TRICARE does cover out-of-network therapy, but only when the therapist is TRICARE-authorized, and you’ll almost always pay more than you would for a network provider. How much more depends on which TRICARE plan you have and whether the therapist agrees to accept TRICARE’s allowable charge as full payment.
What “Out of Network” Means Under TRICARE
A non-network provider is a therapist who meets TRICARE’s authorization standards (state licensure, national accreditation, TRICARE certification) but has not signed a contract with the regional managed care contractor, Humana Military in the East Region or TriWest Healthcare Alliance in the West Region.1TRICARE. Within the non-network category, one question matters more than any other before you book: does the therapist participate with TRICARE? A participating non-network provider accepts TRICARE’s allowable charge as full payment, files your claim, and collects only your cost-share at the visit. A nonparticipating non-network provider can bill you up to 15 percent above the allowable charge, usually expects payment in full upfront, and leaves you to file the claim yourself.1TRICARE. The TRICARE allowable charge (formally the CHAMPUS Maximum Allowable Charge) is tied to Medicare’s rates and varies by procedure code and ZIP code.4Defense Health Agency. If your therapist charges $200 and the allowable amount in your area is $120, TRICARE’s math starts from $120.
Non-Network Costs Under TRICARE Select and Similar Plans
TRICARE Select, Reserve Select, Retired Reserve, and TRICARE Young Adult-Select give the most straightforward access to out-of-network therapy. No referral is required for outpatient mental health visits, and you can see any TRICARE-authorized provider, in-network or not.5TRICARE. For 2026, the non-network cost-share for outpatient mental health (specialty care) is:6TRICARE. Compare Costs
- Active duty family members (Groups A and B): 20 percent of the allowable charge.
- Retirees and their family members (Groups A and B): 25 percent.
- Reserve Select: 20 percent.
- Retired Reserve: 25 percent.
Network copays for the same visits are flat dollar amounts between $33 and $52 depending on beneficiary category.6TRICARE. Compare Costs The percentage cost-share can run above or below the flat copay depending on local allowable rates, but factoring in the deductible and the possibility of balance billing, non-network care usually costs more.
Non-Network Costs Under TRICARE Prime
Prime is where out-of-network therapy gets expensive. Network mental health visits don’t require a referral from your primary care manager.7TRICARE. - A separate POS deductible of $300 per individual or $600 per family.
- A 50 percent cost-share of the allowable charge after the deductible.
- Any balance billing from a nonparticipating provider on top of that.

There is one way to avoid POS costs. If your regional contractor determines that no network providers are available in your area, a Prime enrollee can be approved to see a non-network provider at standard cost-shares. TRICARE For Life
TRICARE For Life follows Medicare’s rules. Medicare pays its approved amount first, and TRICARE covers the remainder for services both programs cover, often leaving you with nothing out of pocket.6TRICARE. Balance Billing and the 15 Percent Cap
Nonparticipating providers can bill above the TRICARE allowable amount, but inside the United States they cannot charge more than 115 percent of it.11TRICARE. Balance Billing12Defense Health Agency. Two cautions. First, if you sign a statement agreeing to pay above the allowable charge, the 15 percent cap no longer applies.13TRICARE Newsroom. Review your Explanation of Benefits after every visit to compare what the provider charged with the allowable amount. What Counts Toward Your Catastrophic Cap
TRICARE caps annual out-of-pocket spending on covered services. For 2026, caps range from $1,000 for active duty family members under Group A to $4,635 for Group B retirees.15Federal Register. The practical effect is significant. If you use a nonparticipating non-network therapist, much of what you spend never brings you closer to the cap. Using a participating non-network provider under a Select-type plan is the version where your standard cost-shares do count.
Filing Your Own Claim
With a nonparticipating non-network therapist, you typically pay upfront and file for reimbursement yourself.19TRICARE. Claims

Telehealth Therapy From Non-Network Providers
TRICARE covers mental health services by telehealth, including audio-only sessions, from any TRICARE-authorized provider.21Defense Health Agency. TRICARE Policy Manual, Chapter 1, Section 15 The same cost-sharing rules apply as for in-person visits, based on network status and participation.
Cross-state licensure is the wrinkle. A temporary rule issued during the COVID-19 public health emergency let TRICARE reimburse providers licensed in any U.S. state as long as they complied with applicable federal or state laws in the beneficiary’s state and weren’t barred from practice anywhere.22Federal Register. What TRICARE Won’t Cover, In-Network or Out
Coverage extends to individual, group, and family psychotherapy, medication management, and psychological testing when done alongside covered psychotherapy. Intensive outpatient and partial hospitalization programs are covered with prior authorization. Several services are excluded no matter who provides them: couples or marital therapy, sex therapy, aversion therapy, telephone-only counseling (with limited exceptions for geographically distant family sessions), off-label ketamine, psychedelic medications, and counseling for lifestyle modifications or stress management.23TriWest Healthcare Alliance.
