How to Fill Out and Submit a Prudential Annuity Withdrawal Form

To withdraw money from a Prudential annuity, you complete the Annuity Automated Withdrawal Form (or the product-specific version your contract requires), provide your contract number and banking details, choose your withdrawal amount and tax withholding, and submit the signed form through Prudential’s online portal, by fax, or by mail. You can download the Prudential annuity withdrawal form from prudential.com/links/forms, or call 888-778-2888 for annuity accounts to have one sent to you.

Make Sure You Have the Right Form

Prudential uses different withdrawal forms for different annuity situations, and submitting the wrong one is one of the fastest ways to delay your money.

The Annuity Automated Withdrawal Form covers most routine withdrawals from an individual annuity contract. If you hold a 403(b) tax-sheltered annuity, though, that automated form handles only required minimum distributions — every other type of 403(b) withdrawal requires the separate 403(b) Withdrawal Form.1Prudential. Annuity Automated Withdrawal Form If you need substantially equal periodic payments under a 72(t) arrangement, use the Annuity Substantially Equal Periodic Payments Form instead.

What to Have Ready Before You Start

Pulling these items together before you open the form saves a round trip:

  • Your Social Security Number (or EIN for a trust or estate) and your Prudential contract number.2Prudential. Prudential Withdrawal Request – Group Universal Life
  • Your bank’s nine-digit routing number and your account number if you want direct deposit. A single transposed digit sends the request back to the start.
  • The withdrawal amount or percentage, or a note that you want Prudential to calculate your RMD for you.
  • Your contract’s surrender schedule, so you know whether a charge applies.
  • A current phone number and email address Prudential can reach you at if something on the form needs clarification.

Filling Out the Form Section by Section

Owner and Contract Information

The opening section asks for your name, contract number, contact phone numbers, and email address.1Prudential. Annuity Automated Withdrawal Form Enter these exactly as they appear on your most recent Prudential statement. A name that doesn’t match the contract, or a transposed digit in the contract number, is the most common reason a form comes back.

Withdrawal Amount

Specify a dollar amount, a percentage, or a full surrender. If you’re taking a required minimum distribution, Prudential can calculate the amount for you based on your contract value and age; you don’t need to run the math yourself. For a partial withdrawal, enter the exact dollar figure. For a complete liquidation, use the full-surrender or total-distribution checkbox.

Payment Method

You can take payment as a paper check mailed to your address on file or as an electronic funds transfer to your bank. EFT is faster by several days and removes the risk of a lost check. If you choose EFT, the form asks for your bank’s routing number, your account number, and whether the account is checking or savings.2Prudential. Prudential Withdrawal Request – Group Universal Life

Federal and State Tax Withholding

The form lets you set your federal withholding election. For any eligible rollover distribution paid directly to you as cash, Prudential must withhold 20% for federal income tax, and you cannot opt out of that 20% or reduce it.3Internal Revenue Service. Pensions and Annuity Withholding You can elect a higher percentage if you expect to owe more at tax time.

The 20% mandatory withholding does not apply if you choose a direct rollover, meaning Prudential sends the money straight to another qualified plan or IRA without it passing through your hands.4Internal Revenue Service. Topic No. 413, Rollovers From Retirement Plans If you intend to roll the funds over, elect the direct rollover on the form; otherwise Prudential withholds 20% and you have to replace that 20% from your own pocket within 60 days or it becomes a taxable distribution.5Internal Revenue Service. Rollovers of Retirement Plan and IRA Distributions

Many states also withhold on retirement distributions. Some require it on eligible rollover distributions, others let you opt out, and states with no income tax (Texas, Florida, Nevada and others) have nothing to withhold. If your state requires withholding, Prudential applies the minimum rate unless you specify a higher amount.

Signature

Sign and date the form. If your contract has joint owners, both signatures are typically required. Unsigned forms do not get processed.

Surrender Charges to Check Before You Submit

If your annuity is still inside its surrender charge period, Prudential deducts a contingent deferred sales charge (CDSC) from the withdrawal. The percentage is highest in the early contract years and steps down over time. The FlexGuard indexed variable annuity, for example, applies a CDSC that starts at 8% in the first two contract years and steps down to 4% in year six before disappearing in year seven.6Prudential Financial. FlexGuard Indexed Variable Annuity

Most Prudential annuity contracts let you withdraw up to 10% of total purchase payments each contract year without a surrender charge, and RMDs that Prudential calculates are also exempt.6Prudential Financial. FlexGuard Indexed Variable Annuity If you’re close to the end of your surrender period, pulling your contract’s issue date and the surrender schedule before you submit can save real money. Waiting a few months is sometimes the difference between a 4% charge and no charge at all.

Separately, if you’re under age 59½, the IRS adds a 10% early-distribution tax on top of regular income tax, with several exceptions (disability, substantially equal periodic payments, qualifying medical expenses, and others).7Internal Revenue Service. Retirement Topics – Exceptions to Tax on Early Distributions That penalty is separate from any surrender charge Prudential applies.

How to Submit the Completed Form

You have three ways to send the signed form back to Prudential:

  • Upload it through the Prudential portal at prudential.com/login. Log in, go to the documents or transfers section, and upload the signed PDF. The system issues a confirmation number once the upload succeeds.
  • Fax it. The fax number is printed on the form itself or available when you call the annuity service line at 888-778-2888.
  • Mail it. The mailing address is on the last page of the form. Use that address rather than a general Prudential corporate address, because different products route to different processing centers.

Keep a copy of the completed form and your confirmation number. If anything goes sideways, that reference is what gets the conversation moving.

Processing Time and When the Money Arrives

After Prudential receives your form, processing typically takes one to three business days. That window covers identity verification, confirmation that the distribution type is allowed under your contract, and any coordination the plan requires.8Prudential. Delivery runs on top of that:

Some annuity products quote a combined window of three to five business days from receipt to payment.1Prudential. Annuity Automated Withdrawal Form Incomplete forms or missing signatures add days or weeks. You can track status through the transaction history in your online Prudential account.

Prudential will send you a Form 1099-R the following January reporting the gross distribution, the taxable portion, and the amounts withheld, and the IRS gets a copy as well.9Internal Revenue Service.
