DD forms are the standardized documents the Department of Defense uses for enlistment, discharge, ID cards, medical screening, travel reimbursement, records corrections, and dozens of other personnel actions. The current, official versions are all posted as fillable PDFs on the Executive Services Directorate site at esd.whs.mil, which is the only authorized source for downloading them.1Washington Headquarters Services. DoD Forms Management Using anything else risks landing on a superseded edition, and agencies that receive an outdated form send it back.
Where to Download the Current Version
Go to esd.whs.mil, find the form by number, and download the PDF directly. Every DD form prints an edition date in its lower corner. Check that date against whatever is posted on the ESD site before you fill anything out; if you have an older copy saved on your computer, throw it away and pull a fresh one.
Active-duty personnel and DoD civilians with a Common Access Card can sign many of the fillable PDFs digitally. Veterans and other civilians without CAC access have to print the form, sign it in ink, and either mail it or scan the signed copy for electronic submission. Pencil is never accepted on any DD form. Use a typewriter, an ink pen, or a ballpoint.
The DD Forms People Actually Need
There are hundreds of DD forms, but a handful account for most of what service members, veterans, and their families run into.
DD Form 214 — Certificate of Release or Discharge
The DD 214 records your dates of service, rank at separation, awards, military education, and the characterization of your discharge. Federal law treats the discharge as incomplete until you or your next of kin receive this form, or a temporary memorandum in its place, along with your final pay.2Office of the Law Revision Counsel. 10 USC 1168 – Discharge or Release From Active Duty Limitations Employers, the VA, VA-loan lenders, and state veteran benefits offices all ask for it. DD Form 4 — Enlistment/Reenlistment Document
DD Form 4 is the contract between an enlisted member and the U.S. government.3MyNavy HR. MILPERSMAN 1070-230 – DD 4 Enlistment Reenlistment Document It states the term, the branch, and the obligations on both sides, and it becomes part of your permanent military personnel record where it feeds into promotion, reassignment, training, and pay decisions.4Department of Defense. DD Form 1172-2 — ID Card and DEERS Enrollment
Anyone who needs a military ID — active duty, reserve, retired, or dependent — starts with DD Form 1172-2. It enrolls you in the Defense Enrollment Eligibility Reporting System and triggers issuance of a CAC or a dependent ID. DEERS enrollment is the gateway to TRICARE, commissary and exchange access, and base entry. Dependents usually need their sponsor to complete part of the form and appear at a RAPIDS ID office in person with the required eligibility documents.5Department of Defense. Getting Your ID Card
DD Form 2807-1 — Report of Medical History
DD Form 2807-1 is the medical-history questionnaire used to decide whether an applicant is fit for service, and it also supports Medical Evaluation Boards deciding whether a current member’s condition warrants separation.6Department of Defense. Report of Medical History DD Form 2807-1 You mark yes or no across a long list of conditions, and every yes requires a written explanation. Vague answers slow accessions medical review more than almost anything else, so give specific dates, diagnoses, and treatment details for each item you flag.
DD Form 1351-2 — Travel Voucher
After official travel, DD Form 1351-2 is how you get reimbursed. Attach your travel orders, hotel receipts, and any single expense receipt of $75 or more. If you used a Government Travel Charge Card, you have to direct a split disbursement to the card contractor equal to your outstanding balance, and you must be enrolled in Electronic Funds Transfer to participate in split disbursement at all.7Department of Defense. Filling Them Out Without Getting Bounced
Most rejections trace back to small formatting mistakes. DD forms follow conventions that differ from civilian paperwork.
- Dates use YYYYMMDD format (20260415 for April 15, 2026) unless the form itself shows a different layout.
- Spell out your full middle name rather than using an initial, unless the form specifies otherwise.
- Have your Social Security number, DoD ID number, exact entry and separation dates, unit designations, and duty stations ready before you start.
- Sign in ink or digitally with a CAC. No pencil.
Each form’s header or instruction sheet identifies which fields are mandatory. Skipping a required field sends the form back, and on documents affecting pay, security clearances, or medical eligibility that delay has real consequences.
