What to Gather Before You Start

- The date your disability began, your last day worked, and any dates you attempted to return.
- Names, addresses, phone numbers, and fax numbers for every provider who has treated the condition.

You provide your identifying information, describe the condition, and explain how it prevents you from doing your job. Be specific about limitations. “I cannot sit for more than 15 minutes without severe lower back pain” tells the examiner something; “I have back problems” does not. Include when symptoms started, when you stopped working, and whether the condition is work-related. Work injuries normally go through workers’ compensation rather than short-term disability.
Your Employer’s Section
HR fills this out, confirming your job title, salary, work schedule, employment status, and last day worked, plus the tax information Unum needs to set up payments. Your Doctor’s Section (Attending Physician’s Statement)
This section carries the most weight. What Happens After You Submit
You can track the claim through the MyUnum portal or mobile app. The dashboard shows status updates, outstanding information requests, and payment details once benefits are approved.1Unum. A dedicated claims examiner reviews your medical evidence against the definition of disability in your specific policy. The examiner may contact you or your physician for clarification or additional diagnostic evidence. Respond quickly. Unum sometimes also arranges an independent medical examination with a doctor of its choosing. Cooperating with that exam is effectively required. How Long Unum Has to Decide
For group plans governed by ERISA, Unum has 45 days from receipt of a completed claim to issue a decision. If circumstances outside Unum’s control prevent a decision within that window, it can extend the deadline by 30 days, but it must notify you in writing before the initial 45 days expire and explain what is still unresolved. Policy Terms That Decide the Outcome
The Elimination Period
Every short-term policy has a waiting period that must pass after disability begins before any benefits are paid. No benefits are paid for days inside that window, so you need to plan for the gap.
Definition of Disability
How your policy defines “disability” is often what decides approval. Most group short-term policies use an “own occupation” standard, meaning you qualify if your condition prevents you from performing the main duties of your specific job. Some policies shift to an “any occupation” standard after a set period, under which Unum evaluates whether you could perform any work suited to your education, training, and experience. Benefit Amount and Offsets
Unum short-term disability benefits replace up to 60% of your monthly income, with the actual percentage depending on the plan your employer selected.7Unum. Your gross benefit may be reduced by offsets, meaning income from state disability, Social Security, or workers’ compensation that your policy requires Unum to deduct.
Pre-Existing Condition Clauses
Most group disability policies exclude or limit benefits for conditions you were treated for, took medication for, or saw a doctor about during a lookback period before coverage started. A common structure is a “3/12” limitation: treatment during the 3 months before coverage began means disabilities arising from that condition are excluded for the first 12 months of coverage. If Your Claim Is Denied
A denial is not the end of the matter, but the response window is strict. The denial letter must list the specific reasons for the decision, the policy provisions Unum relied on, and what additional information could change the outcome. Read it carefully. Many denials turn on insufficient medical documentation rather than a flat finding that you are not disabled. If that is your situation, the appeal is where you submit stronger evidence: updated treatment notes, new test results, a functional capacity evaluation, or a narrative letter from your physician that directly answers the reasons Unum cited.
The appeal matters even more than it looks. For ERISA-governed plans, a federal court reviewing a later lawsuit generally confines itself to the evidence that was in the administrative record at the time of the appeal decision. New evidence raised for the first time in court is often excluded. The appeal is your best, and sometimes only, chance to build the record.
Mistakes That Sink Valid Claims
Filing late is the most avoidable error. Notify your employer and start the process as soon as you know you will be out beyond the elimination period.
