How to Fill Out and Submit AF Form 56: Pages, Interview, Package

To fill out AF Form 56, download the current version from the Air Force e-Publishing site, complete Sections I and II yourself across pages 1 through 4 with your personal data, service history, education, and required certifications, then route the form to your squadron commander or equivalent, who completes Section III on page 5 after conducting a scored interview. What to Have Ready Before You Open the Form

Several blocks on AF Form 56 pull directly from documents you should already have in hand. Trying to fill the form out first and chase paperwork later is how applicants miss cutoffs.

Confirm eligibility first. Non-rated applicants cannot be older than 42 at commissioning; rated candidates for pilot, combat systems officer, air battle manager, and helicopter slots must not be past their 33rd birthday as of the projected board release date; and remotely piloted aircraft applicants must graduate OTS before turning 40. Prior enlisted service is subtracted from your age for eligibility purposes. You need a bachelor’s degree from an accredited institution with a cumulative GPA of at least 2.5 (technical degree holders applying for technical or Critical Accessions Degree boards are exempt from that floor), a minimum AFOQT Verbal composite of 15 and Quantitative composite of 10, U.S. citizenship, a current passing PT test with no component waivers, and a clean or waivered moral record.1Air Force Accessions Center. Gather these before starting the form itself:

  • Official transcripts showing degree conferral. Transcripts marked “Issued to Student” or naming the applicant as the intended recipient are automatically rejected.
  • Your AFOQT score printout showing all composites, plus a PCSM printout if applying for a rated slot.
  • Your vMPF Career Data Verification Brief and a Record Review Update dated within 30 days of the application cutoff.
  • An AF Form 422 reflecting your current medical status, valid within 12 months of the issue date.
  • An AF Form 4428 if you do not meet current tattoo policy standards.
  • The commander’s Master PIF/Local PIF Quality Force Review letter and the AFPC/DP1OR response. Both are mandatory; the submission system blocks you if either is missing.
  • Two letters of recommendation: a Personal Character LOR (one page maximum) and an Endorsement LOR from your group commander endorsed by the wing commander or equivalent (two pages maximum). Both need contact information, a signature, and a date within 12 months of the board convene date.
  • Your four most recent EPRs, front and back, in chronological order with the newest on top. If you lack enough EPRs, a signed AF Form 77 from your squadron commander substitutes for each missing report.

Working Through Pages 1 Through 4

Page 1: Identifying Information and Program Selection

Page 1 captures your name, address, home of record, date of birth, Social Security Number, number of dependents, and the date you are available for training. It also asks for your servicing MPF address, duty information, Control AFSC, and PAS code, which you can pull from your EPR or vMPF. Block 7 is where you designate OTS as the program, list your preferred career fields in order, and indicate whether you volunteer for flying duty.

Page 2: Prior Applications, Service, and Law Violations

Page 2 covers previous OTS applications, prior service in other branches, any previous commissions, and law violations. Block 15 is the one that catches people. Any incident beyond a minor non-traffic offense must be explained on Page 7 and may need corroborating documentation such as court records or Article 15 paperwork routed through a separate waiver packet. If you have nothing to report, mark the blocks accordingly and leave the conditional fields blank rather than writing “N/A” everywhere.

Page 3: Employment, Education, and Certifications

Page 3 asks for employment history and education. Even career active-duty applicants complete this: list the Air Force as your employer, use your current duty title, and pull your net annual salary from your Leave and Earnings Statement. Block 20 is your education record. List every degree with precise dates and institution names exactly as they appear on your transcripts, because the board will cross-reference the two. Block 21 is a series of certification statements referencing specific regulatory provisions. Read each one before initialing, and initial every sub-block that applies to you.

Page 4: Final Certifications and Signature

Page 4 contains additional certifications. Most blocks are marked “N/A” by active-duty enlisted applicants. Sign and date at the bottom using the date you actually plan to submit, not a projected future date.

Page 5: Supervisor Validation and Commander Entries

Block 23 on Page 5 records your degree details, including institution, degree type, and GPA. Block 25 is the supervisor validation, where your supervisor, first sergeant, or commander confirms that everything in Sections I and II is accurate and signs. Typed signatures are not accepted; you need a wet or digital signature.3Air Force Recruiting Service. Blocks 27 and 28A through C are completed by your commander and cover waiver requirements, assignment limitations, and the PIF review results.

The Commander Interview and Block 28K

Section III is not a rubber stamp. Your squadron commander or equivalent conducts a formal interview, reviews the entire application, and scores you on a sheet with marks from 6 to 10 on each question.   • Aptitude, covering academic discipline, GPA, awards, and AFOQT scores.
  • Leadership, describing style and scope of leadership experience drawn from your application and EPRs.
  • Potential, addressing communication skills, adaptability, likelihood of succeeding as an officer, and any red flags such as prior non-selections or law violations.
  • A push line summarizing the recommendation, which may include a stratification.

Extra bullets or multi-line bullets can get the entire package rejected. Prepare for the interview by reviewing your own records so you can articulate why you want a commission and what you offer the officer corps. A weak interview will sink an otherwise competitive package.

Submitting the Completed Package

The completed AF Form 56 travels inside a multi-packet submission: a Board Application Packet (your five-page AF Line Officer Application, AFOQT printout, PCSM if applicable, the two LORs, and the commander interview score sheet), a Source Documents Packet (eligibility determination, transcripts, vMPF products, PIF review documents, AF Form 422, and AF Form 4428 if applicable), an EPR Packet, and, when relevant, separate Waiver and Prior Service packets.1Air Force Accessions Center. Active-duty applicants submit electronically through the Air Force Civilian Employment Plan portal. The package must reach AFCEP by the application cutoff date on the board schedule. There is no grace period. Reserve and Guard applicants route their packages differently, through their detachment commanders and ARPC Education via myFSS.4Air Force Reserve. Mistakes That Get Packages Rejected Before the Board

Screeners cut packages before they reach a selection board, and rejections at this stage are not fixable for that cycle:

  • Missing the commander’s PIF review memo or the AFPC/DP1OR response. The submission system will not accept the package with only one of the two.
  • Transcripts stamped “Issued to Student” or naming the applicant as the intended recipient. Order them in a sealed or official electronic format that does not carry those markings.
  • An Endorsement LOR from below the wing commander level.
  • Any waiver or exemption on any component of the fitness assessment, which renders you ineligible outright.
  • Formatting problems: exceeding page limits, extra or multi-line bullets in Block 28K, or packets submitted out of order.

Applicants who are not selected or who withdraw cannot reapply for 180 days from the date of non-selection or withdrawal, so a package killed by a fixable error still costs you six months.1Air Force Accessions Center. Active Duty Air Force OTS Program Announcement
