Does UnitedHealthcare Cover Semaglutide for Weight Loss?

UnitedHealthcare does cover semaglutide for weight loss, but only on plans where the employer or plan sponsor specifically added weight loss medication as a benefit, and only when the member meets clinical criteria and gets prior authorization. On many UnitedHealthcare plans, weight loss drugs are excluded entirely. The answer, in other words, depends less on UnitedHealthcare than on the plan your employer bought.

Whether Your Plan Includes the Benefit

UnitedHealthcare treats weight loss medication as an “optional program” that employers and plan sponsors elect in or out of.1UHC Provider. PA Notification Weight Loss If your employer did not elect it, Wegovy is not covered for weight management on your plan, and UnitedHealthcare routes those members to a separate set of “Nonformulary Wegovy criteria” that cover the drug only for other medical indications.2UHC Provider. PA Non-Formulary Wegovy

Coverage odds by plan type:

  • Employer-sponsored and self-funded group plans are the most likely to include Wegovy, but still only when the employer opted in.
  • Individual and ACA marketplace plans typically exclude weight loss drugs. The Affordable Care Act does not require insurers to cover them.2UHC Provider. PA Non-Formulary Wegovy
  • UnitedHealthcare Medicare Advantage plans do not cover Wegovy for weight loss. Medicare has a longstanding statutory ban on covering drugs prescribed solely for weight loss, and in April 2025 the Trump Administration confirmed that Medicare and Medicaid would not begin covering anti-obesity medications, formalizing that in the 2026 Medicare Part D final rule.3American Gastroenterological Institute. Anti-Obesity Drugs Will Not Be Covered by Medicare and Medicaid in 2026

The fastest way to confirm your own situation is to look at your plan documents for the term “weight loss medication” or call the member number on your card and ask whether weight loss drugs are a covered benefit.

Clinical Criteria When Coverage Exists

If your plan does include the benefit, UnitedHealthcare still requires prior authorization and enforces specific medical criteria. As of May 2026, the baseline requirements for Wegovy for weight loss are:

  • Age 12 or older for the Wegovy injection. For the oral Wegovy tablet, the minimum age is 16 on some plan types.
  • A body mass index of 30 or higher, or 27 or higher with at least one weight-related condition such as high blood pressure, type 2 diabetes, high cholesterol, or sleep apnea. For pediatric patients, the threshold is a BMI above the 95th percentile.
  • Documented participation in diet, exercise, or behavioral changes, both at the initial authorization and at every renewal.1UHC Provider. PA Notification Weight Loss

UnitedHealthcare does not require step therapy for Wegovy, so you do not have to try and fail other weight loss medications first.1UHC Provider. PA Notification Weight Loss

The initial authorization lasts five months. To renew for another 12 months, you must show at least 5% weight loss from your starting weight and continued lifestyle participation. Fall short of that 5%, and reauthorization can be denied.1UHC Provider. PA Notification Weight Loss

In February 2026, UnitedHealthcare added the Wegovy oral tablet to the policy alongside the injection. Both share the same BMI and lifestyle requirements for weight loss.1UHC Provider. PA Notification Weight Loss

Ozempic Is a Different Answer

Ozempic contains the same active ingredient as Wegovy, but it is FDA-approved only for type 2 diabetes and for cardiovascular risk reduction in diabetic patients. UnitedHealthcare generally does not cover Ozempic when it is prescribed off-label for weight loss. Coverage requires a diabetes diagnosis, supporting lab values, and often step therapy showing that first-line medications like metformin were tried first.4SingleCare. Does United Healthcare Cover Ozempic For semaglutide prescribed specifically for weight management, the covered product is Wegovy.

If Your Plan Excludes Weight Loss Drugs

Even on plans that exclude weight loss medications, Wegovy can still be approved for two medical indications that are not weight loss itself.

