Every AXA claim needs the basics:
- Your policy number, printed on your policy documents or member card. Without it, the system cannot pull your coverage limits or deductible.
- Full legal name, current address, phone number, and email. An outdated contact leaves your claim idle.

Fill Out the Form
AXA claim forms share a common structure across insurance lines: personal information at the top, incident details in the middle, an expense summary toward the bottom, and a signed declaration at the end. A few habits apply regardless of which form you have.
In the incident description section (often labeled “Reason for Claim” or “Details of Incident”), stick to facts. Write what happened, when, and where. Do not speculate about fault or editorialize. What Happens After You Submit
AXA Global Healthcare pays about 83% of eligible online claims within two working days.3AXA Global Healthcare. A missing treatment date on an invoice or a blank field on the form triggers a request for more information before the clock starts.
Travel and commercial claims take longer, especially when liability is disputed or multiple parties are involved. State insurance laws generally require insurers to accept or deny a claim within 30 to 60 days after receiving all proof of loss, with the exact deadline varying by state. If the insurer needs more time, most states require written notice explaining why.
You should receive confirmation with a claim reference number soon after submission. Use that number in every phone call, email, and letter from then on. It is how the adjuster locates your file. If you do not receive an acknowledgment within two weeks, follow up. A missing acknowledgment sometimes means the submission did not go through.
Adding Expenses After You File
Damage you did not spot at first, or medical bills that arrive weeks after treatment, can be added through a supplemental filing. You do not need to start over.
