How to Fill Out and Submit DA Form 2407: Required Blocks and Acceptance

To fill out DA Form 2407, complete Section I as the requesting organization: enter your Unit Identification Code and contact information, identify the equipment with the correct ID code and stock number, record the current meter reading, and write a specific fault description in Block 24. Leave Section II blank for the support maintenance activity. Then deliver all copies of the form with the equipment, its log book, and DD Form 314 to the support shop, and keep Copy 1 as your receipt.

Download the current version of the form from the Army Publishing Directorate at armypubs.army.mil. Keep DA Pam 738-750 open while you work, because several blocks require codes from its tables rather than free text. If the fault description or parts list runs long, you’ll also need DA Form 2407-1, the Maintenance Request Continuation Sheet, which doubles as the place to record any parts the support activity has to order.1Integrated Publishing. Mandatory Blocks for Inoperable Equipment

When the equipment is inoperable, these blocks must be filled in: 1, 5, 6, 7, 10a, 10b, 11, 12, 13, 15, 16, 20, and 24.2TPUB. DA Form 2407 Completion Section I is your responsibility. Section II belongs to the support maintenance activity, so leave it blank. Section III covers equipment identification and is partially yours.

Unit and Contact Information

Block 1a takes the Unit Identification Code of the organization that owns the equipment. That six-character alphanumeric code tells the shop who to contact and who gets the equipment back. Block 1b is the plain-language unit name. Block 1c is the unit phone number. If the equipment is in transit, use Block 2a for the SAMS UIC or TDA unit information.2TPUB. DA Form 2407 Completion

Equipment Identification

Block 5 is the Type Maintenance Request Code, found in Appendix B of the maintenance management update referenced in DA Pam 738-750.

Blocks 6 and 7 work together, and this is where forms most often go wrong. Block 6 is the ID Code that tells the shop what kind of number you’re entering in Block 7:

  • A — National or NATO Stock Number
  • C — manufacturer’s code and part number
  • D — Management Control Number
  • P — other numbers

Block 7 then takes the actual number that matches the code you picked. Block 8 is the model number. Block 9 is the noun nomenclature of the item.2TPUB. DA Form 2407 Completion

Block 11 is the current meter reading: miles, hours, rounds, or whatever usage metric the equipment uses. Pull it directly off the equipment. Estimating throws off maintenance projections for the fleet and raises questions about the rest of the form.

Writing Block 24

Block 24 is the fault description, and vague entries are the fastest way to slow a repair. Pull the symptoms from column “c” of DA Form 2404, the Equipment Inspection and Maintenance Worksheet, and write a brief, specific description of what’s wrong.3TPUB. “Vehicle won’t start” gives the technician nothing. Better: what the operator experienced, when it started, whether it’s intermittent or constant, and any relevant conditions such as temperature, terrain, or load. The clearer the description, the faster diagnosis begins.

What to Bring With the Form

The form alone won’t get the equipment accepted. When you deliver it to the support shop, bring:

  • All copies of the completed DA Form 2407
  • The equipment log book
  • DD Form 314 (Preventive Maintenance Schedule and Record), if applicable

Maintenance personnel will run an acceptance inspection to verify that the items are complete, the form is properly filled out, all organizational-level maintenance has already been done, and the log book is accurate and current.4Fort Jackson. FJ REG 750-15 Maintenance Services and Equipment

Complete whatever maintenance your unit is authorized to perform before you request support. Showing up with a dirty vehicle, missing documents, or a fault your own mechanics could have fixed will get you turned away.

What Happens at Acceptance

When the shop accepts the equipment, maintenance personnel sign and date Block 35a and direct your unit representative to deliver the form and log book to the customer service center.4Fort Jackson. Copy 1 comes back to you as a receipt. File it. That is your proof the equipment left your custody and your ticket to reclaim it when repairs finish. If Copy 1 is lost, your unit commander will have to issue a Disposition Form stating the original receipt was lost before the shop will release the equipment.5Defense Technical Information Center. Copy 2 stays at the maintenance shop and is the only copy annotated with labor-hour requirements during the repair. The remaining copies are used internally by the support activity.3TPUB. Mistakes That Get Forms Rejected

Most rejections come from the same handful of errors. Check for these before you leave the motor pool:

  • Entering the NSN in Block 7 without the matching ID Code in Block 6
  • A vague Block 24 that doesn’t give the technician anything to act on
  • Skipping organizational-level maintenance your own unit was authorized to perform
  • Arriving without the log book or DD Form 314
  • An inaccurate meter reading in Block 11 that doesn’t match the equipment

Take ten extra minutes to check every mandatory block against the code tables in DA Pam 738-750 before you transport the equipment. A form that needs no corrections is the fastest way through the system.