Cardiovascular Risk Reduction

UnitedHealthcare covers Wegovy injection and tablets for members aged 45 or older who have established cardiovascular disease — a prior heart attack, stroke, or symptomatic peripheral arterial disease — and a BMI of at least 27. These members must also be on standard cardiovascular therapies like statins and blood pressure medications, and cannot have a diabetes diagnosis or an A1c above 6.5%.2UHC Provider. PA Non-Formulary Wegovy

MASH (Liver Disease)

The Wegovy injection, but not the tablet, can be authorized for members with metabolic dysfunction-associated steatohepatitis who have moderate to advanced liver scarring (fibrosis stage F2 or F3), confirmed by imaging or biopsy. The prescriber must be a gastroenterologist or hepatologist, and the patient cannot be starting Wegovy at the same time as another MASH drug called Rezdiffra.2UHC Provider. PA Non-Formulary Wegovy

North Dakota’s Mandate

North Dakota is the one state where coverage is required rather than optional. In January 2025, it became the first state to mandate GLP-1 coverage by amending its Essential Health Benefit benchmark, requiring individual and small group plans to cover semaglutide and tirzepatide for conditions including prevention of diabetes, insulin resistance, metabolic syndrome, and morbid obesity.5North Dakota Insurance Department. ND INS EHB Changes

UnitedHealthcare’s clinical policy follows the mandate but applies tighter criteria in North Dakota than on standard commercial plans. Fully insured small group and individual plans in the state require a BMI of 40 or higher, or for pediatric patients a BMI exceeding 120% of the 95th percentile.1UHC Provider. PA Notification Weight Loss The mandate guarantees coverage exists; the medical management criteria narrow who actually qualifies. Nationally, no federal law requires private insurers to cover anti-obesity medications, and most state bills attempting similar mandates have not advanced.6UnitedHealthcare. Demand for GLP-1 Drugs

What It Will Cost You

UnitedHealthcare’s public policy documents don’t publish formulary tiers or copays for Wegovy, because those vary by plan design. When covered, Wegovy generally sits on a high-cost specialty tier, so out-of-pocket costs can still be significant.

For members whose commercial insurance covers Wegovy, manufacturer Novo Nordisk offers a savings card that can bring the monthly cost down to as little as $25, with a maximum savings of $100 per fill.7Wegovy. What to Pay for Wegovy The savings card is not available to people with government-funded insurance like Medicare, Medicaid, or Tricare.8GoodRx. Wegovy for Weight Loss Cost Coverage

Without insurance coverage, the list price is roughly $1,350 for a 28-day supply. Novo Nordisk has also introduced self-pay pricing of $199 per month for the injection and $149 per month for certain doses of the oral tablet through August 2026.7Wegovy. What to Pay for Wegovy

If Your Employer Uses Total Weight Support

Some employers add Wegovy coverage through UnitedHealthcare’s Total Weight Support program, which bundles the medication with a behavioral coaching requirement. UnitedHealthcare says 58% of patients stop GLP-1 medications before reaching a clinically meaningful health benefit, and the program is built around that adherence problem.6UnitedHealthcare. Demand for GLP-1 Drugs

Employers who adopt the program pick one of two vendors: Real Appeal Rx or WeightWatchers for Business. Real Appeal Rx provides one-on-one and group coaching, pharmacist access, digital tracking tools, and a kit with a smart scale and fitness app access. WeightWatchers for Business runs weekly workshops, nutrition tools, and GLP-1 support groups.9UnitedHealthcare. Total Weight Support Participation in monthly coaching is a prerequisite for medication coverage under this program, so if your employer uses it, you have to engage with the coaching to keep the prescription authorized.10UnitedHealthcare. If You Get Denied

Denials for Wegovy are common, and you have the right to appeal. A strong appeal typically includes detailed medical records, BMI documentation, a history of prior weight loss attempts, documentation of related health conditions, and a letter from the prescribing physician explaining why Wegovy is medically necessary. When the plan’s exclusion is specifically for weight loss drugs, framing the appeal around treatment of obesity-related conditions rather than cosmetic weight loss can strengthen the case. Novo Nordisk publishes a sample appeal letter that patients and physicians can adapt.11Medical News Today. How to Appeal Wegovy Denial

If you are on a UnitedHealthcare Medicare Part D plan, the formal first-level appeal is called a redetermination, and it must be filed within 65 days of the denial notice. UnitedHealthcare must respond within seven calendar days for a standard appeal, or within 72 hours if your doctor certifies that waiting could harm your health. If that first appeal is denied, you can escalate to an Independent Review Entity.12UnitedHealthcare. Prescription Drug Appeals Keep in mind that an appeal cannot override a Medicare statutory exclusion for weight loss drugs; appeals succeed when they establish a covered medical indication, not when they argue against the exclusion itself.
