How Much Does the Zepbound Savings Card Cover? Caps and Eligibility

The Zepbound savings card is Eli Lilly’s manufacturer discount for tirzepatide, and what it saves you depends on your coverage. With commercial insurance that covers Zepbound, you can pay as little as $25 per fill on the single-dose pen. With commercial insurance that doesn’t cover it, or no insurance at all, you can pay between $299 and $449 per month on the KwikPen or vial. If you have Medicare, Medicaid, or any other government-funded coverage, you can’t use the card.

If Your Commercial Plan Covers Zepbound

Patients with commercial drug insurance that covers Zepbound and who fill the single-dose pen pay as little as $25 per prescription, whether the fill is a one-, two-, or three-month supply. The card pays the difference between your copay or coinsurance and that $25 floor, up to a cap that scales with the fill size:1Zepbound. Zepbound Savings

  • One-month fill: up to $100 in savings
  • Two-month fill: up to $200 in savings
  • Three-month fill: up to $300 in savings

The annual cap is $1,300 per calendar year across a maximum of 13 fills. A “month” means 28 days and up to four single-dose pens.1Zepbound. Zepbound Savings

An earlier version of the program carried higher limits of $150 per monthly fill and $1,950 per year, and those figures still appear on some third-party sites.2Drugs.com. If Your Commercial Plan Doesn’t Cover Zepbound

You can still use a savings card, but the pricing works differently. Instead of a $25 copay, you pay a reduced flat rate on the KwikPen, which varies by dose:

  • 2.5 mg: as low as $299 per month (up to $215 in savings)
  • 5 mg: as low as $399 per month (up to $115 in savings)
  • 7.5 mg: as low as $449 per month (up to $65 in savings)
  • 10 mg, 12.5 mg, or 15 mg: as low as $449 per month (up to $271 in savings)

This tier allows up to 11 fills per calendar year. For the 7.5 mg dose and above, holding the $449 price requires refilling within 45 days of the previous prescription’s delivery. Miss that window and the price rises to $499 for the 7.5 mg dose and $699 for 10 mg through 15 mg.1Zepbound. Zepbound Savings

If You Have No Insurance

The self-pay program uses the KwikPen or single-dose vial format at retail pharmacies or through LillyDirect, Lilly’s direct-to-patient pharmacy with free home delivery. Monthly prices start at $299 for the 2.5 mg dose, $399 for 5 mg, and $449 for 7.5 mg through 15 mg when refills are timely.3Lilly. LillyDirect Zepbound

Through LillyDirect, the discount applies automatically at checkout. At other pharmacies, you present the KwikPen Self-Pay Savings Card. Self-pay patients are capped at 11 fills per calendar year and cannot submit the cost to any insurer or apply it to a deductible.1Zepbound. Zepbound Savings

Matching the Card to the Pen or Vial

Zepbound comes in three formats, each tied to a specific savings pathway. Presenting the wrong card at the pharmacy won’t work.

  • Single-dose pen: used with the standard savings card for commercially insured patients, whether on the $25 copay program or the not-covered tier.
  • KwikPen (multi-dose, single patient): used with the KwikPen Self-Pay Savings Card for cash payers, or the not-covered commercial card.
  • Single-dose vial: available through LillyDirect at self-pay pricing, with savings applied automatically.

If your plan covers Zepbound, the single-dose pen is the cheapest route because it unlocks the $25 copay. The KwikPen and vial are built around the self-pay and not-covered markets.4Zepbound HCP. Coverage and Savings

Who Can’t Use the Card

You must be at least 18, live in the United States or Puerto Rico, and have a prescription for an FDA-approved use of Zepbound: chronic weight management, or moderate-to-severe obstructive sleep apnea in adults with obesity or overweight.2Drugs.com. What Is the Zepbound Coupon Savings Card

The largest exclusion is government coverage. Patients enrolled in Medicare (Part D, Advantage, or Medigap), Medicaid, TRICARE, VA benefits, Department of Defense programs, or any state pharmaceutical assistance program cannot use the savings card.5Lilly. You’re also ineligible if your plan uses what Lilly calls an “alternate funding program,” meaning the insurer requires you to use a manufacturer copay card as a condition of coverage, or changes your out-of-pocket costs based on whether manufacturer help is available.1Zepbound. Will the Savings Count Toward Your Deductible?

Not always. A growing number of commercial plans use copay accumulator or copay maximizer programs. Under these arrangements, what the savings card pays toward your prescription doesn’t count toward your deductible or out-of-pocket maximum. Once the card’s annual limit runs out, you owe the full remaining cost-share as though no payments had been applied.

Lilly’s terms address this directly: the company may reduce or eliminate card benefits if your plan doesn’t apply card payments toward your copay, deductible, or coinsurance.1Zepbound. Zepbound Savings As of late 2024, 21 states and Puerto Rico have banned copay accumulator programs, though those bans may not reach copay maximizer arrangements.7Crohn’s & Colitis Foundation. If You’re on Medicare or Medicaid

You can’t use the savings card, but you have other options.

Medicare Part D may cover Zepbound when it’s prescribed for obstructive sleep apnea, subject to the individual plan’s formulary and usually prior authorization. Medicare does not cover Zepbound when used solely for weight loss.8American Sleep Apnea Association. Does Medicare Cover Zepbound for Sleep Apnea

For weight management, the Medicare GLP-1 Bridge, a short-term federal demonstration, launched on July 1, 2026, and runs at least through the end of the year. It covers the Zepbound KwikPen, Wegovy, and Foundayo for weight reduction at a flat $50 monthly copay. You must be enrolled in a Part D or Medicare Advantage prescription drug plan and meet BMI-based prior authorization criteria (generally a BMI of 30 or higher with certain comorbidities, or 35 or higher on its own).9CMS. Medicare GLP-1 Bridge The Bridge is intended to transition into the broader BALANCE Model for Medicare Part D, though that launch timeline is uncertain.10Medicare Rights Center. GLP-1 Weight Loss Drug Demonstration Begins July 2026

If you don’t qualify for the Bridge, you can still buy Zepbound through LillyDirect at the standard self-pay prices starting at $299 per month. You can’t layer the savings card on top of that.1Zepbound. Zepbound Savings The Patient Advocate Foundation runs an obesity financial assistance fund for Medicare, Medicaid, and military-benefit patients with household incomes at or below 500% of the federal poverty level, offering up to $2,000 per year toward copays, coinsurance, and deductibles. That program is moving to a new platform called TotalAssist as of July 1, 2026.11Patient Advocate Foundation. Obesity Fund Zepbound is not currently included in Lilly Cares, Lilly’s charitable patient assistance program for low-income patients.12Lilly Cares. How To Apply

How To Get the Card and Use It at the Pharmacy

When the Current Terms Expire

Every version of the Zepbound savings card expires on December 31, 2026. Lilly reserves the right to change the terms, reduce savings, or end the program at any time without notice. The company already dropped the commercial annual cap from $1,950 to $1,300 and the monthly cap from $150 to $100 before the current program year, so verify the current terms on the official Zepbound site before counting on a specific number.1Zepbound.
