to six consecutive months while the medical review runs.8Social Security Administration. Expedited Reinstatement The provisional payment is based on your last SSI benefit, adjusted for cost-of-living increases since your eligibility ended. It continues until SSA reaches a final decision or the six months are up, whichever comes first.
If SSA later denies your reinstatement, you generally do not owe those provisional payments back. What Happens After You File
Your case goes to your state’s disability determination services for a medical review under the medical improvement review standard, the same framework SSA uses for continuing disability reviews. The reviewer looks at whether your impairment is still the same as or related to the one that originally qualified you and whether it prevents SGA.2Social Security Administration. SSA sends the final decision in writing to your address on file. If approved, your SSI continues without a gap after the provisional period. If denied, provisional payments stop and the notice explains how to appeal.
One consequence worth flagging: if you also receive other SSA disability benefits (such as SSDI on a separate record) and SSA denies your SSI reinstatement based on medical improvement, SSA may review and potentially end those other benefits as well. If SSA Denies the Request
You have 60 days to file for reconsideration. SSA counts the 60 days from five days after the date on the denial notice, unless you can show you received it later.10Social Security Administration. Program Operations Manual System – Appeals Process Under Expedited Reinstatement Mark the deadline the day the notice arrives. Missing it usually costs you the right to appeal the EXR decision.
If you miss the window, you can ask SSA to accept a late request by showing good cause. SSA considers circumstances such as serious physical or mental illness, loss of records to fire or theft, homelessness that kept you from receiving mail, incarceration, and difficulty reading or understanding English.11Social Security Administration. The 60-Month Deadline
The 60-month clock starts the month after your last month of SSI eligibility. If your SSI ended in March 2022, you have until March 2027 to file SSA-372.
