To fill out DA Form 2407, complete Section I (Customer Data) with your unit identification, equipment data, and a clear description of the fault, then turn the form in with the equipment, its log book, and supporting documents to the maintenance activity that will do the work. The requesting unit only fills Section I; the support activity completes Section II after it accepts the equipment. If your unit runs GCSS-Army, you’ll use the electronic DA Form 5988-E for day-to-day faults and reach for the paper 2407 mainly when equipment leaves your unit for a higher-echelon repair facility or when the digital system is unavailable.1Tpub. Mandatory Blocks in Section I
When equipment is inoperable (not mission capable), the following blocks are mandatory: 1, 5, 6, 7, 10a, 10b, 11, 12, 13, 15, 16, 20, and 24.3Tpub. DA Form 2407 Completion Skipping any of them is one of the fastest ways to have your form kicked back at acceptance.
Unit Identification (Blocks 1 and 2)
- Block 1a (UIC Customer): the Unit Identification Code of the organization that owns the equipment.
- Block 1b (Customer Unit Name): the full unit name that matches the UIC in 1a.
- Block 1c (Phone Number): the unit’s contact phone number.
- Block 2a (SAMS UIC/TDA): if the equipment is in transit, the UIC for the SAMS2 or SAMS1/TDA unit.
- Block 2b (Utilization Code): the appropriate utilization code from DA PAM 750-8, Appendix B.
- Block 2c (MCSR Item): enter “Y” if the item is reportable under AR 700-138; leave blank if it is not.3Tpub. DA Form 2407 Completion
Equipment Data (Blocks 5 Through 9)
- Block 5 (Type Maintenance Request Code): the code identifying the type of work requested, drawn from DA PAM 750-8, Appendix B, Table B-20.
- Block 6 (ID Code): the letter identifying what kind of number goes in Block 7. Use “A” for National/NATO Stock Number, “C” for manufacturer’s code and part number, “D” for Management Control Number, or “P” for other numbers.
- Block 7 (NSN): the National Stock Number or other identifying number matching the code in Block 6. The NSN is the standard label applied to any item repeatedly procured and issued through the federal supply system.4Defense Logistics Agency. National Stock Number Brochure
- Block 8 (Model): the equipment’s model number.
- Block 9 (Noun): the noun nomenclature of the item, such as “TRUCK, CARGO” or “GENERATOR SET.”3Tpub. DA Form 2407 Completion
Fault Description (Block 24)
Block 24 is where most 2407s succeed or fail as a work order. Describe every deficiency or symptom of the item to be repaired. Skip vague language like “doesn’t work.” Write down what you actually observed: where a fluid leak is coming from, what noise the engine makes and when it makes it, specific error codes on a display, or the conditions under which the fault appears. Documents and Equipment You Bring With the Form
A completed 2407 is only part of what the maintenance activity expects. The support unit conducts an acceptance inspection before it takes custody, and it checks the paperwork and the equipment together.5United States Army. FJ REG 750-15 – Maintenance Services and Equipment
Bring these with the equipment:
- The completed DA Form 2407, with all mandatory blocks filled for the equipment’s status.
- The equipment log book, accurate and current.
- DD Form 314, if the equipment category requires it.
- DA Form 2408-14 (Uncorrected Fault Record), showing organizational-level deficiencies and corrective action already taken, including any parts on order with document numbers.
The equipment itself has to meet minimum turn-in standards. Vehicles must be free of oil and grease deposits, mud, trash, and dirt, and have at least a three-quarter tank of fuel. Components submitted for repair must be clean, with machined surfaces and openings protected by caps, plugs, or dust covers. Battery-powered communication and electronic gear must have batteries removed before turn-in unless the repair facility approves otherwise.5United States Army. All organizational-level maintenance must be current. If your unit skipped scheduled services or left correctable faults unaddressed, the shop will refuse the turn-in and send you back to finish that work.
Turning the Form In
Deliver the completed 2407, the log book, and the accompanying documents along with the equipment to the maintenance shop or unit Shop Office. Maintenance personnel run the acceptance inspection on the spot. If the paperwork checks out and the equipment meets turn-in standards, the shop accepts the item, assigns it a local work order number, fills in its portion of the form, and adds it to the active workload.1Tpub. In GCSS-Army units, the paper equivalent starts with operators recording faults on DA Form 5988-E during PMCS. The operator turns the worksheet in to the maintenance supervisor, who verifies the reported faults and assigns a mechanic. A GCSS-Army clerk enters the fault data into the system, which generates the formal work order and creates parts requests for any needed components automatically.2U.S. Army Logistics, G-4. Why Work Orders Get Rejected
Shops turn equipment away more often than most operators expect. The common reasons are worth knowing before you make the trip:
- Incomplete organizational maintenance. On-post units get rejected outright; off-post units get notified through command channels.
- Missing or incomplete documentation. Without the log book, DD Form 314 where applicable, and DA Form 2408-14, the shop cannot verify the equipment’s history.
- Damaged equipment without authorization. Damage beyond fair wear and tear requires either a request for Estimated Cost of Damage (ECOD) or a memorandum stating the equipment is no longer needed for an authorized adjustment action.
- Dirty or improperly prepared equipment. Mud, grease, trash, or less than three-quarters of a fuel tank sends the equipment back to the unit.
- Fabrication requests without approval. Any 2407 submitted for metal, wood, or fabric fabrication needs a supervisor’s or commander’s signature and prior written approval from the Director of Logistics and Engineering.5United States Army. FJ REG 750-15 – Maintenance Services and Equipment
After the Shop Accepts It
Once the equipment is in the shop’s hands, the local work order number is your reference for every status check. Repairs at the field level cover fault diagnosis, parts replacement, battle damage assessment and repair, lubrication, and authorized modification work orders performed on or near the system by the unit or its direct support element.6The United States Army. If the shop needs parts, the associated requisitions carry a priority designator that governs how quickly the parts arrive. Category 1 requisitions (priority 01–03) have a CONUS delivery goal of 4 days, Category 2 requisitions (04–15 with expedited delivery dates) 7 days, and Category 3 requisitions (04–15 routine) 14 days.8United States Marine Corps.
