How to Fill Out and Submit Form 5A: EPF Return of Ownership

Form 5A, the EPF Return of Ownership, is filed online through the EPFO Unified Portal within 15 days of your establishment receiving its PF code, and again within 15 days of any change to the ownership, management, address, or banking details already on record. To fill it correctly you collect establishment, owner, banking, and branch information in advance, enter it into the portal under Establishment → Form 5A, and authenticate the submission using a Digital Signature Certificate or Aadhaar-based e-Sign.1Employees’ Provident Fund Organisation. When You Have to File

Paragraph 36A of the Employees’ Provident Funds Scheme, 1952 requires every employer of a covered factory or establishment to furnish ownership particulars in duplicate to the Regional Commissioner using Form 5A.2Indian Kanoon. The Employees’ Provident Funds Scheme, 1952 EPFO’s Employer Information Booklet states that Form 5A is “normally submitted to the EPFO at the time the company is covered by EPF Schemes for the first time.”3Employees’ Provident Fund Organization. There are two filing triggers. The first is initial coverage: once EPFO allots you a PF code, you have 15 days to submit the form.1Employees’ Provident Fund Organisation. Paragraph 36A requires intimation of any change within 15 days, by registered post or another method the Regional Commissioner specifies.2Indian Kanoon. What to Have Ready Before You Log In

EPFO publishes an Input Data Sheet listing every field on the form. Collecting the right documents in advance saves you from timing out or saving a partial form you cannot submit. The form is organized into establishment details, people in charge, lessee information (if applicable), banking details, and branch offices.4Employees’ Provident Fund Organisation. Establishment Details

The opening section captures the business itself. You will enter the establishment name exactly as it appears on the PAN card, the PAN itself, the registered address (two address lines, city, state, district, PIN), a proof of address, phone and fax numbers, email, web address, the date of setup, whether the premises qualifies as a factory, the primary business activity, the ESIC registration number, and the ownership type — proprietorship, partnership, company, trust, or another structure.4Employees’ Provident Fund Organisation. Fields marked with an asterisk on the portal are mandatory and will block submission if left blank. Fields marked with a hash symbol become required only when a related item is answered “Yes.”4Employees’ Provident Fund Organisation. INPUT DATA SHEET FOR FILLING OF THE FORM 5A

Owner and Manager Details

The form asks for personal details of two categories of people. First, the manager or occupier who runs day-to-day operations. Second, the owners — directors for a company, partners for a partnership firm, or the proprietor for a proprietorship. For each person you provide name, gender, designation, date of birth, father’s name, full residential address, mobile, email, PAN, Director Identification Number (DIN) where applicable, and the date from which the person has held the position. You also indicate whether each owner is the person in charge of the establishment’s business.4Employees’ Provident Fund Organisation. INPUT DATA SHEET FOR FILLING OF THE FORM 5A

These are the people EPFO will identify as responsible for the establishment’s contribution obligations, so incorrect PAN or DIN entries lead to correction requests and delay processing.

Lessee Details

If the establishment runs from leased premises, a separate section opens for the lessee’s name, gender, designation, date of birth, father’s name, residential address, mobile, email, and the lease start and end dates. The section activates only when you indicate the premises are leased.4Employees’ Provident Fund Organisation. INPUT DATA SHEET FOR FILLING OF THE FORM 5A

Bank Details

You will provide the IFS code, bank name, branch name, account number, and account type. If the bank document shows the same address as your registered establishment address, you can mark it as your address proof.4Employees’ Provident Fund Organisation. INPUT DATA SHEET FOR FILLING OF THE FORM 5A

Branch Offices

The final section covers branches. For any branch that already has its own separate PF code, you enter that code and the system pulls the name and address. For branches without a separate code, you enter each branch’s location details manually. Section 2A of the Act governs these branch disclosures.4Employees’ Provident Fund Organisation. Filling and Submitting on the Unified Portal

The filing is entirely electronic. Once your data is ready the sequence is short.

