To make a UOB supplementary card application in Singapore, the primary cardholder completes UOB’s Supplementary Card Application Form, attaches the applicant’s identification and residency documents, and submits it either online through Personal Internet Banking or on paper. The new card links to the primary account’s credit line, and the primary cardholder remains responsible for all charges on it.
Who Can Be a Supplementary Cardholder
The supplementary applicant must generally be at least 18. Singapore’s Banking (Credit Card and Charge Card) Regulations make it an offence for an issuer to give a supplementary card to anyone under 18, except where the person needs the card for overseas travel.1Singapore Attorney-General’s Chambers. The form asks for the applicant’s relationship to the principal cardholder but does not publish a closed list. In practice, supplementary cardholders are usually spouses, parents, children, or siblings of the primary cardholder.
Documents to Gather Before You Start
What you attach depends on the supplementary applicant’s residency status. Pull these together before opening the form so you’re not stopped halfway.
Singapore Citizens and Permanent Residents
- A copy of the NRIC, front and back.
- A recent billing statement dated within the last six months (for example, a telephone or utilities bill), but only if the applicant’s current address differs from the NRIC address.
Foreign Nationals
- A copy of the passport biodata page, with at least six months of remaining validity.
- A copy of a valid Singapore pass: Employment Pass, S-Pass, Work Permit, EntrePass, Student Pass, Dependant’s Pass, or Long-Term Social Visit Pass.
- A recent billing statement, within the last six months, showing the applicant’s local home address.

What the Form Asks
The supplementary card section sits in Section 10 of UOB’s combined credit card application. Every field marked with a cross (+) or asterisk (*) is mandatory, and incomplete forms will be delayed.3United Overseas Bank. UOB Credit Cards – Define Your Rewards
Enter the applicant’s full legal name exactly as it appears on their NRIC or passport, with the surname underlined. The “Name to appear on Card” field allows up to 19 characters including the surname, so an abbreviation may be needed. Fill in the NRIC or passport number, nationality, gender, date of birth, country of birth, and race. If the applicant holds more than one citizenship, tick “Yes” for other nationalities and specify.
A home telephone number is mandatory; UOB uses it for card activation and One-Time Password SMS delivery. Add the mobile number too. The address fields ask for street or building name, house or block number, unit number, and postal code.
Under employment, list company name, occupation, and industry from the provided dropdown, then pick the annual income bracket (from “No Income” up to “S$300,000 and above”). Tick the self-employed box if it applies.
Setting a Credit Sub-Limit
You can pre-set a credit limit for the supplementary card in multiples of S$500, starting from S$500.4United Overseas Bank. UOB Supplementary Card This caps what the supplementary cardholder can spend. Tick “No preference” if you’d rather the supplementary card share the full credit limit of the primary account. Either way, the supplementary card draws from the same overall credit line and does not increase the total limit.
Both the primary cardholder and the supplementary applicant must sign the declaration. Signatures should match the identification documents you’re attaching.
How to Submit
There are two routes.
The online route is faster. The primary cardholder logs into UOB Personal Internet Banking and starts the supplementary card application there. UOB then emails the supplementary applicant a unique code, and the applicant completes their portion online within 10 calendar days of receiving that email.4United Overseas Bank. UOB Supplementary Card Fields can be auto-filled from Myinfo, and document verification begins immediately.
The paper route works too. Download the PDF form, print and complete it, attach the required identification copies, and either mail it to UOB or drop it at any UOB branch. The exact processing centre address is not printed on the supplementary card form, so call UOB’s card service hotline or ask at a branch for the current mailing address before you post it.
UOB does not publish a set processing timeline for supplementary cards. If more information is needed, the follow-up will come by email or SMS to the primary cardholder.
Fees

How the Shared Limit and Rewards Work
A supplementary card does not add to the primary account’s total credit limit. Both cards draw from the same pool. If the primary account has a S$10,000 limit and the supplementary card is capped at S$3,000, the primary cardholder can still use up to S$10,000 when the supplementary card has a zero balance; the sub-limit only restricts the supplementary cardholder.

Who Pays the Bill
UOB’s cardmember agreement makes the primary cardholder responsible for all liabilities on the account, including charges, annual fees, and any other fees incurred by supplementary cardmembers. Each supplementary cardmember is also individually responsible for the charges they personally incur on their card.3United Overseas Bank. UOB Credit Cards – Define Your Rewards If the supplementary cardholder doesn’t pay, the bank comes to the primary cardholder for the entire balance.
Setting a lower sub-limit is one way to contain that exposure. Late or missed payments on the shared account affect the primary cardholder’s credit record, since UOB reports account performance under the primary cardholder’s name.
Cancelling a Supplementary Card Later
The primary cardholder can request cancellation through UOB’s customer service channels, and the supplementary cardholder can also cancel their own card. Any outstanding balance on the supplementary card remains the primary cardholder’s responsibility after cancellation. Cut the physical card before disposing of it and confirm with UOB that the cancellation has gone through, so an annual fee doesn’t reappear on the next billing cycle.
