TRICARE covers vitamin D testing when your provider documents that it’s medically necessary to diagnose or manage a specific condition, but it does not cover routine vitamin D screening for people without symptoms. That split has been in place since 2020 and applies whether you’re on TRICARE Prime, TRICARE Select, or TRICARE For Life.
When TRICARE Pays for a Vitamin D Test
Coverage turns on medical necessity. If you have signs or symptoms a provider connects to possible vitamin D deficiency, or you have a condition that puts your vitamin D at risk, the test is covered under standard lab benefits. The TRICARE West Region’s lab guidance describes covered use as generally restricted to patients who show clinical signs or symptoms of deficiency, including metabolic bone disease, osteomalacia, and conditions associated with malabsorption.1TriWest Healthcare Alliance. Two practical points follow. First, the provider has to include an ICD diagnosis code on the lab order that supports the test. Without one that establishes medical necessity, the claim is likely to be denied.1TriWest Healthcare Alliance. TRICARE West Region Lab and Pathology Provider Kit Second, if you want a vitamin D test, describe your symptoms or risk factors to your provider rather than asking for it as part of a general wellness check. Bone pain, unexplained fatigue, muscle weakness, or an abnormal result from another lab are the kinds of findings that can support ordering the test.
Reporting on the 2020 policy change confirmed that TRICARE continues to cover vitamin D screenings for patients who exhibit symptoms of deficiency or where a physician determines the test is medically necessary.2Military.com. TRICARE Ends Coverage for Routine Vitamin D Screenings
What’s Not Covered
TRICARE lists “Vitamin D Screening” on its official exclusions page.3TRICARE. Exclusions The dedicated screening page identifies three situations where the test will not be paid for: the patient has no symptoms, the test is ordered as a preventive measure, or it’s part of an exam that produces no abnormal findings.4TRICARE. Vitamin D Screening The TRICARE Policy Manual uses the same language, excluding vitamin D screening “in asymptomatic individuals, in the general population, as a preventive measure, and/or during examinations without abnormal findings.”5Health.mil. The distinction is between screening and diagnostic testing. Screening checks for a condition in someone with no signs of it. Diagnostic testing investigates something the provider has already noticed. TRICARE covers the second and not the first. As of the policy’s most recent update in October 2024, there is no indication TRICARE plans to reverse this position.4TRICARE. Vitamin D Screening
What You’ll Pay for a Covered Test

- TRICARE Prime with a network provider: $0.
- TRICARE Prime with a non-network provider: point-of-service fees apply (50% of the allowable charge after a separate deductible).
- TRICARE Select with a network provider: $0.
- TRICARE Select with a non-network provider: 20% cost-share for active duty family members, or 25% for retirees and their family members, after the annual deductible.7TRICARE. Compare TRICARE Plans
If you stay in-network and your provider documents medical necessity, the lab work itself typically costs you nothing.
TRICARE For Life and Medicare
If you have TRICARE For Life, Medicare pays first. Medicare also does not cover routine vitamin D screening, but it does cover vitamin D assay testing when reasonable and necessary for conditions including chronic kidney disease (stage III or greater), osteoporosis, osteomalacia, rickets, calcium disorders, parathyroid disorders, malabsorption states, and use of medications known to lower vitamin D levels such as anticonvulsants or glucocorticoids.8CMS. When Medicare covers the test, TRICARE picks up what you would otherwise owe. When Medicare denies the test as not medically necessary, TRICARE cannot pay for it either, and any challenge has to start as a Medicare appeal.9Health.mil. TRICARE Reimbursement Manual, Chapter 4, Section 4 Medicare also caps frequency: once a year for most conditions, up to four times a year for active vitamin D deficiency, rickets, osteomalacia, or aluminum bone disease.8CMS. If Your Claim Is Denied
You can appeal. TRICARE handles two kinds of medical appeals. A factual appeal is filed with your regional contractor when TRICARE didn’t pay for what you believe is a covered service. A medical necessity appeal is filed when TRICARE denied pre-authorization on the ground that the service wasn’t medically necessary.10TRICARE Newsroom. Understanding the TRICARE Claims Process
One point worth knowing before you appeal: the TRICARE Reimbursement Manual instructs contractors not to place blanket denials on all care in targeted low-value categories, and requires reimbursement for care that is medically necessary and appropriate.11Health.mil. TRICARE Reimbursement Manual, Chapter 18, Section 2 If your provider documented symptoms or a risk condition and the claim was still denied, that documentation is what a factual appeal turns on.
A Note on Vitamin D Supplements
Testing coverage doesn’t extend to over-the-counter vitamin D. TRICARE covers vitamins and supplements only for narrow categories: home internal nutrition therapy (such as for cancer patients) and enrollees with metabolic disorders including inborn errors of metabolism, malabsorption conditions, gastrointestinal pathologies, and certain neurological or physiological conditions.
