How to File Form DS-64 for a Lost or Stolen Passport

To file Form DS-64 for a lost or stolen passport, use one of three routes: submit it online through the State Department’s Online Form Filler at pptform.state.gov, mail the completed paper form to the Consular Lost and Stolen Passport Unit in Sterling, Virginia, or hand it in at a passport acceptance facility together with a DS-11 application if you also want a replacement. The online route is fastest — the State Department cancels the passport within one business day and emails you a confirmation.¹ Filing the form itself is free.

You should file as soon as you realize the passport is missing, even if you have no immediate travel plans. Once the report is recorded, the passport is permanently invalid and cannot be reactivated, so nobody else can use it.²

Filing Online

Go to pptform.state.gov and use the Online Form Filler. You enter the DS-64 information directly on the site, submit electronically, and receive an email confirmation once the passport is canceled, usually within one business day.¹ Choose this route when reporting is all you need to do and you aren’t applying for a replacement at the same time.

Filing by Mail

Download the paper form from travel.state.gov, complete it, sign and date it, and mail it to:

Send the DS-64 by itself. Don’t include a DS-11 application or any other passport paperwork in the same envelope; if you want a replacement, use the in-person route below instead.³

Filing in Person With a Replacement Application

You can report the loss and apply for a new passport in a single visit. Bring your completed DS-64 along with Form DS-11 (Application for a U.S. Passport) and the supporting documents to any passport acceptance facility, regional passport agency, U.S. embassy, or consulate.³ Because your previous passport is gone, the replacement must go through DS-11 in person; the mail-in renewal form DS-82 is not available to you.⁴

The State Department notes that a separate DS-64 may not be required if you give enough detail about the loss on the DS-11 itself. If your description is incomplete, staff can pause the application and ask you to submit a DS-64 before moving forward.¹ Bringing both forms is the safer approach.

What You Need to Complete the Form

The DS-64 collects the biographical details the Department needs to match your report to the right passport record:

Your full legal name as it appeared on the lost passport. If your name has changed since issuance, provide both.³
Date and place of birth.
Social Security number.
Passport number, if you remember it. The book and card carry different numbers; include whichever you know. It isn’t mandatory but speeds processing.³
Which document was lost: book, card, or both.

The form also asks for a narrative of how the loss or theft happened. Give the date it occurred (or the last date you had the passport) and the location: city and state within the U.S., or city and country abroad.³ Be specific. “Left in a taxi in Chicago on March 5” is far more useful than “I think I lost it somewhere.”

Everything on the form is submitted under penalty of perjury, and knowingly providing false information to a federal agency is a felony.⁵

Filing for a Child

A parent or legal guardian files the DS-64 on a child’s behalf, using the same three routes above. The replacement step is where it gets more involved. For a child under 16, both parents or legal guardians must appear in person with the child at the acceptance facility to apply for a new passport on DS-11. If one parent cannot attend, the absent parent completes Form DS-3053 (Statement of Consent), signed and notarized; that consent is valid for 90 days from signing.⁶

When the second parent cannot be reached because of death, sole custody, or inability to locate, the applying parent submits supporting evidence: a death certificate, a court order granting sole legal custody, or Form DS-5525 with a written explanation.⁶ For children 16 and 17, only one parent’s awareness is required, though the acceptance agent has discretion to ask for written consent.

If Your Passport Is Lost or Stolen Abroad

Outside the United States, contact the nearest U.S. embassy or consulate right away. You’ll need to appear in person and apply for a replacement on Form DS-11. Bring a 2×2 inch passport photo, whatever identification you still have (a driver’s license, for example), proof of U.S. citizenship such as a birth certificate or a photocopy of the missing passport, and your travel itinerary.⁷

If some of those documents are gone too, consular staff will work with what you have. When citizenship can’t be proven from your documents, they can run a free file search of Department records to verify your identity. If your travel date is too close for a standard replacement, the consulate can issue an emergency passport valid for up to one year. Victims of serious crime who cannot afford the fee may qualify for a free emergency passport. Most posts can issue a replacement the next business day but generally cannot process passports on weekends or holidays; after-hours duty officers handle genuine life-or-death emergencies only.⁷

After the Report Is Processed

Once your DS-64 goes through, the passport is permanently canceled and its serial number is entered into the Consular Lost and Stolen Database, which the United States shares with INTERPOL for screening at borders worldwide.⁸ There is no way to reactivate the passport.

If you later find the passport you reported, do not try to travel with it. It will be flagged and can be confiscated at the border. Mail it in a sturdy envelope to the CLASP address in Sterling for physical cancellation.¹

Cost and Timing for the Replacement

The DS-64 report is free. The replacement passport costs the same as a first-time application because it goes through DS-11. Fees as of February 2026:

Adult passport book (16+): $130 application + $35 execution = $165
Adult passport card (16+): $30 application + $35 execution = $65
Adult book and card together: $160 application + $35 execution = $195
Minor passport book (under 16): $100 application + $35 execution = $135
Minor passport card (under 16): $15 application + $35 execution = $50

Add $60 for expedited processing, which takes two to three weeks before mailing time (mailing can add up to two more weeks). Routine processing runs four to six weeks before mailing. If you’re traveling within two weeks, book an appointment at a regional passport agency for same-day or next-day service, subject to limited availability.¹⁰
