How to Complete the Optum HSA Rollover or Transfer Request Form

The Optum HSA Rollover or Transfer Request Form moves money from another financial institution’s health savings account into your Optum Bank HSA, and it covers two different methods on a single document: a trustee-to-trustee transfer, where the funds move directly between custodians, and a rollover, where you deposit a check your old provider already sent you. Which section you complete, where you mail the form, and what tax paperwork follows all depend on that choice. You can download the form after logging into your Optum Bank account under Help & Tools > Forms & Documents, or get a copy from your employer’s benefits portal.

Transfer or Rollover: The Decision That Drives Everything

Before filling in a single field, decide which method applies to you. The two paths behave differently and carry different risks.

A trustee-to-trustee transfer is the direct route. Your current HSA administrator sends the money straight to Optum Bank and you never touch it. There’s no limit on how many you can do in a year, and the IRS doesn’t count the movement as a distribution, so nothing hits your tax return.1Internal Revenue Service. A rollover is what you do when your old HSA provider has already cut you a check. You then mail that check with this form to deposit it into your Optum HSA. Two rules bind the rollover method. First, you have 60 days from the date you received the distribution to complete the deposit; miss it and the IRS treats the full amount as a taxable distribution, potentially with a 20% additional tax. Second, you’re limited to one rollover per 12 months, measured from the date you received the prior distribution rather than the calendar year.2Office of the Law Revision Counsel. 26 USC 223 – Health Savings Accounts

Unless a check is already in your hands, use the transfer section. It’s cleaner, has no time pressure, and leaves nothing to report at tax time.

What to Have Ready Before You Start

Gather these details before you begin:

  • Your Optum Bank HSA account number, from your dashboard or welcome letter.
  • Your current HSA administrator’s full name and mailing address. The form’s “Instructions to current administrator” block needs this.3Optum. HSA Rollover or Transfer Request
  • Your account number at that current provider.
  • The physical check from your old provider, if you’re doing a rollover.

One more thing to confirm before you start writing: call your current HSA administrator and ask whether they’ll accept Optum’s form. Many banks require their own paperwork, and the form itself warns about this.3Optum. HSA Rollover or Transfer Request If they refuse Optum’s version, ask for their proprietary outgoing transfer form and use that.

Sell Investments Before the Transfer

If your current HSA holds mutual funds or other invested positions, those need to be sold and sitting in cash before the transfer can go through. Optum Bank doesn’t accept in-kind securities transfers because investment lineups vary between custodians.4Optum Bank. Transfer Your HSA Log into your current HSA, sell the holdings, and turn off any automatic investment feature so the cash doesn’t get reinvested while the transfer is pending. Fund settlement can take a few business days, so start this ahead of mailing the form.

Completing the Form Section by Section

Section 1: Participant Information

Enter your full legal name, Social Security number, date of birth, and contact information exactly as they appear in Optum’s records. If anything has changed since you opened the account, update it with Optum first. Mismatches between the form and your account profile can stall processing.

Section 2: Rollover Option

Complete this section only if you already have the check in hand from your previous administrator. Initial the acknowledgment line, which confirms you understand the 60-day deposit deadline and the one-rollover-per-year limit.3Optum. HSA Rollover or Transfer Request If you’re using this section, leave Section 3 blank.

Section 3: Transfer Option

Complete this section to move funds directly between custodians. Initial the acknowledgment line, which authorizes your current administrator to transfer all assets to Optum Bank and close the old account. The acknowledgment also authorizes liquidation of any invested funds in the account.3Optum. HSA Rollover or Transfer Request Fill in the “Instructions to current administrator” block with your existing provider’s name, mailing address, and your account number there. Leave Section 2 blank.

Section 4: Signature

Sign and date the form. Most versions require a handwritten signature; some fillable PDFs accept an electronic one. Date it the day you submit it. A stale date can get the form kicked back.

Where to Mail the Form

This is where the two paths split, and sending it to the wrong place can set you back weeks.

  • Rollover: mail the completed form together with the check from your previous provider to Optum Bank, P.O. Box 60099, Newark, NJ 07101.3Optum. HSA Rollover or Transfer Request
  • Transfer: send the form to your current HSA administrator, not to Optum. The form explicitly tells you not to mail it to Optum Financial for transfers. Your current provider initiates the outgoing transfer from their end.5Optum. Transfer Your HSA

Either way, use certified mail or a trackable shipping service. The form carries your Social Security number and account details, and delivery confirmation protects you if anything goes missing.

Fees, Timing, and Tracking

You won’t pay Optum anything to receive the funds. Your current provider, however, will likely charge an account closing or outgoing transfer fee, which comes out of your balance before the money moves.3Optum. HSA Rollover or Transfer Request Check their fee schedule beforehand so the deduction doesn’t catch you short.

Once Optum Bank receives a properly completed form, it processes the request within five to seven business days. Total end-to-end time depends on your current administrator; allow four to six weeks from submission for the money to show up in your Optum account.6Optum Bank. Optum HSA Rollover or Transfer Request Form Log in periodically to check for the deposit. If six weeks pass with no debit at the old account and no credit at Optum, call your current administrator first; that’s usually where the holdup is.

If you’re doing a rollover, watch the 60-day clock closely. Processing delays don’t stop it, and a check that doesn’t get deposited in time becomes a taxable distribution.

Tax Reporting After the Move

A trustee-to-trustee transfer leaves almost no paper trail. You don’t report the transferred amount as income, you don’t claim it as a new contribution, and you don’t list it as a distribution on Form 8889.1Internal Revenue Service. Publication 969 – Health Savings Accounts and Other Tax-Favored Health Plans The funds simply changed custodians.

A rollover generates more paperwork. Your previous provider issues Form 1099-SA for the distribution, and Optum issues Form 5498-SA showing the rollover contribution. You use both to complete Form 8889, which you file with your federal return. The form has a line for reporting the rollover so the IRS knows the distribution wasn’t spent. Skipping that step can trigger a notice treating the full amount as taxable income.

If any of the money came from an Archer Medical Savings Account rather than another HSA, MSA distributions are reported separately on Form 8853.7Internal Revenue Service. Form 1099-SA – Distributions From an HSA, Archer MSA, or Medicare Advantage MSA

Mistakes That Delay the Process

  • Sending a transfer form to Optum instead of your current administrator. Optum can’t process the outgoing side.
  • Not confirming your current bank accepts Optum’s form. If they reject it, you’ll lose weeks finding out.
  • Leaving invested positions in the account. The transfer can stall or get returned.
  • Choosing the rollover section when the transfer section would work. The rollover starts a 60-day clock, caps you at one per year, and creates extra tax reporting.
  • Submitting the form unsigned or with a months-old date. Either will come back to you.