  1. Log in to the EPFO Unified Portal for Employers using your establishment credentials.5Employees’ Provident Fund Organisation. EPFO Unified Portal for Employers
  2. From the dashboard, go to Establishment → Form 5A.3Employees’ Provident Fund Organization. Employer Information Booklet
  3. Fill in or review the fields. Some data pre-populates from your registration; complete every mandatory field.
  4. Save and review the form before authenticating.
  5. Authenticate using a Digital Signature Certificate (DSC) or Aadhaar-based e-Sign.3Employees’ Provident Fund Organization. Employer Information Booklet
  6. Submit. The portal generates a confirmation and a downloadable PDF of the return.

Keep the PDF. You will need it for inspections and audits, and the October 2025 display circular assumes you have the extract readily available.

DSC or Aadhaar E-Sign?

For a Digital Signature Certificate, EPFO requires a Class 2 or Class 3 certificate from a licensed Certifying Authority. The authorized signatory’s name in the DSC must exactly match the name registered on the portal. Registration happens through the Online Transfer Portal (reached from the EPFO homepage, using the same credentials as the ECR portal) under Digital Certificate → Register Certificate. You upload the certificate as a .PFX file or through a USB token, enter the PIN, and submit. After registration, print the confirmation letter EPFO generates, sign it, and send it to your regional EPFO office for final approval.6Employees’ Provident Fund Organisation. Process Flow for Registering Digital Signature Certificate

Aadhaar e-Sign is faster if the authorized signatory has an Aadhaar-linked mobile number. Selecting e-Sign triggers Aadhaar authentication and an OTP to the linked mobile. Enter the OTP, the system generates the electronic signature, and the form is submitted. The signatory must already be mapped as the authorized person on the establishment’s portal account.

Errors That Get Filings Rejected

The most common problem is a PAN verification failure. The portal automatically checks the establishment name against Income Tax Department records. If the name on your form does not match the PAN-registered name, the system rejects the PAN check. You can save the form, but it sits in “PAN Verification Failed” status until you correct the name. EPFO’s Input Data Sheet directs employers to check the correct PAN-linked name on the Income Tax e-Filing portal before resubmitting.4Employees’ Provident Fund Organisation. INPUT DATA SHEET FOR FILLING OF THE FORM 5A

Other frequent snags: leaving mandatory (asterisked) fields blank, which blocks submission outright; entering an incorrect DIN for a director; and trying to authenticate before the DSC has been registered. If you recently changed your bank account, verify the IFS code — an incorrect code will not match the bank name and branch during validation.

Display the Extract at Your Workplace

An EPFO circular issued on 7 October 2025 introduced a new obligation for every covered establishment: prominently display an extract of the submitted Form 5A. The display can be placed at the entrance of the establishment or published on the establishment’s official website and mobile application. Establishments were given 15 days from the date of the circular to comply.

The extract must show five data points: the EPF code, the registered name, the date of EPF coverage, the number of branches with the primary branch address, and the regional EPFO office that handles the establishment. The purpose is to let an inspector arriving at the premises verify registration data without requesting documents from management. Failure to display the extract can result in inspection notices or show-cause communications from the regional office.

Penalties for a Missing or Wrong Filing

Section 14 of the EPF and Miscellaneous Provisions Act sets out the penalties for failing to meet obligations under the Act or its Schemes. For Form 5A specifically, the relevant provision is Section 14(2), covering contraventions of the Scheme: up to one year of imprisonment, a fine of up to ₹4,000, or both. Section 14(1) adds up to one year of imprisonment, a fine of up to ₹5,000, or both, for false statements made to avoid payment.7India Code. Employees’ Provident Funds and Miscellaneous Provisions Act 1952 – Section 14

The individuals named on Form 5A are the people EPFO holds personally accountable. An outdated filing does not shield a current owner or director from liability; it just means EPFO may pursue the wrong person first, creating problems for everyone. Keeping the form current after every ownership or management change is the practical protection.
