How to Fill Out and Submit DA Forms in the U.S. Army

Filling out and submitting a DA form in the U.S. Army comes down to four steps: pull the current version from the Army Publishing Directorate, complete it in Adobe Acrobat with accurate identifiers, sign it with your CAC, and route it through your chain of command or through IPPS-A depending on the action. Getting any of those steps wrong is the fastest way to have a request kicked back at the first review.

Start With the Current Version

The Army Publishing Directorate at armypubs.army.mil is the official repository for every DA form, regulation, and pamphlet. Search by form number or title and download directly. Copies sitting on unit shared drives or third-party sites are often outdated, and an obsolete version usually gets rejected before it reaches the approving authority.

Most forms on the site are publicly accessible. A handful of publishing templates and administrative guidance sit behind a Common Access Card login, but the forms soldiers use day to day — DA Form 31, DA Form 4187, the evaluation report forms — do not require CAC access to download. Family members and veterans without a CAC can still pull blank copies from the public side.

Software and Signatures

The Army moved its forms library from the legacy .xfdl format, which required IBM Lotus Forms Viewer, over to fillable Adobe PDF files. Any current DA form you download today should open, fill, and save in Adobe Acrobat Reader without special software. Older .xfdl files sometimes surface in unit archives, but you should not be filling one out unless someone with authority tells you to.

To digitally sign a form you need a CAC-enabled card reader and the DoD middleware installed on your machine. Under AR 25-50, a CAC-authenticated digital signature is equivalent to wet ink for most Army documents. Digital signatures are not accepted on letters, or on any document that requires a wet signature by law, regulation, or congressional tasking.1Army Publishing Directorate. Information Every DA Form Asks For

Nearly every DA form collects the same core identifiers: your full legal name, Social Security Number, and ten-digit Department of Defense Identification Number. The DoD ID Number is printed on your CAC and has replaced the SSN as the primary identifier for most military transactions, though many forms still collect both.2Military Health System. The small fields cause more problems than the big ones. An incorrectly formatted date or a misspelled unit designator can trigger a digital validation error that prevents the form from saving. If the form references a governing regulation — DA Form 31 points to AR 600-8-10, for example — pull that regulation up before entering anything on a complex financial or legal field.

Every DA form that collects personally identifiable information carries a Privacy Act Statement, usually near the top or bottom. It discloses the legal authority for the collection, the purpose, routine uses, and whether disclosure is mandatory or voluntary. What Happens if the Information Is Wrong

Providing false information on a DA form can be prosecuted under Article 107 of the Uniform Code of Military Justice, which covers false official statements and false official documents made with intent to deceive. Unintentional errors aren’t prosecutable, but they still create real problems: delays, rework, and permanent inaccuracies in your record if the form gets filed before anyone notices. Read every entry once more before signing.

Pick the Right Form for the Action

Hundreds of DA forms exist. Grabbing the wrong one for a routine request is the most common mistake outside of using an outdated version. A short function map for the forms soldiers reach for most:

  • DA Form 4187 (Personnel Action) is the general-purpose form for individual administrative requests: reassignments, name changes, special school applications, separate rations, identification card issues, reclassifications, and dozens of other actions listed in DA Pamphlet 600-8.   • DA Form 31 (Request and Authority for Leave) covers ordinary leave, emergency leave, and pass time, and is governed by AR 600-8-10.   • The DA Form 67-10 series records officer performance, and the DA Form 2166-9 series records NCO performance. Both feed directly into promotion boards, so the duty description and the rater’s narrative comments have to be right.7Georgia National Guard.   • DA Form 3349 (Physical Profile Record) documents medical limitations using the PULHES system. A profiling officer completes the medical assessment and specifies what activities the soldier can perform, including weapon carry, body armor, and the fitness test.   • DA Form 5988-E and DA Form 2404 record equipment faults from inspections. The 5988-E is the automated version printed by motor pool clerks; the 2404 is the manual worksheet used primarily for individual equipment.

Routing and Submitting the Completed Form

Once you sign a DA form, it follows a routing path to the approving authority. The path depends on the form. A leave request may only need a supervisor and commander. An evaluation report passes through rater, senior rater, and sometimes a reviewer. A personnel action can travel further depending on what’s being requested.

IPPS-A is now the primary digital platform for many personnel actions. Through self-service, soldiers can initiate personnel action requests, submit absences, request special pay, and track pending items. The system also handles assignments, awards, rank changes, and separations.10IPPS-A. Any email carrying a completed DA form is almost certainly carrying PII, so it has to be digitally signed and encrypted. Army policy prohibits sending PII from a .mil server to a .com address under any circumstances, and the subject line should be marked “FOUO.”11Department of the Army. Processing times vary. A routine leave request can clear in a day or two. A personnel action like a reassignment or reclassification may take weeks. Evaluation reports follow strict timeline windows tied to the rating period. Fixing a Form After It Was Filed

If you spot an error in your record after submission, how you fix it depends on the severity. Minor administrative mistakes — a misspelled name, a wrong date of birth, or an incorrect unit on a single form — can usually be corrected at the unit level through a DA Form 4187 or by working directly with your S-1.

Anything more significant goes to the Army Board for Correction of Military Records on DD Form 149 (Application for Correction of Military Record). The ABCMR is the Army’s highest-level appellate review authority for records, so you have to exhaust other administrative correction procedures first. The application must identify the specific document or entry, explain why it’s wrong, and include supporting evidence.12Department of Defense. Federal law requires you to file within three years of discovering the error.
