The TransUnion credit report request form is the Annual Credit Report Request Form, a single-page document you mail to a centralized service in Atlanta to receive a free copy of your TransUnion file by return mail. Federal law entitles you to one free report from each nationwide bureau every 12 months, and this form is the paper route to claim it.1Office of the Law Revision Counsel. 15 USC 1681j – Charges for Certain Disclosures Free weekly online reports are also available at annualcreditreport.com, so the mail-in form makes the most sense when you want a physical copy or would rather not use the website.2Federal Trade Commission. Information to Gather Before You Start
Have all of the following ready. The form warns that omitting any of these can delay your request.3Federal Trade Commission. Annual Credit Report Request Form
- Your full legal name, including any suffix (Jr., Sr., III) exactly as it appears on your ID.
- Your full nine-digit Social Security number. This is the primary identifier the bureaus use to find your file.
- Your date of birth.
- Your current mailing address, which is where the report will be sent.
- Your previous address, but only if you have lived at your current address for less than two years.
The previous-address requirement is a fraud safeguard: it helps the processing center confirm you are the actual consumer and not someone rerouting a report elsewhere. If the information you provide does not match what the bureau has on file, the processing service may ask for additional documentation before releasing your report. A government-issued photo ID and a utility bill or bank statement showing your current address are the typical supporting documents. Sending copies of two IDs with your initial mailing can prevent a round-trip delay when you already suspect an address discrepancy.
How to Fill Out the Form
The form has personal-information fields across the top and a row of checkboxes at the bottom, one for each of the three nationwide bureaus. To request only your TransUnion report, check the TransUnion box and leave the others blank. Checking all three gets you reports from Equifax and Experian at the same time; each bureau sends its report separately.3Federal Trade Commission. Annual Credit Report Request Form
Use black or blue ink and print in clear block letters. The processing center runs forms through optical scanners, so legibility matters more here than for ordinary mail. Skip cursive. Double-check every digit of your Social Security number and date of birth. A smudged or ambiguous number is the most common reason a form gets pulled for manual review, which adds days to the process.
Photocopy the completed form before you seal the envelope. If the report never arrives, that copy is your proof of what you sent and when.
Where to Mail It
Fold the form (do not staple or tape it), put it in a standard #10 envelope, and mail it to:

Certified Mail Is Optional
Certified mail with a return receipt gives you a tracking number and a signed confirmation that the processing center received your envelope. As of January 2026, USPS charges $5.30 for certified mail service plus $4.40 for a physical return receipt, on top of $0.78 First-Class postage at the counter, for roughly $10.48 total. An electronic return receipt costs $2.82 instead of $4.40.6USPS. Notice 123 – Price List Certified mail is not required, but it creates a paper trail if you ever need to prove the bureau missed a deadline.
How Long It Takes and What Arrives
The report is mailed within 15 days of the processing service receiving your form, and the CFPB advises allowing two to three weeks total for delivery after that.4Consumer Financial Protection Bureau. How Long Does It Take to Get My Free Credit Report After I Order It So from the day you drop the envelope in the mailbox, budget four to five weeks: a few days for your letter to reach Atlanta, up to 15 days for processing, and another week or so for the report to travel back.
The report arrives as a multi-page document in a standard envelope. By law it must include everything in your file at the time of the request: account histories, balances, payment records, the names of every entity that pulled your report in the past year (two years for employment-related inquiries), and the sources of the information.7Office of the Law Revision Counsel. 15 USC 1681g – Disclosures to Consumers Credit scores are not included; those are sold separately.
If nothing arrives within five weeks, call the centralized service at 877-322-8228. Have your copy of the form and any certified mail receipts handy so the representative can trace the request.
When You Qualify for More Than One Report a Year
The one-per-year free report is the baseline, but the FCRA grants extra free disclosures in several situations:
- If a lender, insurer, or employer denies you based on your credit report, you can request a free copy from the bureau that supplied it within 60 days of the denial notice.8Office of the Law Revision Counsel. 15 USC 1681j – Charges for Certain Disclosures
- If you are unemployed and plan to apply for jobs within the next 60 days, you can certify that in writing and get a free report.8Office of the Law Revision Counsel. 15 USC 1681j – Charges for Certain Disclosures
- Recipients of public welfare assistance qualify for a free disclosure on the same written-certification basis.8Office of the Law Revision Counsel. 15 USC 1681j – Charges for Certain Disclosures
- If you believe your file contains inaccurate information because of fraud, you can request a free report by certifying that belief in writing.8Office of the Law Revision Counsel. 15 USC 1681j – Charges for Certain Disclosures
- Placing a fraud alert on your file also triggers a right to a free disclosure from each nationwide bureau.8Office of the Law Revision Counsel. 15 USC 1681j – Charges for Certain Disclosures
The adverse-action denial notice will name the bureau and how to contact it. For the other categories, you send a written request directly to TransUnion or use the centralized mail-in form with a brief statement certifying your qualifying circumstance. These additional free reports do not count against your one-per-year entitlement.
A security freeze on your TransUnion file does not block you from requesting your own free annual report. The freeze stops third-party access for new credit applications; your own request for your own file goes through a different process, and you do not need to lift the freeze before mailing in the form.9Annual Credit Report.com. Security Freeze Basics
The standard form is for adults requesting their own file. Requests involving minors or a deceased person go through TransUnion directly rather than the centralized service, with different documentation (a birth certificate for a child’s file, a death certificate mailed to TransUnion at P.O. Box 2000, Chester, PA 19016 for a deceased person).10TransUnion. Child Identity Theft11TransUnion. Get Your Credit Report by Mail or Phone
What to Do After Your Report Arrives
Read every section. Look for accounts you do not recognize, balances that seem wrong, late payments you believe were on time, and inquiries from companies you never contacted. Errors on credit reports are not rare, and catching them early prevents damage to your score and to future lending options.
If you find a mistake, dispute it in writing with both TransUnion and the company that reported the information (the furnisher). Your dispute letter to TransUnion should include your contact information, the specific error, an explanation of why it is wrong, and copies (not originals) of any supporting documents. Disputes can also be filed online at dispute.transunion.com.12Consumer Financial Protection Bureau. How Do I Dispute an Error on My Credit Report
The furnisher generally has 30 days to investigate after receiving your dispute. If the bureau decides a dispute is frivolous, because it does not identify a specific error, it can decline to investigate, but it must notify you of that decision within five business days.12Consumer Financial Protection Bureau. How Do I Dispute an Error on My Credit Report Be specific. Vague complaints along the lines of “this doesn’t look right” are the fastest way to get a dispute dismissed.
