PS Form 5436, the paper “Inquiry Regarding a Missing Item” that senders once mailed to the Mail Recovery Center, has been retired. The Postal Service moved the process online, and current USPS resources no longer reference the form by name. The same inquiry is now filed through the Missing Mail application at MissingMail.USPS.com, which collects the same categories of information the old paper form did: sender and recipient addresses, a tracking number, and a detailed description of the package and its contents. The online system is faster, accepts photo uploads, and sends email updates as the search moves forward.
When You Can File
A missing mail search request can be submitted starting seven days after the mailing date.1United States Postal Service. Missing Mail and Lost Packages Before that window opens, check USPS Tracking. Many packages that look lost are still in transit with a stalled scan, and giving tracking a few days to update often resolves the problem without a search.
There is no published outer deadline, but filing sooner improves your odds. The Mail Recovery Center holds items for limited periods that vary by mail class, so a request filed months later may come back empty because the package was already disposed of.
What to Gather Before You Start
Missing any of these details slows the search or prevents the system from matching your inquiry to a physical package.
- Sender and recipient information: full names, street addresses, apartment or suite numbers, and ZIP codes for both parties. The system cross-references these against undeliverable mail records, so a transposed digit in a ZIP code matters.
- Tracking number, if the shipment had one. This is the single most useful piece of data and can be found on your mailing receipt, Click-N-Ship label record, or shipping confirmation email.2United States Postal Service. Missing Mail – The Basics
- Mailing date: the day the item was dropped off or accepted at the counter.
- Container type and approximate size: flat-rate box, padded envelope, plain brown box, and rough dimensions such as “12 × 10 brown box” if you have them.2United States Postal Service. Missing Mail – The Basics
You do not need your mailing receipt to file, but having it speeds things up. If you shipped through Click-N-Ship or a third-party platform such as eBay or Etsy, pull the tracking number and mailing date from your account history there.
How to Submit the Search Request
Go to MissingMail.USPS.com and sign in to your USPS.com account, or create one if you do not already have one. The form walks you through the information above: addresses, tracking data, package description, and contents.1United States Postal Service. Missing Mail and Lost Packages
The form also asks for photos that could help identify the item. A picture of the product, a screenshot of the online listing you bought it from, or a photo of the box before you sealed it all help. Clerks at the Mail Recovery Center sort through thousands of packages, and a visual reference is often the fastest way they can match a found item to an open inquiry.1United States Postal Service. If you cannot use the online system, visit your local post office and ask a clerk to start the search for you. The clerk enters the same information into the Postal Service’s internal system.
Describing the Package and Contents
Vague descriptions like “clothing” or “electronics” are almost useless when a warehouse holds pallets of undeliverable mail. Be specific. Include brand names, model numbers, colors, and sizes for everything inside.2United States Postal Service. Missing Mail – The Basics “Blue Nike Air Max 90, men’s size 11” is the kind of detail that produces a match. “Shoes” is not.
Describe the outside of the package too: color of the box or envelope, stickers or labels you applied, handwritten markings, or printed logos. If you reused an Amazon box and taped over the old label, say so. These external details let clerks pull the right package off a shelf without opening every one.
When the contents are a gift or handmade item without a receipt, write a short description of what the item is and roughly what it would cost to replace. That estimated value does not trigger any fee; it helps the Mail Recovery Center decide which items to hold longer.
What Happens Next
USPS sends a confirmation email once the request is received, then follows up with periodic status updates.1United States Postal Service. Missing Mail and Lost Packages The outcome will be one of three things: the item is found and sent to the address you specified, the item is confirmed as already delivered (sometimes to the wrong address or scanned incorrectly), or the item cannot be located.
If the package is found, USPS forwards it to the recipient or returns it to the sender, depending on how much of the original addressing is intact and whether the postage allows forwarding. If the search turns up nothing, the case closes. You will not receive a bill or penalty for an unsuccessful search.
Also File an Insurance Claim When It Applies
A missing mail search and an insurance claim are two different processes. The search is a free locate-my-package inquiry. An insurance (indemnity) claim is a formal request for monetary reimbursement, available only when you purchased insurance or used a service that includes it, such as Priority Mail Express or Registered Mail.3United States Postal Service. Domestic Mail Manual 609 – Filing Indemnity Claims for Loss or Damage
You can file both. Start with the missing mail search to try to recover the actual package. If it comes back empty and you had insurance, file an indemnity claim for compensation. Filing windows vary by service:
- Priority Mail Express: no sooner than 7 days, no later than 60 days from mailing.
- Insured Mail, Registered Mail, and COD: no sooner than 15 days, no later than 60 days.
- APO/FPO/DPO Insured or Registered Mail: no sooner than 45 days, no later than one year.
Insurance claims require proof of value, such as a sales receipt, invoice, paid repair bill, or an online listing printout showing what the item cost.3United States Postal Service. Domestic Mail Manual 609 – Filing Indemnity Claims for Loss or Damage File online at usps.com/domestic-claims.
International Shipments Follow a Different Path
The process above applies only to domestic mail. International inquiries require coordination with the destination country’s postal service, and only the U.S. sender can start one. They are handled through a dedicated online form on USPS.com rather than the standard MissingMail portal.4United States Postal Service. File a USPS Claim: International
International inquiries are not accepted for ordinary letters or parcels sent without Registered Mail service. If you shipped using a trackable option like Priority Mail International or Priority Mail Express International, log in to your USPS.com account and look for the “Create an Inquiry” option under international claims. Expect longer resolution times than domestic searches, since the foreign postal administration must investigate on its end as well.
