Sell Your Investments First
If any of your balance sits in mutual funds or other securities through Optum’s investment platform, sell those holdings before you submit the closure form. Optum does not liquidate investments automatically when it receives a closure request.2Optum Financial. Health Savings Account (HSA) Account Closure Form
Log in to your Optum Bank account, select “Sell Investments,” and choose “Liquidate Entire Portfolio.” If you set up automatic investments at some point, pause them so your cash isn’t immediately reinvested. That option lives under “Manage Automatic Investments,” then “Pause Automatic Investments.”2Optum Financial. Fill Out the Closure Form
The HSA Account Closure Form is a download from Optum Bank’s site.2Optum Financial. Health Savings Account (HSA) Account Closure Form Have your Optum account number and current balance ready before you start.
On the form, you pick how the remaining money leaves the account. Two options:
- Direct transfer to a new HSA custodian. Optum sends the funds straight to another HSA provider, and you supply the receiving institution’s name, address, and account number. The money stays tax-advantaged.
- Distribution check mailed to you. Optum cuts you a check. If you want that money in another HSA, you have 60 days to deposit it yourself or the whole amount becomes taxable and may be penalized.
Errors on the form, especially a missing signature or wrong receiving-institution details, are the usual reason closures stall. Look everything over twice.

Fee and Timing
Optum Bank charges $20 for an outbound transfer or rollover to another HSA custodian.5Optum Bank. Schedule of Fees The fee comes out of your balance before the money is sent. If your balance is smaller than the fee, the account may simply close with no payout. Employer arrangements can change the fee schedule, so check your plan documents if the number you see differs.
Processing usually runs about seven to ten business days from the day Optum receives a complete form. Watch your online account: when the balance hits zero, the closure is done. At that point your HSA debit card is canceled. Let any pending card charges clear before you submit the form so they don’t bounce back after the account is closed.
Direct Transfer or Indirect Rollover
The choice you make on the form has real tax consequences.
Trustee-to-Trustee Transfer
In a direct transfer, the money moves from Optum to the new custodian without passing through your hands. It isn’t a taxable distribution, and there is no cap on how many direct transfers you can do in a year.6Internal Revenue Service. Instructions for Form 8889 (2025) For most people closing an Optum HSA, this is the cleanest option.
Indirect Rollover
With an indirect rollover, Optum sends the funds to you and you deposit them into a new HSA within 60 days. Miss the deadline and the full amount becomes taxable income, plus a 20 percent additional tax if you’re under 65. An HSA can only receive one rollover contribution in any 12-month period, another limit that doesn’t apply to direct transfers.6Internal Revenue Service. Instructions for Form 8889 (2025)
If You Take the Cash Instead
Money you pull out and spend on qualified medical expenses stays tax-free.7Office of the Law Revision Counsel. 26 USC 223 – Health Savings Accounts Anything else gets added to your gross income for the year, and you owe an extra 20 percent tax on top.6Internal Revenue Service. Instructions for Form 8889 (2025)
The 20 percent penalty is waived in three situations:
- You are 65 or older. Non-medical distributions are still taxed as ordinary income, but the penalty drops off.
- You become disabled.
- The account holder has died and the distribution goes to a beneficiary.
The distribution is still taxable in these cases unless it went to qualified medical expenses.8Internal Revenue Service. Publication 969 – Health Savings Accounts and Other Tax-Favored Health Plans Keep receipts for anything you paid for with HSA funds; you may need them to show the money was spent on qualified care.
What You’ll Get at Tax Time
Optum Bank is required to report every distribution from your HSA to the IRS.7Office of the Law Revision Counsel. 26 USC 223 – Health Savings Accounts It does that by issuing Form 1099-SA, which lists the total distributed during the tax year. Expect it by the end of January after the year you closed the account.
On your federal return, you report HSA activity on Form 8889, which handles contributions, distributions, and taxable amounts.6Internal Revenue Service. Instructions for Form 8889 (2025) Employer contributions and any pretax payroll contributions you made through a cafeteria plan show up on your W-2 in box 12 with code W; they go on Form 8889 but aren’t deductible because they were never in your taxable wages.
A direct trustee-to-trustee transfer generally shows on the 1099-SA but is not taxable. You still report it on Form 8889 to document the money stayed inside the HSA system.6Internal Revenue Service. Instructions for Form 8889 (2025) Hold on to the 1099-SA, any transfer confirmation letters, and your medical expense receipts for at least three years.
Consider Whether You Still Need to Close
If the reason you’re closing is that your current health plan no longer seems HSA-eligible, the rules for 2026 are worth a second look. The 2026 contribution limit is $4,400 for self-only coverage and $8,750 for family coverage, with a $1,000 catch-up for those 55 or older.9Internal Revenue Service. Rev. Proc. 2025-19
The One, Big, Beautiful Bill Act, signed July 4, 2025, expanded eligibility starting in 2026. Bronze-level and catastrophic marketplace plans qualify as HSA-compatible, and people enrolled in certain direct primary care arrangements can contribute too.10Internal Revenue Service. One, Big, Beautiful Bill Provisions If your plan fits one of these new categories, you may be able to keep the account open. A handful of states don’t follow the federal tax treatment of HSA contributions, so check your state’s rules if state tax savings matter to your decision.
