Does Tufts Cover Weight Loss Medication? 2026 Changes and Appeals

Tufts health plans generally do not cover weight loss medication as of 2026. Point32Health, which operates Tufts Health Plan, removed Wegovy, Zepbound, and other anti-obesity drugs from most of its formularies effective January 1, 2026. GLP-1 medications prescribed for type 2 diabetes are still covered, and a small number of Harvard Pilgrim employer plans retain weight loss drug coverage, but members on Tufts Health Direct and most other Tufts products now pay out of pocket.

What Changed on January 1, 2026

Tufts Health Direct, sold through the Massachusetts Health Connector, stopped covering all weight loss medications on January 1, 2026. The exclusion is broad. It covers drugs prescribed for weight loss and GLP-1 medications prescribed for what the plan calls “alternative indications,” such as cardiovascular risk reduction or other obesity-related conditions. Existing approvals did not carry over. Members with active prior authorizations for weight loss medications saw those authorizations terminated on January 1, 2026, no matter how recently they were approved.2Point32Health. This change built on an earlier step. On July 1, 2025, Tufts Health Direct had already stopped covering certain weight loss prescriptions and shifted members to cheaper alternatives. Coverage by Plan Type

Which Tufts or Point32Health product you have determines whether any pathway to coverage exists.

  • Harvard Pilgrim Commercial (Premium or Value formularies). Some large employer groups can opt into formularies that still include weight loss drug coverage. Zepbound is the preferred GLP-1.   • Tufts Health Public Plans (MassHealth). Effective July 1, 2026, coverage ends for Wegovy, Zepbound, Saxenda, Contrave, and Qsymia when prescribed for obesity or overweight. GLP-1s prescribed for specific conditions such as cardiovascular disease, MASH, or obstructive sleep apnea remain covered, but a new prior authorization is required.5Point32Health. If your coverage is through an employer, check which formulary your group selected. Some large employers on the Harvard Pilgrim side may have chosen a Premium or Value formulary that keeps coverage. GLP-1s Prescribed for Diabetes Are Still Covered

The exclusion does not touch GLP-1 medications prescribed for type 2 diabetes. Ozempic, Mounjaro, Rybelsus, Trulicity, and others remain covered for blood sugar management. Point32Health has tightened the rules here, though. As of May 1, 2026, an automated step therapy process was replaced with mandatory prior authorization for every diabetic GLP-1 prescription. The plan has stated that these prior authorization criteria are “intended to reduce or prevent off-label use for weight loss.” Prescribers writing GLP-1 prescriptions nominally for diabetes but effectively for weight management can expect scrutiny.7Point32Health. If You Lost Coverage, What Are Your Options

Nothing fully replaces insurance coverage, but a few routes can reduce what you pay or offer a different clinical path.

Harvard Pilgrim fully insured members on a formulary that retained weight loss coverage have one additional requirement: a six-month behavior modification program called ResetStrong, with coaching by registered dietitians, must be completed before a GLP-1 prescription is approved. Members already taking a weight loss GLP-1 when the policy took effect are exempt. ResetStrong does not apply to Tufts Health Direct, which excludes the drugs entirely.14Point32Health. Appealing a Denial or Requesting an Exception

If you are on Tufts Health Direct, there is no case-by-case denial to appeal. The drugs are categorically excluded from the formulary, which is a harder posture to overturn than an individual prior authorization denial. You can still ask your prescriber to submit a formulary exception request explaining why the specific medication is medically necessary and why covered alternatives are not appropriate for your situation.

For Tufts Medicare Preferred members, the formal process is a Request for Redetermination. You have 60 to 65 days from the date of the denial notice to file. Requests can be submitted by mail, fax, online, or by phone. If waiting for the standard seven-day decision could seriously harm your health, you can request an expedited review with a 72-hour turnaround. Including medical records and a prescriber statement that addresses the plan’s specific coverage criteria strengthens the request.15Tufts Health Plan. Massachusetts Law Does Not Require Coverage

Massachusetts does not have a law requiring commercial health insurers to cover anti-obesity medications. Coverage decisions sit with each insurer and plan design. Federal law prohibits Medicare from covering drugs for weight loss, though CMS now allows Part D plans to cover Wegovy for its FDA-approved cardiovascular risk reduction indication.16Massachusetts Health Policy Commission.
