Form AD-2047 Customer Data Worksheet: Filling, Signing, and Submitting

USDA Form AD-2047, the Customer Data Worksheet, is the one-page intake form that registers you in the Department of Agriculture’s shared customer database. To fill out Form AD-2047, complete Part A with your legal name, taxpayer identification number, contact information, citizenship status, and the USDA agencies you plan to work with, then sign, date, and deliver it to your local USDA Service Center. Part B is filled in by the Service Center employee who processes your record, so you can leave it blank. The form itself takes about three minutes once you have your documents in front of you.

What to Have on Hand Before You Start

Pull these together first. The form moves quickly once you have them.

  • Your taxpayer identification number. That’s your Social Security Number if you’re filing as an individual, or an EIN if you’re filing for a corporation, LLC, trust, or estate.   • Mailing address with ZIP code, plus home, business, and mobile phone numbers, and an email address.
  • Citizenship or residency documentation if you’re an individual. Resident aliens need their Permanent Resident Card (Form I-551). If you’re neither a U.S. citizen nor a resident alien, you’ll list your country of citizenship.3U.S. Department of Agriculture. USDA Form AD-2047

Filling Out Part A

Item 1 — Reason for Request. Check “New Customer” if you’ve never registered with USDA. Check “Update Existing Customer Record” if you’re changing something already on file.1U.S. Department of Agriculture. USDA Form AD-2047

Items 2A–2G — Contact information. Enter your full legal name or business name, mailing address, business type (individual, corporation, LLC, estate, trust, and so on), phone numbers, and email. Item 2G asks whether you consent to receive farm-specific emails from USDA that may contain sensitive but non-personally-identifiable information. Answer yes or no.

Items 3A–3D — Identification. Enter your TIN and mark whether it’s an SSN, EIN, or ITIN. Item 3B asks for a birthdate only when the customer is a minor. Item 3C is your citizenship status: U.S. citizen, resident alien, or neither. Foreign entities skip 3C and complete Item 3D with their country of origin instead.1U.S. Department of Agriculture. Items 4A–4C — Race, ethnicity, and sex. These fields are collected for civil rights reporting. Answering is voluntary, and you can select all that apply for race and ethnicity.

Item 5 — Agency interest. Check every USDA agency you expect to work with: AMS, FSA, NRCS, RMA, or RD. More than one is fine.

Item 6 — Multi-county producer. If you farm or ranch across county lines, check yes and list the states and counties where you operate. This lets USDA coordinate your record across offices.1U.S. Department of Agriculture. USDA Form AD-2047

Signing the Form

Items 7A through 7C are the signature block. Sign, print your title or relationship to the entity if you’re signing in a representative capacity (President, Trustee, Managing Member, and so on), and date the form. Your signature certifies the information is accurate. When Someone Else Signs for You

An individual who wants a representative to sign the AD-2047 must first have a completed Form FSA-211 (Power of Attorney) on file with the servicing USDA office. The FSA-211 has to be notarized unless it’s witnessed by an FPAC employee or bears the corporate seal of the granting entity. The grantor specifies on the FSA-211 which programs and actions the representative can handle, and the authority stays in effect until it’s revoked in writing, the grantor dies, or the grantor becomes incapacitated.6Farm Service Agency. Submitting the Form

Deliver the completed AD-2047 to the USDA Service Center that serves the area where you operate. If you work across multiple counties, submit it to the office that administers your primary records. If you already have USDA electronic access credentials, you can submit through the USDA eForms site. Electronic submission is allowed when you’re the only signer required, when the person signing for you has an approved FSA-211 on file, or when an authorized representative of a legal entity signs.8Farm Service Agency. Updating Your Record Later

The AD-2047 is not a one-time filing. Any time your name, address, TIN, business structure, or other identifying information changes, submit a new form with “Update Existing Customer Record” checked in Item 1. Fill in only the fields that changed, along with the last four digits of your TIN so the office can locate your existing record.1U.S. Department of Agriculture.
