How to Fill Out and Submit AF Form 24: Sections, Waivers, and Submission

AF Form 24, the Application for Appointment as Reserve of the Air Force or USAF Without Component, is the paperwork the Department of the Air Force uses to decide whether you qualify for an officer commission.1Department of the Air Force. AF Form 24 – Application for Appointment as Reserve of the Air Force or USAF Without Component To fill it out correctly, download the current version from Air Force e-Publishing, complete every block against your official records, disclose every legal and employment item the form asks about, and submit the finished package through the recruiter or unit point of contact for your commissioning path. The form runs four pages, and most returned packages fail on the same handful of blocks. Work through it in order, with your documents in front of you.

Get the Current Form

The authoritative copy lives at e-publishing.af.mil. Search “AF Form 24” and download the fillable PDF. Save it to your computer before you begin. Browser-based PDF viewers sometimes strip the fillable fields, and older copies floating around unit websites may not match the current edition.

You’ll use AF Form 24 whether you’re applying through Officer Training School, the Health Professions Scholarship Program, a Judge Advocate General application, an Air National Guard board, or an Air Force Reserve vacancy. Your recruiter will tell you whether it’s the primary application or a supporting document alongside another form such as AF Form 56.

Page One: Appointment Type and Personal Data

At the top of page one, check the box for the appointment you’re seeking — Reserve of the Air Force, or USAF without component. Your recruiter tells you which.

Block 1 wants the office symbol of the requesting organization. JAG applicants, for example, enter “AF/JAR.”2U.S. Air Force. Guide for Completing Air Force Form 24 Block 2 is the career field. Both come from your recruiter.

Blocks 3 through 12 are personal data: full legal name, Social Security number, date and place of birth, current address, marital status, and number of dependents. Block 12 asks whether you’re a U.S. citizen; if you were naturalized, you’ll enter your certificate number and the court that granted citizenship.1Department of the Air Force. Your name has to match your government-issued ID exactly. A missing middle name or an initial where the full name belongs is one of the most common reasons packages come back. If you changed your name through marriage or court order, have the documentation ready.

Education History

Block 14 asks for a chronological list of every post-secondary institution you attended, whether or not you earned a degree. For each school, enter the type of institution, the dates you attended, years completed, degree type, and major.1Department of the Air Force. If you attended more schools than fit in the space, continue on page four.

Federal law requires a bachelor’s degree for nearly all officer career fields, and certain health and legal positions require a graduate or professional degree. Military Service History

Block 17 asks for a chronological statement of any service in any branch or component, including Reserve and National Guard time. For each period, list your highest grade held, the organization, active or reserve status, dates, and specialty. Block 19 asks whether every discharge was honorable. Civilian Employment

Block 25 wants a chronological record of your civilian employment, including part-time jobs. For each position, list the start and end dates, the employer’s name and full mailing address with ZIP+4, and your position and duties.1Department of the Air Force. Legal Disclosures

Block 26 is where applicants most often break their own packages. It asks whether you have ever been involved in, arrested for, indicted for, or convicted of any violation of civil or military law, including nonjudicial punishment under Article 15 of the UCMJ and minor traffic violations.1Department of the Air Force. Dismissed charges still count. Expunged records still count for the purposes of this question.

Disclose everything. Explain what happened, when, and how it was resolved. A background investigation will surface what you tried to leave out, and deliberately hiding a disqualifying fact to obtain an appointment is a criminal offense under 10 U.S.C. § 904a, Article 104a of the UCMJ, punishable by court-martial.4Office of the Law Revision Counsel. 10 USC 904a – Art. 104a. Air Force Specialty Code

You’ll identify the career field you’re applying for as an Air Force Specialty Code. The AFSC system organizes officer careers into numbered groups: 1XXX for operations (pilots at 11X, intelligence at 14N), 2XXX for logistics, 3XXX for support, 4XXX for medical (including the 46XX nursing series), 5XXX for legal and chaplain roles, and 6XXX for acquisition and financial management.5United States Air Force. Age and Waiver Limits Worth Checking Before You Submit

