Where to Get the Form

The CS-L(MVR)82 is a single-page PDF on the Department of Customer Services forms page at honolulu.gov, under the Motor Vehicle Registration section.2City and County of Honolulu. Department of Customer Services Forms Blank copies are also available at any Satellite City Hall counter.

What to Fill In

Have your current registration card in front of you. The form asks for:

  • License plate number, exactly as it appears on your registration.
  • The full 17-character Vehicle Identification Number, found on the registration card or on the metal plate at the base of the driver’s side windshield.
  • The legal name or names of every registered owner as shown on the title.
  • Your previous address on file with the county.
  • The new address where you want future registration correspondence sent.
  • Signatures from every registered owner listed on the vehicle.

If you own more than one vehicle, fill out a separate form for each. Recheck the VIN before you submit. A single wrong digit will keep the county from matching your record, and the address change will not go through.

How to Submit

Two options, both offline. The city’s online renewal system and self-service kiosks require the correct address to already be on file, so they cannot be used to make this change.3City and County of Honolulu. Motor Vehicle Registration

The 30-Day Deadline and What a Lapse Costs

Hawaii Revised Statutes Section 286-47.5 requires a registered owner to notify the county director of finance in writing within 30 days of an address change. Violating the requirement carries a fine of up to $25.1Justia. Hawaii Code 286-47.5 – Notice of Change of Address

The bigger practical risk is a missed registration renewal. If the county mails your renewal notice to the old address and you never see it, late renewal on a passenger vehicle adds a flat $16 annual penalty; commercial vehicles are hit with $40.3City and County of Honolulu. Motor Vehicle Registration Driving on expired tags can also draw a citation.

Confirming the Change Went Through

The county does not usually send a confirmation. The clearest sign the update took hold is your next registration renewal notice arriving at the new address. If renewal is months away and you want reassurance sooner, call the Division of Motor Vehicle, Licensing and Permits and ask them to read back the address they have on file.

If a renewal notice shows up at your old address or never arrives, contact the division right away. Clerical errors happen during data entry, and missing a renewal because of one does not excuse the late penalty.

This Form Does Not Change Your Driver’s License Address

CS-L(MVR)82 updates vehicle registration records only. Your Hawaii driver’s license or state ID address is handled separately by the same Department of Customer Services, and that update requires an in-person visit with an appointment booked through alohaq.org.5Department of Customer Services.
