Forklift PM Checklist: OSHA Rules, Intervals, and Wear Limits

A forklift PM checklist is the form a technician or operator works through to confirm every safety-critical system on a powered industrial truck is sound. It runs on two clocks: a daily pre-shift examination the operator performs before the truck goes into service, and deeper technician services triggered by the hour meter at roughly 250, 500, and 2,000 hours. Federal rules require the daily check and require any unsafe truck to be pulled from service, but the hour-based intervals themselves come from manufacturer schedules and industry practice.1eCFR. 29 CFR 1910.178 – Powered Industrial Trucks

The Daily Pre-Shift Check

The daily examination belongs to the operator, not the maintenance shop. It happens before the truck leaves its parking area and takes five to ten minutes when nothing is wrong. Facilities running around the clock have to examine each truck after every shift rather than once a day.1eCFR. 29 CFR 1910.178 – Powered Industrial Trucks

The checklist splits into what you can see with the truck off and what you test with it running.

Visual walk-around:

  • Forks: cracks, bending, and uneven wear at the heel where the blade meets the shank.
  • Tires and wheels: chunking, flat spots, cuts, excessive wear.
  • Mast and chains: damaged rollers, dry or kinked chains, hydraulic leaks around the lift cylinders.
  • Fluid leaks under the truck and around hoses.
  • Safety equipment: overhead guard intact, seat belt working, warning decals legible.
  • Lights and horn: headlights, taillights, strobes, and the audible warning device all working.

Operational test:

  • Steering through its full range for stiffness, dead spots, or unusual noise.
  • Service brake and parking brake — the truck should stop smoothly without pulling.
  • Lift and tilt through the full range, listening for grinding or hesitation.
  • Gauges: hour meter running, battery charge indicator (electric) or engine temperature (IC) in normal range.

Hour-Based Technician Services

Daily checks catch obvious problems. Hour-based PM catches the gradual wear that causes them. The meter is a more accurate trigger than the calendar because a two-shift truck burns hours far faster than one used a few times a week.

250-Hour Service

The routine tune-up. On internal combustion (IC) trucks, change the engine oil and filter. Inspect and clean the air filter. Lubricate all grease points. Check every fluid level. Inspect mast chains for stretch and adjust tension. Test all controls and safety devices.

500-Hour Service

Everything in the 250-hour list plus deeper mechanical checks. Replace the fuel filter on IC units and inspect drive belts for cracking or glazing. Give brake components a thorough inspection. On electric trucks, check battery electrolyte levels and clean terminals. Wheel bearings, steering linkage, and suspension also get attention.

2,000-Hour Service

The annual overhaul for a high-use truck. Change the hydraulic oil, differential oil, and coolant (IC). Flush the brake system and replace the fluid. Inspect and service the carriage rollers. Verify instrument calibration. This is the natural point for a full safety compliance inspection against the manufacturer’s specifications.

These intervals are industry standards rather than federal requirements. OSHA requires the truck to be maintained in safe condition but does not prescribe a specific hour-based schedule. The manufacturer’s service manual is authoritative for the exact tasks, torque values, and fluid specs for your model. Adjust for environment: a truck in a dusty grain facility or an outdoor lumber yard needs air filter service far more often than one in a clean warehouse.

What Belongs on the PM Form

The header captures identifying information: make, model, serial number, current hour meter reading, date, and technician name. Getting the serial number right matters more than it sounds. In a twenty-truck fleet, mixing up records means one unit quietly skips maintenance while another gets serviced twice.

Below the header, organize the form by system so nothing gets missed:

  • Engine or motor: oil level and condition, coolant level (IC), air filter, belt tension, exhaust system, battery charge and connections (electric).
  • Hydraulic system: fluid level and condition, cylinder and hose integrity, filter condition, lift and tilt function under load.
  • Transmission and drivetrain: fluid level, shift quality, axle condition, differential oil.
  • Brakes: pad or shoe thickness, pedal travel, parking brake holding power, brake fluid level.
  • Steering: responsiveness, fluid level, tie rod and linkage condition.
  • Mast and carriage: chain condition and tension, roller wear, fork condition and alignment, backrest integrity.
  • Tires and wheels: wear pattern, tire pressure (pneumatic), lug nut torque.
  • Safety equipment: horn, lights, backup alarm, seat belt, overhead guard, data plate legibility.

