To get IRMAA reduced, file Form SSA-44 with the Social Security Administration and show that a qualifying life-changing event has lowered your income since the tax year SSA used to set your premium. The form only works for specific events on SSA’s list — retirement, death of a spouse, divorce, marriage, loss of pension, involuntary loss of income-producing property, or an employer settlement payment.1Social Security Administration. Request to Lower an Income-Related Monthly Adjustment Amount (IRMAA) If your event doesn’t fit that list, SSA-44 isn’t the path; an amended tax return may be, and a denial can be appealed.
Why Your Premium Is Based on Old Income
Social Security calculates IRMAA using your modified adjusted gross income from two years earlier. Your 2026 premium is set from your 2024 tax return. MAGI here means adjusted gross income (line 11 of Form 1040) plus tax-exempt interest (line 2a), so municipal bond interest counts even though it’s federally tax-free.2Social Security Administration. The lookback is the whole reason SSA-44 exists. If you worked through 2024 and retired in 2025, your 2024 return still shows a full salary, and SSA would otherwise charge you a surcharge based on income you no longer earn. Before filing, check the 2026 brackets against your current income: your income has to drop enough to move you into a lower tier, because staying inside the same tier won’t change what you pay.3GovInfo. 20 CFR 418.1205 The single-filer threshold where any surcharge kicks in for 2026 is $109,000; the joint threshold is $218,000.4Centers for Medicare & Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles
Which Life-Changing Events Qualify
SSA maintains a specific list, and events outside it don’t qualify no matter how much your income changed.1Social Security Administration. Request to Lower an Income-Related Monthly Adjustment Amount (IRMAA) The seven accepted events are:
- Work stoppage or reduction — you or your spouse retired, were laid off, or significantly cut hours
- Death of a spouse
- Divorce or annulment
- Marriage
- Loss of pension income from plan termination, reorganization, or a benefit reduction
- Involuntary loss of income-producing property (disaster, arson, disease destroying livestock or crops, or investment fraud)
- Employer settlement payment for back pay that inflated a prior year’s income
Retirement is by far the most common ground for a successful request. If your 2024 return pushed you into a high tier and you left the workforce in 2025, filing SSA-44 lets the agency use your actual post-retirement income instead.5Social Security Administration. What Doesn’t Qualify
A one-time income spike from selling a rental property, cashing in stock, or converting a traditional IRA to a Roth is not a life-changing event. Even though the spike is a single-year event, SSA treats these as ordinary income and won’t reduce your premium because of them.
The Form SSA-44 instructions state that loss of income-producing property only counts when the loss was involuntary and “not at your direction (e.g., not due to the sale or transfer of the property).”5Social Security Administration. Medicare Income-Related Monthly Adjustment Amount – Life-Changing Event Form SSA-44 A capital gain from selling investment property is voluntary, so it has no remedy through SSA-44. Same for large Roth conversions: the income was real and elective, and the corresponding IRMAA year is locked in once it hits your return. Planning matters here. If you’re considering a conversion or a major sale, timing it against the two-year lookback or spreading it across years is the only way to control the surcharge.
How to File Form SSA-44
The form itself asks three things: which event happened, when it happened, and what your income actually looks like now. You provide an estimated adjusted gross income and tax-exempt interest for the affected year. Simple to fill out, but the documentation you attach is what carries the request.
Proof to Attach
Documentation requirements are event-specific.5Social Security Administration. Medicare Income-Related Monthly Adjustment Amount – Life-Changing Event Form SSA-44
- Work stoppage or reduction: a signed employer statement, pay stubs showing the change, or business-transfer documents
- Death of a spouse: a certified death certificate or certified public record of death
- Divorce or annulment: a certified copy of the decree or annulment order
- Loss of pension: a letter from the plan administrator explaining the reduction or termination
- Loss of income-producing property: an insurance adjuster’s statement of loss or a government letter documenting it; if the cause was investment fraud, add proof of a criminal conviction for theft or fraud
Include your most recent tax return. SSA cross-checks your estimate against IRS data, and a large mismatch delays processing or produces a denial.

What Happens After You File
An SSA representative reviews your documentation against the qualifying-event criteria and your reported income. There is no officially published processing timeline, so expect several weeks and keep copies of everything.
If SSA approves, you get a revised determination notice showing your new premium and the effective date. When the reduction reaches back to months you already overpaid, SSA makes retroactive corrections.7Social Security Administration. POMS HI 01194.110 – IRMAA Annual Verification Overage Notice In practice, the overpayment is usually applied as a credit against future premiums rather than refunded as a check.
If SSA denies the request, the notice explains why. The usual reasons: the event isn’t on SSA’s list, documentation is missing, or the income estimate doesn’t actually move you into a lower tier.
Appealing a Denial
The first level of appeal is reconsideration. You have 60 days from receiving the denial notice to file.8Social Security Administration. Request Reconsideration Use Form SSA-561-U2, which you can submit through your my Social Security account or by mail. Resubmitting the same package rarely produces a different result.
If reconsideration also goes against you, you can request a hearing before an Administrative Law Judge at the Office of Medicare Hearings and Appeals within 60 days, and continue upward to the Medicare Appeals Council and eventually federal district court.10Department of Health and Human Services. If the Problem Is an Error on Your Tax Return
SSA-44 is for life-changing events. If your IRMAA is high because your original return overstated your income — a missed deduction, a misreported distribution, a data entry error — the fix is an amended return with the IRS, not SSA-44. SSA receives updated IRS data periodically, but you can speed things up by calling 1-800-772-1213 after the IRS has accepted your amendment and letting SSA know the corrected figures are available.1Social Security Administration. Request to Lower an Income-Related Monthly Adjustment Amount (IRMAA) The IRS acceptance has to come first; SSA can’t act on a return the IRS hasn’t yet processed.
