Form 15119: Response to Notice CP54, Filing and Submission

Form 15119 is the one-page response form the IRS asks you to send back after Notice CP54, and completing it correctly is what lifts the hold on your refund. You mark which of three situations applies to you, enter your name and identification number exactly as they appear on your Social Security card or CP565 ITIN Assignment Notice, attach the supporting documents that fit your situation, sign, and return the package through the IRS Document Upload Tool or by mail to the address printed on your notice.1Internal Revenue Service. Why the IRS Sent You a CP54 Notice

The IRS issues CP54 (and its variants, including CP54B and CP54E) when the name or taxpayer identification number on your return does not match what the Social Security Administration or the IRS’s ITIN database has on file. Your refund is held until the conflict is cleared. The notice shows both versions side by side, so the source of the mismatch is usually visible on the page: a recent marriage, a legal name change that never made it to the SSA, a typo or transposed digit, or an ITIN name that doesn’t match your CP565.2Internal Revenue Service. Identifying which of those describes your situation matters, because it determines which box you check in Section 1 and which documents you attach.

Filling Out Form 15119

The form has six numbered sections on a single page. You can use the copy that came with your notice or download a fresh one.1Internal Revenue Service. Form 15119 – Response to Notice CP54

Section 1: Pick the Situation That Applies

Three checkboxes, and you choose one.

  • Your Social Security card or CP565 notice is correct, but your tax return information doesn’t match. This is the most common case, covering typos on the return and returns filed under a name you had not yet registered with the SSA.
  • Your tax return is correct, but your Social Security card is wrong. Use this if you have legally changed your name, filed under the new name, and haven’t updated the card yet.
  • Both your return and your Social Security card or CP565 notice are correct. This points to outdated records inside the IRS or SSA. Check the box, complete the rest of the form, and call the IRS at 1-800-829-1040 so the agency can update its files.

Section 2: Enter Your Name and ID Number

Write your Social Security number or ITIN and your full legal name exactly as they appear on your Social Security card or CP565 notice. First name, middle name, and last name each go in their own field. The IRS compares character by character, so a dropped middle name or an initial in place of a full name can push you into another round of correspondence.

Section 3: Explain the Discrepancy

Use the blank space to describe, briefly and factually, why the return and the records didn’t line up. One sentence is enough. Something like “I married in June 2025 and filed under my new last name before updating my Social Security card” gives the reviewer what they need. Don’t leave it blank.

Section 4: Contact Information

Enter your current address only if it has changed since you filed. If it hasn’t, you can skip this section.

Section 5: Sign and Date

Your signature confirms that the identifying information on the form is what the IRS should use for your return.

Documents to Attach

What you attach depends on the box you checked in Section 1.1Internal Revenue Service. Form 15119 – Response to Notice CP54

  • SS card or CP565 is correct: attach a copy of your Social Security card or CP565 ITIN Assignment Notice.
  • Return is correct, SS card is wrong: attach copies of legal documents supporting the name on your return, such as a marriage certificate, divorce decree, or court-ordered name change.
  • Both are correct: no specific attachment is required, though a copy of your Social Security card doesn’t hurt.

Send copies only. Original documents mailed to the IRS are not guaranteed to come back.

Submitting the Form

IRS Document Upload Tool

The upload tool is the faster route. You scan or photograph the completed form and the attachments as JPG, PNG, or PDF files and submit them through a secure portal. Your CP54 notice should include an access code; if it doesn’t, you can enter the notice number instead, along with the name shown on the notice and your SSN or ITIN. The tool confirms receipt, which gives you a record that the IRS has your response.3Internal Revenue Service. IRS Document Upload Tool

Mail

If you’d rather mail your response, use the address printed on your CP54 notice rather than a general IRS address. Different notices route to different processing centers, and sending it to the wrong place adds delay. Certified mail with return receipt gives you proof of delivery.

What to Expect After You Respond

Once the IRS matches the information on your form to its records, your refund should arrive in four to six weeks. If nothing has moved after six weeks, check “Where’s My Refund?” on IRS.gov or call 1-800-829-1040. If the information still doesn’t reconcile, the IRS will send another notice asking for more clarification, which usually means the attachments didn’t resolve the conflict or the underlying SSA or ITIN records still need to be corrected before your return can process.2Internal Revenue Service. Fix the Underlying Record So It Doesn’t Recur

Sending Form 15119 releases your refund for this year, but if the mismatch traces back to the SSA, the same notice can turn up next filing season. To update your name with the SSA, file Form SS-5 (Application for a Social Security Card) at your local Social Security office with original or certified copies of documents showing the reason for the change, such as a marriage certificate, divorce decree, or court order. There is no fee for a corrected card.4Social Security Administration. Application for Social Security Card

ITIN holders update their records with the IRS ITIN Operation rather than the SSA, since the CP565 assignment sits inside the IRS. Once your card or CP565 matches what you plan to put on future returns, the CP54 trigger goes away.
