How to Complete the ADA DENTPIN Request Form: Online or Paper

To register for a DENTPIN, go to the ADA’s online portal at adadts.my.site.com, fill in your personal information, and confirm your identity through the verification email the system sends you. The number is free, it’s eight digits, and it stays with you from your first dental admission test through licensure. One important caveat before you start: if you have ever taken or applied for any ADA-administered dental exam, or if you are already licensed as a dentist or dental hygienist in the U.S., you already have a DENTPIN and should retrieve it rather than register again.1American Dental Association. Check Whether You Already Have a DENTPIN

The ADA warns against creating a second account, and for good reason. A duplicate DENTPIN ties your new activity to a blank record while your exam scores and application history sit in the original one, which can complicate exam scheduling and future licensure applications.1American Dental Association. If you think you may already have one, log in at adadts.my.site.com and use the “Retrieve a forgotten DENTPIN” option. You can also reset your password through the same portal if that is what’s blocking you.1American Dental Association. Manage your DENTPIN If the self-service tool won’t work — for example, you no longer have access to the email you originally used — complete the paper DENTPIN Request Form and email it to dentpin@ada.org with as much identifying information as you can supply: any record of your assigned DENTPIN, the school you attended, and your program type.2American Dental Association. Already created a duplicate by mistake? The ADA does not publish a self-service merge process. Contact the Department of Testing Services at dentpin@ada.org to request that your records be consolidated.1American Dental Association. Manage your DENTPIN

Registering Online

First-time testers must register for a DENTPIN before applying to take any exam.1American Dental Association. Manage your DENTPIN Open the ADA portal at adadts.my.site.com, follow the registration link, and complete the required fields. The system validates your entries in real time and will flag missing or improperly formatted data before it lets you submit.

After you submit, a verification email goes to the address you entered. Click the link in that email to confirm your identity and activate the number. No fee is listed on the ADA’s registration page or the paper form, and the ADA does not advertise a cost; the process appears to be free.

What Information You’ll Need

Enter your full legal name exactly as it appears on the government-issued photo ID (passport or driver’s license) you’ll bring to the testing center. This matters more than it looks: if your DENTPIN record doesn’t match the name on your ID at a Prometric center, you won’t be allowed to sit for the exam, and name discrepancies must be corrected at least seven days before your exam date.3National Board of Medical Examiners. Based on the paper DENTPIN Request Form, registration collects:2American Dental Association. DENTPIN Request Form

  • Full legal name (first, middle, last) as listed on your application or ID
  • Any previous names or aliases, including maiden names
  • Date of birth
  • Email address, which is also your channel for security alerts and DENTPIN retrieval
  • Daytime phone number (home, cell, or work)
  • Mailing address, including country
  • Program type: pre-dental, dental, advanced program, or dental hygiene
  • School name and year of graduation, if applicable

You will not be asked for your Social Security Number. The DENTPIN system was created specifically to replace SSNs, Canadian Social Insurance Numbers, and similar identifiers across dental education records.1American Dental Association. Manage your DENTPIN Double-check your ZIP code and email before submitting; wrong entries in those two fields are a common source of validation errors.

Using the Paper Form Instead

If you can’t complete registration online, the ADA provides a PDF request form as a fallback. It covers the same fields as the online portal.2American Dental Association. DENTPIN Request Form Fill it out completely and email it, along with any supporting documentation, to dentpin@ada.org.

Paper requests take approximately one to three business days to process — slower than the online path. If you have a testing registration deadline coming up, register online.2American Dental Association. DENTPIN Request Form

Updating Your Record Later

Routine updates — address, phone, email — can be made directly through the ADA portal after you log in. No documentation is required for those.1American Dental Association. Manage your DENTPIN

Legal name changes and other corrections need supporting documents submitted with the paper form to dentpin@ada.org:2American Dental Association. DENTPIN Request Form

  • Name change from marriage, divorce, or court order: government-issued photo ID plus the marriage certificate, divorce decree, or court order
  • Date of birth correction: birth certificate or driver’s license showing the correct date
  • School or graduation date change: official transcript or diploma

These take one to three business days. Whatever the change, do not open a new account to fix it.

What You’ll Use the Number For

Your DENTPIN is the login credential across dental admission tests (including the DAT and ATDH), board and licensure exams (including the INBDE, NBDHE, DLOSCE, and DHLOSCE), the advanced admission test (ADAT), and the major application services: ADEA AADSAS for dental school, PASS for advanced dental education programs, CAAPID for international dental graduates, TMDSAS for Texas applicants, and the Dental Match program.1American Dental Association. International Dental Graduates

If you graduated from a dental school outside the U.S. or Canada and are applying through ADEA CAAPID, you need a DENTPIN before you can start the application.5ADEA.