Age caps depend on the track. OTS non-rated applicants cannot be older than 42 at commissioning. Remote Piloted Aircraft applicants top out at 40. Prior-service applicants over the limit may qualify if subtracting military service time brings them under, though that adjustment doesn’t apply to pilot, combat systems officer, air battle manager, or helicopter pilot slots. Healthcare professionals applying through separate programs may commission up to age 48.6Air Force Recruiting Service. Active Duty Air Force Officer Training School Program Announcement If you need an age waiver, moral waiver, or medical waiver, that goes into a separate waiver packet with the request memorandum, commander’s endorsement, and any supporting court or medical records.

Supporting Documents That Have to Match the Form

AF Form 24 by itself is not a complete package. The list varies by commissioning source, but expect most of the following:

  • Official transcripts, sent from each registrar and showing degree conferral. If you haven’t graduated yet, some programs accept current transcripts plus an official letter confirming your expected graduation date.
  • Proof of citizenship — a certified birth certificate or naturalization certificate.
  • DD Form 214 for prior-service applicants, documenting your separation date, character of service, and reenlistment eligibility code. Include a DD Form 215 or NGB Form 22 if you have one.6Air Force Recruiting Service. Active Duty Air Force Officer Training School Program Announcement

  • Letters of recommendation. OTS applicants need two: a personal character letter from someone who knows you personally (not immediate family), limited to one page; and an endorsement letter signed by a group commander or equivalent and endorsed by a senior rater, capped at two pages. Both must be dated within 12 months of the board convene date and addressed to the specific selection board.6Air Force Recruiting Service.   • Professional licenses or certifications for health professions and legal applicants. JAG applicants must address bar examination history and any board certifications.2U.S. Air Force. Guide for Completing Air Force Form 24

Cross-check the documents against the form before you hand anything off. If your transcript shows a different degree date than block 14, or your DD 214 shows a job you left off block 25, the mismatch stalls the review or ends it.

Mistakes That Get Packages Returned

Recruiters and selection boards see the same errors on repeat. Getting past them puts you ahead of a surprising share of applicants.

  • Incomplete legal disclosures. Leaving out a dismissed charge or a traffic ticket because you assumed it “didn’t count.” Block 26 asks about every contact with the legal system, not just convictions.
  • Name mismatches across AF Form 24, your government ID, your transcripts, and any clearance paperwork.
  • Stale letters of recommendation. Anything dated more than 12 months before the board convene date is invalid and the package comes back.6Air Force Recruiting Service. Active Duty Air Force Officer Training School Program Announcement
  • Unofficial transcripts. Boards require the registrar’s official copy. A student portal printout or a PDF you downloaded from your own account won’t be accepted.
  • Missing continuation pages. If education, employment, or service history overflows, use page four. Leaving entries off because they didn’t fit is not an option.
  • Generic letters of recommendation. Endorsements must reference the specific board (for example, “Officer Selection Board 25OTS03”). Boilerplate addressing reads as a recycled letter.

Where and When to Submit

Once the form, supporting documents, and any additional required paperwork are assembled, you deliver the package to an Officer Accessions recruiter, a Health Professions recruiter, or the unit’s designated point of contact for Guard and Reserve applicants. That recruiter or POC reviews the package for completeness and forwards it to the Air Force Recruiting Service or the applicable selection board.

Selection boards meet multiple times a year. The FY25 schedule, for example, held six OTS boards spanning rated, non-rated, and whole-person concept slots, along with rolling boards for specific programs. You’ll get the result through your recruiter or unit. A “Select” comes with instructions for a training date. A “Non-Select” is not permanent, but you cannot reapply within 180 days of the non-selection date. The only exception is when a new fiscal year opens a fresh board cycle for a different board type. Requests to shorten the 180-day wait are granted only in rare cases where a commander can document substantial improvement, such as a meaningful jump in AFOQT scores, additional flying hours, a higher degree, or a significantly higher GPA.6Air Force Recruiting Service.
