To cancel Wellmark insurance on an individual plan, complete the Individual Health Plan Contract Change Form (form N-5428), fill in Section G for “Cancellation of Entire Policy,” and send it to Wellmark by mail, fax, or email. Cancellations take effect only on the last day of a calendar month, and the earliest possible effective date is the last day of the month after Wellmark receives your request. If your coverage is through an employer, you go through your company’s benefits administrator instead, and if you have a Wellmark Medicare Advantage plan, you drop it during a Medicare election window rather than by sending a form to Wellmark.
How to Submit Your Cancellation
For an individual plan bought directly from Wellmark or through the ACA Marketplace, form N-5428 is the document Wellmark uses. Complete Section G along with the other required sections. The signed form has to reach Wellmark within 15 days of your signature date, or your requested effective date and eligibility may change.

If you’d rather start by phone, call Wellmark member services at 800-524-9242, Monday through Friday, 7:30 a.m. to 5:00 p.m. Central.1Wellmark. Contact Us Have your Member ID, Social Security number, and requested termination date ready before you dial. Even if you call, follow up in writing. Mailing the form by certified mail gives you proof of the date Wellmark received it, and because the effective date depends on that receipt date, the proof matters.
If your Wellmark plan comes through your job, don’t send anything to Wellmark directly. Your employer’s HR or benefits department processes the change, and Wellmark acts on what the employer submits. Ask your benefits administrator for the specific form and timing your company uses.
What You Need Before You Start
Gather these items first so the form goes in cleanly:
- Your Wellmark Member ID number, from your ID card.

- Your requested termination date, which must be the last day of a calendar month.
- Names and dates of birth for all covered dependents if you’re canceling the entire policy.
- Supporting documentation if a qualifying life event is driving the cancellation. Wellmark requires it within 60 days of the event.
One side effect worth knowing: vision or dental benefits for anyone under 19 that are bundled with your health coverage cancel automatically when the health plan ends, and there’s no way to keep them separately.
Effective Dates and Stopping Automatic Payments
The timing rule is strict. Cancellation lands on the last day of a calendar month, and the earliest date Wellmark will use is the last day of the month after it receives your form. Submit on March 5 and the earliest cancellation date is April 30, which means you owe premiums through April. Missing the 15-day window between your signature date and Wellmark’s receipt can push the timeline further out.
If your premium comes out of your bank account automatically, Wellmark needs your cancellation of the auto-draft by the 10th of the month before the next scheduled withdrawal. If your coverage ends April 30 and a May draft is scheduled, Wellmark needs the auto-withdrawal cancellation by April 10. Miss that cutoff and you’ll need to contact your bank directly to stop the payment, and any bank fees are on you.3Wellmark Blue Cross and Blue Shield. Automatic Withdrawal Authorization Form
If Wellmark collects a premium for a period after coverage ends, the overpayment is credited to your account and applied to any balance due. If there’s no balance to apply it against, call the number on the back of your ID card to request a refund. Wellmark does not automatically mail a refund check.4Wellmark. For Marketplace plans, federal rules define “reasonable notice” as at least 14 days before the requested termination date. Give less notice than that and the termination defaults to 14 days after you make the request.5eCFR. 45 CFR 155.430 – Termination of Coverage or Enrollment for Qualified Individuals
When You’re Actually Allowed to Cancel
Individual Plans
If you bought your plan directly from Wellmark or through the Marketplace, you can request cancellation any time. The harder part is on the other side. Once you cancel, you generally cannot enroll in a new Marketplace plan until the next Open Enrollment Period (November 1 through January 15) unless a qualifying life event, such as marriage, a birth, or a move, opens a Special Enrollment Period.6HealthCare.gov. Employer-Sponsored Plans
If premiums come out of your paycheck pre-tax, which is how most employer plans are structured, the IRS limits mid-year changes to a short list of change-in-status events: marriage or divorce, a new child, loss of other coverage, gaining Medicare or Medicaid eligibility, or a significant change in your employment hours. Outside those events, you wait for your employer’s annual open enrollment. If your premiums are paid with after-tax dollars, this restriction doesn’t apply and you can cancel mid-year.
Medicare Advantage Plans
To drop a Wellmark Medicare Advantage plan and return to Original Medicare, use the Medicare Advantage Open Enrollment Period, January 1 through March 31.7Medicare. Joining a Plan One plan change is allowed during that window: switch to another Medicare Advantage plan, drop yours and return to Original Medicare, or add a standalone Part D plan if you’ve returned to Original Medicare. Dropping a Medicare Advantage plan that includes drug coverage can trigger a Part D late enrollment penalty if you go 63 or more consecutive days without other creditable prescription drug coverage.8Medicare. Don’t Create a Coverage Gap
The practical risk of canceling is the stretch between old coverage ending and new coverage starting. In that gap you pay full price for any care, prescriptions can be interrupted, and you lose the network rates your insurer had negotiated.
Before you cancel, confirm that your new coverage starts on or before the day after your Wellmark coverage ends. Employer plans typically begin on the first of the month following hire or the end of a waiting period. For a Marketplace plan, the next Open Enrollment runs November 1 through January 15, and outside that window you need a qualifying life event to buy individual coverage.9Centers for Medicare & Medicaid Services. Deductible progress does not carry over to a new plan. If you’ve paid $3,000 toward a $4,000 deductible on your Wellmark plan and switch mid-year, your deductible resets to zero with the new insurer. Late in the calendar year, when you may be close to meeting your deductible or out-of-pocket maximum, waiting until January is often the cheaper move.
COBRA If You’re Losing Job-Based Coverage
If you’re losing Wellmark coverage because you left a job or had hours cut, COBRA may let you keep the group plan temporarily. COBRA applies to employers with 20 or more employees and generally allows up to 18 months of continuation (36 months in some situations, such as divorce or a dependent aging out). The cost shock is real: you pay the full premium plus a 2% administrative fee, up to 102% of the total plan cost.10U.S. Department of Labor. Continuation of Health Coverage (COBRA)
Your employer has 30 days after the qualifying event to notify the plan administrator, and the administrator then has 14 days to send the COBRA election notice.11Office of the Law Revision Counsel. 29 USC 1166 – Notice Requirements If the employer is also the plan administrator, the combined window is 44 days.12Centers for Medicare & Medicaid Services. HSA and Tax Credit Consequences
If your Wellmark plan was a high-deductible health plan paired with a Health Savings Account, canceling changes your HSA eligibility. You must stop contributing once you no longer have qualifying HDHP coverage, and your annual contribution limit is prorated by the number of months you were eligible. The IRS treats you as eligible for any month where you had HDHP coverage on the first day. Cancel on June 30 and you were eligible for six months, so your contribution limit is roughly half the annual amount. Anything you contributed above the prorated limit is an excess contribution subject to a 6% excise tax unless you withdraw it before your tax filing deadline. The balance already in your HSA stays yours and can still pay qualified medical expenses.
If you received advance premium tax credits on a Marketplace plan, canceling mid-year triggers a reconciliation at tax time. The Marketplace will send Form 1095-A showing the months you were covered and the advance credits paid. For tax years after 2025, there is no repayment cap on excess advance premium tax credits: if your actual income came in higher than your enrollment estimate, you repay the full difference between the credits received and what you were entitled to.14Internal Revenue Service.
