How to Complete the EFTPS Authorized Account Holder Verification Form (9787)

EFTPS Form 9787 is the paper form the IRS sends you to correct or update the enrollment information behind your Electronic Federal Tax Payment System account. You fill in the corrections on the right-hand column, sign the authorization, and mail the form to the EFTPS Enrollment Processing Center, P.O. Box 173788, Denver, Colorado 80217-3788.1Internal Revenue Service. 21.1.7 Campus Support It is not a security screening you have to hunt down on your own. It arrives with a cover letter that tells you exactly what to fix, and it also lives inside your EFTPS confirmation packet as the “Confirmation/Update Form” you can use later to change bank or contact details.

Why You Received Form 9787

The IRS or its financial agent mails Form 9787 when your EFTPS enrollment cannot be completed as submitted. The accompanying cover letter identifies the specific fields at issue. Some are marked “critical,” meaning enrollment stops until you correct them. Others are “non-critical” — flagged for your attention but not blocking activation.2Reginfo.gov. The same form is also the tool for later changes: a new bank account, a different contact person, an address update, or a corrected business name. Bank changes can also be made online at eftps.gov or by phone at 800-555-4477, but the paper form remains available.3Internal Revenue Service. EFTPS The Electronic Federal Tax Payment System

A Note for Individual Taxpayers

If you are an individual filer rather than a business, Form 9787 is probably not in your future. As of October 17, 2025, individuals can no longer create new EFTPS enrollments at eftps.gov. Individuals already enrolled can keep paying for now but are expected to transition to IRS Direct Pay or an IRS Online Account by approximately September 2026.4Electronic Federal Tax Payment System. Welcome to EFTPS Online The form is now primarily used by businesses and other entities that remain on the platform.

What to Gather Before You Start

Have this information in front of you before filling anything in:

  • Your nine-digit Employer Identification Number as it appears on IRS records
  • Your legal business name, exactly matching IRS filings
  • Name, mailing address, phone number, and email for the primary contact
  • The nine-digit routing transit number for your bank
  • Your checking or savings account number as your bank has it on file
  • Which type of account it is (checking or savings)

Every number and name must match your bank’s records exactly. A single wrong digit in a routing or account number can cause your financial institution to reject the debit, and a rejected deposit can trigger an IRS failure-to-deposit penalty.2Reginfo.gov. One boundary worth flagging: EFTPS requires a U.S. bank account. If you operate internationally without a domestic account, EFTPS is not available to you, and the IRS suggests checking whether your foreign bank has a U.S. affiliate that can help with same-day wires instead.5EFTPS.gov. How to Complete Form 9787

The form is laid out in two columns. The left column shows what the IRS currently has on file. The right column is where you write the corrections. Some fields on the right are shaded solid and cannot be changed through this form. Your cover letter identifies which items need attention.2Reginfo.gov. Tax Form 9787 with Instructions (OMB 1545-1467)

Taxpayer and Contact Information

Enter or correct your EIN and business name in the taxpayer boxes. Use the legal business name from your IRS filings. A mismatch here is one of the most common reasons enrollment stalls. Below the taxpayer section, complete the primary contact’s name, mailing address, phone number, and email.

Financial Institution Information

Enter the routing transit number, the account number, and the account type for the bank account EFTPS will debit when you schedule payments.

Box 28 asks whether you want EFTPS to verify your bank account information directly with your financial institution. Checking “YES” adds roughly ten days of processing but confirms the details before any payment attempts to clear. Checking “NO” lets you start using EFTPS as soon as your PIN arrives, with the tradeoff that you carry the risk of any error. If the numbers are wrong, the first payment fails and penalties follow. Leaving Box 28 blank defaults to verification.2Reginfo.gov. Tax Form 9787 with Instructions (OMB 1545-1467)

Authorization and Signature

The authorization section holds two agreements. The first authorizes the contact person and the financial institutions involved in processing your EFTPS payments to receive the confidential information needed to complete enrollment and answer payment inquiries. The second applies to payments made through the EFTPS website or phone system, authorizing designated financial agents of the U.S. Treasury to initiate debit entries against the bank account you listed.2Reginfo.gov. Tax Form 9787 with Instructions (OMB 1545-1467)

Sign and date the form, then print your name and title. The signer should be someone authorized to approve debits from the business account, such as a company officer, owner, or an individual with signing authority at the financial institution listed.

Where to Mail It

Send the completed form to:

Denver, Colorado 80217-37881Internal Revenue Service. 21.1.7 Campus Support

Photocopy the signed form before you drop it in the mail. If you later need to prove that corrections went in on time, particularly during a penalty dispute, that copy is your evidence.

After You Send the Form

Once the Enrollment Processing Center receives your form, expect a new PIN by mail in about seven to ten days.6EFTPS. International Electronic Federal Tax Payment/Deposit Instruction Booklet If you asked for bank verification in Box 28, or left it blank, add roughly ten more days for that step. Your enrollment is active once the PIN and updated confirmation materials arrive, and you can start scheduling payments.

If a tax deposit deadline is bearing down while your corrections are in transit, do not wait. Under IRC Section 6656, failure-to-deposit penalties start at 2 percent for deposits one to five days late and climb to 15 percent after a delinquency notice deadline passes.7Office of the Law Revision Counsel. 26 U.S. Code 6656 – Failure to Make Deposit of Taxes On a large payroll deposit those percentages add up quickly. A same-day wire through your bank, using the Same-Day Taxpayer Worksheet from irs.gov, is the usual fallback for businesses with a deadline they cannot miss.8Internal Revenue Service. Same-Day Wire Federal Tax Payments The wire fee will almost always be less than a penalty on a five- or six-figure deposit.

Changing Bank Information Without the Paper Form

If you are already enrolled and simply want to switch bank accounts rather than correct an enrollment error, you have faster options than mailing Form 9787:

  • Online at eftps.gov: log in, open “My Profile,” select “Edit Financial Institution Information,” and follow the prompts. New confirmation materials and a new PIN come by mail.
  • By phone at 800-555-4477 (or 303-967-5916 from outside the U.S.): press 3 to add a new bank account and enter the details. A separate PIN for the new account is mailed to you.6EFTPS. International Electronic Federal Tax Payment/Deposit Instruction Booklet
  • By mail using the Confirmation/Update Form from your original enrollment packet, which is Form 9787. Complete the new bank details in the right-hand column, sign, and mail to the Denver address.

Each bank account you add gets its own PIN. Once the new PIN works, call EFTPS Customer Service to remove any old account information you no longer need.6EFTPS. International Electronic Federal Tax Payment/Deposit Instruction Booklet
