How to Complete and Submit FCC Form 498: Filing and 498 ID Upkeep

To complete FCC Form 498, register your organization in the USAC E-File portal at forms.universalservice.org, fill in only the blocks that apply to your entity and to the Universal Service Fund programs you participate in, and have a Company Officer electronically certify the submission. USAC uses the form to assign your 498 ID, also called a Service Provider Identification Number (SPIN), which is what lets money from the High Cost, Lifeline, Rural Health Care, or Schools and Libraries (E-Rate) programs actually reach your bank account.1Universal Service Administrative Company. The form itself is straightforward once you have the right numbers in front of you. Most delays come from missing information, mismatched IDs, or waiting on a Company Officer to sign. Work through the preparation below before you log in.

Gather Your Information First

Have every one of these ready. A single missing or wrong item can stall the filing or trigger a rejection.

  • Federal Employer Identification Number (EIN). It must match the EIN on your FCC Form 499 if you file one, because USAC cross-checks the two.
  • SAM.gov Unique Entity Identifier (UEI), the twelve-character number that replaced the old DUNS number. Beginning August 2026, USAC will use SAM.gov banking information to pay all USF invoices, so your UEI must be active and linked to a valid bank account in SAM.gov.1Universal Service Administrative Company.   • FCC Registration Number (CORES ID), a nine-digit number from the Commission Registration System.
  • Banking details: the nine-digit ABA routing number, account number, account type, and the financial institution’s name. Copy these from a voided check or your bank’s ACH documentation rather than typing from memory.
  • Contact information for the Company Officer, the General Contact, and any program-specific remittance contacts. You’ll need names, titles, phone numbers, and email addresses.
  • Legal entity name and physical mailing address. The name must match your official government records, and P.O. boxes are not accepted.

If you don’t yet have a SAM.gov UEI, register at SAM.gov before starting. That process can take a week or more, and your Form 498 cannot move forward without it.

Log In to E-File and Choose Your Filing Action

Form 498 is filed online through the USAC E-File portal. New filers register directly in E-File, and the system assigns user IDs for the Company Officer and General Contact during that registration.2Universal Service Administrative Company. How to Use E-File

The first choice on the form is the filing action. There are four, and each requires certification by a Company Officer:3Federal Communications Commission. FCC Form 498 – Service Provider and Billed Entity Identification Number and Contact Information Form

  • Original Application, for your first-ever Form 498 to obtain a new 498 ID.
  • Revision, to update information on an existing 498 ID. You enter your existing 498 ID number when prompted.
  • Merger/Consolidation, to fold multiple 498 IDs into one or absorb another entity’s 498 ID after an acquisition. USAC requires additional documentation.
  • Deactivation, to shut down your 498 ID when you stop participating in all USF programs. High Cost and Lifeline recipients must also comply with 47 C.F.R. § 54.205 when relinquishing support.

If this is your first filing, choose Original Application.

Fill In the Numbered Blocks

The form is organized into numbered blocks, and you only complete the blocks relevant to your entity type and the programs you participate in.

Block 1 asks for your legal company name, any doing-business-as or former names, your holding company’s name and Tax ID if applicable, and your physical mailing address. If you have affiliate relationships as defined in the Communications Act, flag them here.

Block 2 collects the General Contact, the person who handles billing, collection, and disbursement questions. The General Contact also serves as the main point of contact for E-File and can make revisions, though only the Company Officer can certify them.4Universal Service Administrative Company. Block 3 is where you enter your federal EIN, your UEI (formerly DUNS number), and your nine-digit FCC Registration Number.

Blocks 5 through 13 are program-specific. Complete only the blocks for programs you participate in: High Cost, Lifeline, Rural Health Care, or Schools and Libraries. Each program block asks for a remittance contact and your financial institution details, including routing number, account number, and account type. Double-check every digit. Blocks 15 through 17 and Block 19 ask whether you want USAC to offset your disbursement payments against any outstanding universal service contribution obligations. If your company both receives USF support and owes USF contributions, this netting arrangement can simplify cash flow.

Certify With a Company Officer

After the data entry is complete, a Company Officer must electronically certify the form. For service providers, an officer is someone whose position is specified in the corporate bylaws, typically a president, vice president, comptroller, or treasurer. Sole proprietors sign for themselves. For schools and libraries, this means a superintendent, principal, district administrator, or comparable official.3Federal Communications Commission. The electronic signature carries full legal weight. The officer attests under penalty of perjury that everything on the form is true and accurate. If someone else fills out the form, the officer has 14 days from submission to log in and certify it. USAC will not process an uncertified form.5Universal Service Administrative Company. What Happens After You Submit

USAC processes original applications within seven to ten business days and assigns your 498 ID upon approval. During that window, USAC validates your banking details with your financial institution to confirm the routing number and account are legitimate and active.

For E-Rate applicants filing for an applicant 498 ID, the timeline is shorter. USAC verifies banking information and notifies you within about two business days whether the form is approved or rejected.6Universal Service Administrative Company. If banking validation fails because the account number doesn’t match, the account is closed, or the routing number is wrong, USAC rejects the form and you’ll resubmit with corrected information. A rejection carries no penalty, but it does reset the processing clock.

Mistakes That Trigger Rejection

USAC is required to reject any Form 498 with missing or incomplete information or incorrect bank account details.5Universal Service Administrative Company. Manage Your 498 ID A handful of errors come up again and again:

  • Mismatched EINs. Your EIN on Form 498 must match the EIN on FCC Form 499. A mismatch flags your account for manual review.
  • Wrong routing or account numbers. Transposing even one digit sends the banking validation into failure.
  • Uncertified forms. If the General Contact submits the form but the Company Officer doesn’t certify within 14 days, USAC treats the submission as incomplete.
  • Inactive SAM.gov UEI. With the August 2026 shift to SAM.gov-based payments, an expired or unregistered UEI will block disbursements even when banking information is otherwise correct.
  • P.O. box as mailing address. The form requires a physical street address.

Verify every number against its official source document before submitting, and coordinate with your Company Officer in advance so certification happens promptly.

A Valid 498 ID Alone Isn’t Enough

Even a perfectly filed Form 498 won’t get you paid if your organization is in red light status with the FCC. When any entity’s account becomes delinquent by even one day on amounts owed to the FCC, USAC withholds all payments to that entity, and to any other entity sharing the same taxpayer identification number, until the debt is resolved or a satisfactory payment plan is in place.7Universal Service Administrative Company. Keep the 498 ID Current

An active 498 ID isn’t a permanent record. File a revision whenever any of the following changes:

  • Your bank account or financial institution.
  • Your company name, address, or EIN.
  • The contacts listed on the form, whether General Contact, remittance contact, or Company Officer.
  • Your program participation, meaning you add or drop a USF program.
  • A merger or consolidation involving your 498 ID.

Only the General Contact or a Company Officer can submit a revision, and the Company Officer must certify it within 14 days.5Universal Service Administrative Company. If funds are sent to a closed account, the payment bounces and you wait for the next disbursement cycle, so update the form the moment anything changes.