Read the Privacy Act Statement
DD forms that collect personal identifiers include a Privacy Act Statement. The Privacy Act of 1974 requires any federal agency asking for personal information to disclose the legal authority for collecting it, the purpose, how the data may be used, and what happens if you decline.8Office of the Law Revision Counsel. 5 USC 552a – Records Maintained on Individuals The statement also tells you whether a given item is mandatory or voluntary, which matters if you are hesitant about a particular disclosure.
Requesting a Copy of Your DD 214 or Other Records
Veterans who need a replacement DD 214 or other military personnel records request them from the National Personnel Records Center in St. Louis. Two routes are available.
Online Through eVetRecs
The fastest option is the eVetRecs portal at vetrecs.archives.gov, where you can start a request, check status, or retrieve a completed response.9National Archives. eVetRecs You verify your identity through ID.me before submitting a new request or downloading a response electronically.10National Archives. By Mail or Fax Using SF-180
If you would rather use paper, download Standard Form 180 from archives.gov, fill it out, print it, and sign it in ink. Federal rules require a handwritten signature and a date within the last year on every written request for military personnel records.11National Archives. How Long It Takes
Wait at least 90 days before sending a follow-up. Duplicate requests actually slow the process down.11National Archives. Request Military Personnel Records Using Standard Form 180 Records touched by the 1973 fire at the NPRC, which destroyed millions of Army and Air Force personnel files, may require reconstruction and take considerably longer. If your file might fall in that group, include your place of discharge, last unit of assignment, and place of entry to help the search.10National Archives. Request Military Service Records
Fixing Errors on Your Records
Errors happen more often than you would expect: a misspelled name, a missing award, a discharge characterization that doesn’t reflect the facts. The right form depends on what needs fixing and how long ago you separated.
DD Form 215 for Minor DD 214 Corrections
Small administrative errors on a DD 214, such as a typo, a missing decoration, or a wrong date, are corrected through DD Form 215, which attaches to the original as an official amendment. The National Archives no longer produces DD 215 documents itself; the correction has to come through your service branch.12National Archives. Correcting Military Service Records
DD Form 293 for Discharge Review
If you believe the characterization or reason for your discharge was unjust, DD Form 293 applies to your branch’s Discharge Review Board. You have 15 years from your discharge date to file. After 15 years the Discharge Review Board can no longer hear the case, and you have to apply to the Board for Correction of Military or Naval Records instead, using DD Form 149.13Department of Defense. DD Form 149 for Broader Corrections
DD Form 149 covers any error or injustice in a military record: pay and allowances, decorations, evaluations, promotions, disability determinations, and discharge characterization once the DRB window has closed.14U.S. Department of War. Request Correction of Military Records Under 10 U.S.C. § 1552 you have three years from the date you discovered the error to file. The board can waive that deadline if it finds doing so is in the interest of justice, but you have to explain the delay in your application.15Office of the Law Revision Counsel. 10 USC 1552 – Correction of Military Records Claims Incident Thereto Your submission needs enough supporting evidence to show a probable error or injustice, and if you were denied before you can only resubmit with new evidence the board did not previously consider. Anyone seeking a discharge upgrade who separated less than 15 years ago has to go through the DRB on DD Form 293 first.
Penalties for False Statements
Submitting false information on a DD form is a federal crime. Under 18 U.S.C. § 1001, knowingly making a false statement or concealing a material fact on a document submitted to a federal agency carries a fine and up to five years in prison; if the false statement involves terrorism, the maximum is eight years.16Office of the Law Revision Counsel. 18 USC 1001 – Statements or Entries Generally The DD 1351-2 travel voucher prints its own warning about criminal and civil penalties under 18 U.S.C. §§ 287 and 1001 and the civil False Claims Act at 31 U.S.C. § 3729.7Department of Defense. Travel Voucher or Subvoucher DD Form 1351-2 Beyond the criminal exposure, false statements can lead to administrative separation, loss of benefits, and a permanent mark on your record. An honest mistake is worth correcting. Lying about it is not.
Active-Duty Members Use Branch Portals
If you are still on active duty, you generally do not mail forms to the NPRC. Each branch runs a secure personnel portal for managing records electronically. The Army uses the Integrated Personnel and Pay System-Army (IPPS-A), a web-based platform available around the clock that consolidates personnel data in one place.17Defense Technical Information Center. These portals handle Social Security numbers and other sensitive data at a higher security level than standard email and create a verifiable audit trail for every submission.
