How to Complete and Submit the DS-2001 Immigrant Visa Readiness Form

The DS-2001 immigrant visa readiness form is an optional notification you send to a U.S. Embassy or Consulate to confirm you have gathered every document required for your visa interview. The Foreign Affairs Manual states the form is not mandatory: posts will accept any reasonable notification, signed or unsigned, and cases forwarded from the National Visa Center are treated as ready by default unless flagged for expedited handling.1U.S. Department of State. 9 FAM 504.4 (U) Pre-Appointment Processing Even so, several posts, including the U.S. Consulate General in Hong Kong, include the DS-2001 in their instruction packets, and completing it gives you a clean way to move your case into the interview queue.

When You Actually Need to File a DS-2001

Most immigrant visa cases route through the National Visa Center. You pay the fees, file the DS-260, and upload civil and financial documents. Once NVC has all of that, it marks the case documentarily qualified and forwards it to the consulate, which generally considers you ready without any further notification.1U.S. Department of State. 9 FAM 504.4 (U) Pre-Appointment Processing

The DS-2001 becomes relevant in two situations. The first is a petition filed directly at a consular post rather than through NVC. The second is any post whose local immigrant visa instructions specifically ask for the form. If your embassy’s packet includes it, complete it. There is no downside, and it removes ambiguity about whether you are ready to be scheduled.

Documents You Must Have Before You Sign

Signing the DS-2001 is a self-attestation that every document the consular officer will expect at your interview is already in your hands. Showing up without something you attested to can result in a refusal under Section 221(g) of the Immigration and Nationality Act, which gives you one year to supply what is missing before the case terminates and you have to reapply with a new fee.2U.S. Department of State. Civil Documents

You and every family member immigrating with you need an original or certified copy of a birth certificate issued by the official authority in the country of birth.3U.S. Department of State. Civil Documents Beyond that, gather whichever of the following apply:

  • Marriage certificates for every marriage, not just the current one, as originals or certified copies.
  • Marriage termination records: a final divorce decree, death certificate, or annulment papers for each prior marriage.
  • Certified copies of court and prison records for any criminal conviction, even if you were later pardoned or granted amnesty.
  • A photocopy of your military record from any country where you served.
  • A certified copy of any adoption decree, plus legal custody records if custody preceded the adoption.

Plain photocopies will not be accepted at the interview. If a required civil document is unavailable for reasons specific to the issuing country, submit a written explanation to NVC when you scan your other documents. The consular officer decides at the interview whether the gap is a dealbreaker.3U.S. Department of State. Civil Documents

Police Certificates

Police certificate rules are stricter than most applicants expect. The State Department requires certificates from applicants age 16 and older, but the triggering conditions depend on the country and how long you lived there:3U.S. Department of State. Civil Documents

  • From your country of nationality if you lived there more than six months at any point in your life.
  • From your current country of residence, if different from nationality, if you have lived there more than six months.
  • From any other country where you lived 12 months or more while age 16 or older.
  • From the city or country of any arrest, regardless of your age at the time or how long you lived there.

Police certificates are valid for two years from issuance. If yours will expire before the interview, or if you turned 16 after your case became documentarily complete, request a fresh one.4U.S. Department of State. Interview Preparation

Financial Evidence

Your U.S.-based sponsor files Form I-864, Affidavit of Support, showing they can support you at 125 percent of the federal poverty guidelines, or 100 percent for active-duty military sponsors petitioning for a spouse or child. For 2026, a sponsor with a household of two needs at least $27,050 in annual income under the standard threshold.5USCIS. I-864P HHS Poverty Guidelines for Affidavit of Support The I-864 and recent federal tax returns or transcripts go to NVC as part of the financial documents package. If the sponsor’s income falls short, a joint sponsor or applicant assets can cover the gap.

Photos, Translations, and Passport

Any document not in English, or not in the official language of the country where you are applying, needs a certified translation. The translator signs a statement confirming the translation is accurate and that they are competent in both languages.3U.S. Department of State. Civil Documents You cannot translate your own documents. Some posts also require notarization of the translator’s signature, so check your embassy’s local instructions.

Filling Out the DS-2001

The DS-2001 is not on the Consular Electronic Application Center, which handles the DS-260 and DS-261. It lives as a PDF on individual embassy and consulate websites. The U.S. Consulate General in Hong Kong, for example, posts a version labeled “DS2001 HNK.”8U.S. Consulate General Hong Kong and Macau. The form itself is short. Enter your Department of State case ID: three letters followed by nine or ten digits, for example ABC0123456789. Diversity Visa selectees use a different format, four numbers, two letters, then five numbers.9USCIS. Tips on Finding Your A-Number and DOS Case ID List every family member immigrating with you by name. Check each box confirming you have the corresponding document category on hand: photos, financial evidence, civil records. Sign and date at the bottom.

Do not sign until every document is physically in your possession. Under the INA inadmissibility rules, a false representation made to a government official to obtain an immigration benefit, even an unsuccessful one, can be treated as willful misrepresentation and make you permanently inadmissible.10USCIS. Overview of Fraud and Willful Misrepresentation A consular officer who finds you checked boxes for records you did not actually have could view that as material misrepresentation. The practical risk is low if you are waiting on one last document, but the safe move is to hold the form until everything is truly collected.

Submitting the Form

Delivery instructions vary by post. Hong Kong asks applicants to mail the completed DS-2001 and document checklist to the Immigrant Visa Unit at a physical address.8U.S. Consulate General Hong Kong and Macau. DS-2001 Immigrant Visa Readiness Form Other posts accept scanned copies by email or fax, and the Foreign Affairs Manual treats electronic notification as equally acceptable.1U.S. Department of State. 9 FAM 504.4 (U) Pre-Appointment Processing

Confirm the exact submission method and address on your embassy’s immigrant visa page. Sending the form to the wrong branch delays processing, and there is generally no automatic acknowledgment of receipt. If you are unsure it arrived, contact the post’s immigrant visa unit directly.

What Happens After You Submit

Once your case is documentarily qualified and the post has your notification, your file enters the interview scheduling queue. NVC fills interview slots first in, first out using dates provided by the embassy.11U.S. Department of State. Helpful Hints – IV Processing Applicants in numerically limited preference categories also need a current priority date before they can be scheduled. Wait times depend on staffing, facilities, and local conditions.12U.S. Embassy & Consulates in Canada. Immigrant Visa Process

When your date is set, NVC emails the appointment notice to you, your petitioner, and your attorney or agent. That notice triggers two things: booking the medical exam and organizing every original document for the interview.

Medical Examination

Every immigrant visa applicant needs a physical and mental examination by a panel physician authorized by the U.S. Embassy in your country.13Centers for Disease Control and Prevention. Technical Instructions for Panel Physicians For consular processing overseas, you must use a panel physician in that country, not a U.S.-based civil surgeon.14USCIS. Finding a Medical Doctor Your embassy’s site lists approved physicians nearby.

The exam covers your vaccination history. Depending on age, you may need proof of vaccination against measles, mumps, rubella, polio, tetanus, hepatitis A and B, varicella, and other diseases on the CDC’s required list. If the physician gives you a sealed envelope, keep it sealed and hand it to the consular officer at the interview.4U.S. Department of State. Interview Day and the 221(g) Clock

Bring every original civil document you uploaded to NVC, your two photos, the sealed medical envelope, your valid passport, and the appointment letter. The officer reviews your file, asks questions, and makes an eligibility determination. Originals are returned at the end of the interview.15ustraveldocs.com. If something is missing or needs verification, the case is refused under Section 221(g). You then have one year from that refusal to submit what is missing.