Each line gets a pass/fail notation, a measurement where one applies, and space for comments about anything that needs follow-up. The technician signs at the bottom and the completed form goes to the maintenance supervisor.

Wear Limits That Retire a Component

Two items have defined thresholds many operators don’t know about, and both deserve a dedicated line on the checklist.

Fork Wear

Under the ANSI/ITSDF B56.1 standard, a fork must be pulled from service when the blade or shank thickness has worn to 90 percent of its original thickness. That 10 percent loss represents a significant drop in load capacity. Wear concentrates at the heel, where the vertical shank meets the horizontal blade, so that’s the measurement point. A caliper and the original thickness spec are all you need.

Tire Wear

Cushion (press-on) tires have a wear line molded into the rubber, often called the 60J line or safety line. When the rubber above that line is gone, the tire needs immediate replacement. A common rule of thumb for cushion tires is the 33 percent rule: replace once the tire has lost more than one-third of its original rubber height. Also look for chunking, sidewall cuts, flat spots from repeated hard stops, and any areas where rubber is torn or missing. Pneumatic tires should be checked with a tread depth gauge and inspected for punctures and sidewall damage.

Extra Lines for Electric Trucks

Electric forklifts add a battery section. Skipping battery care is one of the fastest ways to destroy an expensive component.

Watering happens after charging, not before, using deionized water. Tap water contains minerals and chlorine that corrode the plates and shorten battery life. Fill each cell to about a quarter inch below the bottom of the vent well. Overfilling causes boil-over during charging, spreading corrosive acid across the battery top and the truck’s electrical components.3Crown Equipment. An equalization charge, a controlled overcharge that balances voltage across all cells, should happen roughly once a week. Without it, individual cells drift out of balance and weaker cells sulfate, dragging down total capacity. The checklist should have a line for the last equalization date and the next scheduled one.

Also inspect terminal connections for corrosion, cables for fraying or exposed wire, and the battery restraint system for a secure hold in the compartment. Do Checklists Have to Be Written?

A 2000 OSHA interpretation letter states plainly that pre-operation forklift examinations are not required to be documented, and the regulation does not specify a retention period for PM records.5Occupational Safety and Health Administration. If an operator is injured and OSHA investigates, the first thing they ask for is maintenance records. A company with a complete paper trail showing consistent PM service is in a fundamentally different position than one that says the inspections happened but nobody wrote them down.

Keep completed PM checklists, daily inspection forms, and repair work orders for at least the life of the equipment, searchable by serial number and date. A physical logbook on or near each truck gives operators quick access to recent service history and helps them spot when a truck is overdue.

Every completed checklist should carry the truck’s serial number, the hour meter reading, the date, the technician’s name and signature, and a clear pass/fail for each inspection point. When a deficiency is found, the record should note whether the truck was pulled from service and when the repair was completed. That level of detail is what turns a checklist from a formality into legal protection.

Who Can Do the Work

OSHA’s language is simple: repairs must be made by “authorized personnel.”1eCFR. 29 CFR 1910.178 – Powered Industrial Trucks The regulation does not define a specific certification, but the expectation is that the person has the training and competence to do the work safely and correctly. Operators who handle battery changing, propane tank swaps, or fueling need training specific to those activities. Most facilities either employ trained in-house mechanics or contract with the dealer’s service department for major PM work.

Whoever does the service also has to follow the general industry standards that ride along with it: lockout/tagout under 29 CFR 1910.147 before working on the engine, hydraulic system, or electrical components; hazard communication for chemicals handled during service; and the asbestos standard for brake and clutch work on older trucks that may contain asbestos.7Occupational Safety and Health Administration.
