A banker will review your original documents, verify your identity, and update the account records. If you hold a brokerage account through J.P. Morgan Securities LLC, Chase provides a dedicated PDF called “Account Holder Name Change Request.” It is specifically for brokerage accounts and cannot be used for any other account type.
What the Form Asks For
The form asks for your account number, your Social Security number or Tax ID, and a certification under penalty of perjury that the taxpayer identification number is correct.3Chase. Attach certified copies of the legal documentation. Credit Cards
Chase credit card name changes require the same document trail: certified legal proof and an updated photo ID. Joint Accounts and Power of Attorney
Chase’s public materials don’t spell out whether both owners of a joint account must appear at the branch, or whether only the person changing their name needs to. Call ahead and ask, since requirements can depend on the specific account agreement. Bringing both account holders avoids a wasted trip.
For accounts managed under a power of attorney, Chase reviews the POA to confirm the agent’s authority before approving any changes, and requires that the POA specifically grant authority for the requested action rather than assuming broad powers from a general POA.6Chase. After Chase Updates Your Name
Cards and Checks
Existing debit and credit cards linked to the account will need to be replaced. Replacement debit cards from Chase arrive within 5 to 7 business days.7Chase. Direct Deposits and Automatic Payments
Changing your name at Chase does not notify your employer, the IRS, or any company billing your account. Update your direct deposit information at work and let recurring billers know. Most automatic debits run on account number rather than name, so payments usually keep clearing, but the mismatch can cause confusion during disputes or when setting up new payment relationships.
Watch for a Name-TIN Mismatch on Tax Forms
Keeping your name aligned across SSA, the IRS, and Chase matters at tax time. When a payer submits a 1099 with a name and taxpayer identification number that don’t match IRS records, the IRS can issue a “B Notice” requiring the payer to solicit a corrected TIN.
