How to Fill Out and Submit Form FNS-252-E: SNAP Retailer Application

Any retail food store that wants to accept SNAP files Form FNS-252 with the USDA Food and Nutrition Service, and there is no application fee. To fill out Form FNS-252, gather your ownership details, sales breakdown, and staple food inventory counts, attach photo ID for each principal along with your business license and a bank authorization, and submit everything at once through the SNAP Retailer Service Center’s online portal. FNS then has up to 45 days from receipt of a complete application to make a decision.

Confirm Your Store Qualifies Before You Start

The form asks about your inventory and sales because FNS uses the answers to apply one of two federal standards. If your store cannot satisfy either, the application will be denied.

Under Criterion A, your store must continuously stock at least seven different varieties of qualifying staple foods in each of four categories: meat, poultry, or fish; bread or cereals; vegetables or fruits; and dairy products. Seven canned vegetables, seven boxed cereals, seven canned meats, and seven shelf-stable dairy products still fails.

Criterion B is the specialty-store path. If more than 50 percent of your total gross retail sales come from staple foods, you qualify even without the Criterion A breadth. Information to Have Ready Before You Open the Form

  • Legal business name and the Employer Identification Number the IRS assigned to your entity.
  • Every owner, partner, corporate officer, or shareholder’s full name, Social Security Number, date of birth, and home address.
  • The store’s physical address, weekly hours of operation, phone number, and the date current owners acquired the business.
  • Total gross retail sales for the previous tax year, or projected figures if the store is new, broken into staple foods, accessory foods, prepared foods, hot foods, and non-food items. Alcohol and tobacco sales report separately.
  • The number of staple food varieties currently stocked in each of the four categories, plus confirmation that perishable items are present.
  • Whether any listed owner or officer has been sanctioned, suspended, or debarred from SNAP, WIC, or any other federal program, and any history of alcohol, tobacco, lottery, or health violations.

The sales breakdown is where most applications run into trouble. FNS uses it to check that your business model aligns with nutrition assistance goals, and a store reporting that most of its revenue comes from lottery and cigarettes will not survive review. Keep purchase invoices and sales records on hand; FNS may ask you to back up the percentages.

Report the numbers honestly. Documents You’ll Need to Attach

  • Photo ID for every listed principal: driver’s license, passport, or military ID for each owner, partner, officer, and shareholder.
  • A current business license — health permit, food inspection permit, sales tax permit, seller’s permit, general business license, lottery license, or beer/wine license, whichever your jurisdiction requires to operate.
  • The signed and dated certification page from the FNS-252 itself.
  • A bank authorization form so the EBT system can deposit reimbursements into the store’s bank account.

Send everything in one package. Submitting the form now and the documents later just resets your 45-day clock.

How to Submit the Form

The fastest route is the SNAP Retailer Service Center’s online portal at fns.usda.gov/snap/retailer/apply-to-accept. You’ll create a Login.gov account, fill out the form, and upload your attachments. If you can’t use the portal, download the paper form from the same FNS website and mail it with all supporting documents in one package. Use a service with delivery confirmation for proof of receipt. What Happens After You Submit

FNS has up to 45 days from the date it receives your complete application to decide. During that window, FNS may contact you with follow-up questions, request more documentation, or send an inspector to your store without advance notice.4Food and Nutrition Service. SNAP Retailer Service Center

A site visit typically involves photographs of the inside and outside of the store, a sketch of the layout (aisles, shelves, coolers, freezers, storage areas), and an inventory count against the stocking standards. The inspector is checking whether what’s actually on your shelves matches what you reported on the form. Stores that look substantially different from their application face denial or further investigation.

If approved, FNS issues a SNAP permit showing your store name, location, listed owners, and a unique FNS Number. If Your Application Is Denied

The denial letter will state the reason. You have 10 days from the date the denial notice is delivered to file a written request for administrative review. Weekends and federal holidays don’t count toward the deadline if they fall on the last day, and the filing date is the postmark date if you mail the request.6eCFR. 7 CFR Part 279 – Administrative and Judicial Review

Ten days is short. If the inspector visited on a restocking day, documents were misread, or you have evidence your inventory clears the threshold, get your written request out immediately with supporting materials attached: purchase invoices, photographs of stocked shelves, or corrected sales records. Drafting on day nine leaves almost no room for a mistake.
