FDA Form 3514 is a summary cover sheet that helps the Center for Devices and Radiological Health (CDRH) categorize and route medical device submissions. It sits at the front of the package so reviewers can identify the submission type, the product, and the responsible applicant without opening the full filing.1Federal Register. One boundary to clear up first: Form 3514 is a device form. Drug and biologic applications use Form FDA 356h and go to the Center for Drug Evaluation and Research, not CDRH.2Food and Drug Administration. Submissions That Use Form 3514
The cover sheet supports the full range of CDRH premarket programs. The most common use is with 510(k) premarket notifications, where a manufacturer shows that a new device is substantially equivalent to one already legally marketed. - Investigational Device Exemptions (IDEs) for devices that have not yet been cleared or approved but need to be studied in a clinical trial.
- De Novo requests for novel, lower-risk devices with no legally marketed equivalent.
- Humanitarian Device Exemptions (HDEs) for devices treating or diagnosing conditions affecting fewer than 8,000 people per year in the United States.
- CLIA Waiver applications to classify an in vitro diagnostic as waived under the Clinical Laboratory Improvement Amendments.
- Q-Submissions, including pre-submission meeting requests and other feedback interactions with CDRH staff.
- Emergency Use Authorizations (EUAs) during a declared public health emergency.
- 513(g) requests asking the FDA to classify a device or confirm its regulatory pathway.

How to Complete Each Section
Start with the submission type. The categories on the form mirror the device programs listed above, and this selection drives how CDRH routes and prioritizes your filing. Get it wrong and the package can land in the wrong review division before anyone with the right expertise sees it.
Next, fill in the contact information for the responsible applicant or regulatory representative. If you are an authorized agent submitting on behalf of the manufacturer, use the contact details for the party CDRH should reach with questions or review decisions. The name and address here should match the applicant information in the body of the submission; mismatches create confusion when the agency sends deficiency letters.
Enter the device’s trade name and common name so reviewers can identify the product quickly. For submissions that build on a previously cleared or approved device, list the predicate device information and any cross-referenced submission numbers. In a 510(k), that linkage is central to the substantial equivalence determination and should not be left for reviewers to reconstruct.
Adding Your User Fee Payment ID
Most device submissions carry a user fee under the Medical Device User Fee Amendments (MDUFA). After you complete the applicable MDUFA cover sheet through the FDA’s online system, the system issues a unique payment identification number beginning with “MD” that ties your fee to your submission.5Food and Drug Administration. If you pay by wire, add the bank’s transfer fee to the amount and include the “MD” number with the wire so the FDA can match the payment to the filing.
Electronic Signature Rules
When Form 3514 is signed as part of a digital filing, the signature has to comply with 21 CFR Part 11. That means the signed record shows the signer’s printed name, the date and time of signing, and what the signature means: authorship, review, or approval. Submitting Through ESG NextGen
The FDA’s Electronic Submissions Gateway Next Generation (ESG NextGen) is the primary portal for transmitting device submissions. It is the single entry point for securely sending, receiving, acknowledging, and routing regulatory submissions to the agency.7Food and Drug Administration. Electronic submission is not optional for most device types. Under section 745A(a) of the Federal Food, Drug, and Cosmetic Act, the FDA will not file or receive a submission that fails to meet the required electronic format specifications unless you have obtained a waiver or the submission type qualifies for an exemption.8Food and Drug Administration. Mistakes That Delay Review
The most frequent error is marking the wrong submission type. A 510(k) checked as a PMA, or the reverse, sends the package to the wrong review division and builds in delay from day one.
Contact information that does not match the body of the submission causes the same kind of trouble further downstream, when the agency cannot tell who to write to about deficiencies.
The third common problem is the user fee payment ID: missing entirely, or present but not corresponding to the fee the FDA has on record. Confirm the “MD” number on your cover sheet matches the payment on file before you transmit through ESG NextGen. A slow read through every field, checked against the underlying application, is the simplest way to keep an administrative issue from holding up a filing that has nothing wrong with the device itself.
