Does the VA Cover Mounjaro? Diabetes Only, Not Weight Loss

The VA does cover Mounjaro for veterans with type 2 diabetes, but only with local prior authorization, and it will not prescribe the drug for weight loss. Veterans who want tirzepatide for obesity have to go through a separate, more restrictive pathway using Zepbound, the weight-management version of the same active ingredient.

Mounjaro for Type 2 Diabetes

Mounjaro sits on the VA national formulary with a status of “PA-F,” which means any VA provider can prescribe it for a qualifying veteran, but the local VA pharmacy has to approve the prescription before it’s filled.¹ The clinical requirements come from a document called “Criteria for Use: Tirzepatide (MOUNJARO) Injection for Type 2 Diabetes Mellitus,” most recently updated in April 2026.

Those criteria are applied at the facility level rather than through a single national standard, so the exact documentation a provider has to submit can vary from one VA medical center to the next.¹ Ask your VA pharmacy what your facility specifically requires before you assume a prescription will go through.

The VA Will Not Prescribe Mounjaro for Weight Loss

Mounjaro is FDA-approved only for type 2 diabetes, and the VA will not prescribe it off-label for obesity or weight management.² If you want tirzepatide for weight loss, the drug you’re looking for is Zepbound, which carries the same active ingredient but a separate FDA approval for chronic weight management.

What the VA Covers for Weight Loss

For obesity treatment, the VA offers several FDA-approved medications sorted into two tiers by formulary status.

On the national formulary with prior authorization: phentermine/topiramate, orlistat, and naltrexone/bupropion. VA policy generally requires providers to try these first-line options before moving on.³
Non-formulary with criteria for use: liraglutide, semaglutide (Wegovy), and tirzepatide (Zepbound). These require a non-formulary drug request and prior approval.³

Despite that hierarchy, a 2024 medication-use evaluation across 37 VA medical centers found GLP-1 medications like semaglutide were frequently used as first-line therapy, likely because many patients met BMI or comorbidity criteria that justified skipping the formulary alternatives.³

Who Qualifies for Weight Management Medication

To be considered for any VA weight-loss drug, a veteran has to meet clinical criteria drawn from the VA/DoD Clinical Practice Guideline for the Management of Adult Overweight and Obesity, and has to participate in a lifestyle program.

BMI and Related Health Conditions

You need a BMI of 30 or higher, or a BMI of 27 or higher with at least one weight-related health condition. Qualifying conditions include type 2 diabetes, hypertension, high cholesterol, metabolic syndrome, obstructive sleep apnea, osteoarthritis, and metabolic dysfunction-associated steatotic liver disease.⁴

Eligibility is based on those clinical factors rather than on whether your obesity is service-connected. Any veteran receiving VA health care who meets the thresholds and participates in a lifestyle intervention can be considered.² Service connection affects what you pay, not whether you qualify.

The MOVE! Program Requirement

Weight-loss medications are treated as add-ons to lifestyle intervention, not standalone treatments. The VA’s main vehicle for that intervention is the MOVE! weight management program, and you have to enroll and demonstrate satisfactory participation before medication is considered.² The 2024 evaluation found 88% of veterans receiving weight-management medications had documented participation in MOVE! or an approved alternative.³ Some facilities set specific attendance thresholds before a medication appointment can be scheduled.⁵ Ask your primary care provider for a referral, or self-schedule into a MOVE! enrollment class if your facility allows it.⁶

The Four-Month Weight Loss Check

For veterans on tirzepatide (Zepbound) for weight management, the VA expects at least 5% weight loss from baseline after four months. If you’re not losing weight, the VA directs you to talk with your health care team. The available guidance does not spell out whether the medication is automatically discontinued or simply reassessed.⁴

Supply Constraints and Facility Rationing

Demand for GLP-1 medications has strained supply. The FDA removed tirzepatide from its drug shortage list in December 2024, and semaglutide followed in February 2025.⁷ Effects on VA prescribing have lingered. During an earlier semaglutide shortage, a study of 37 VA medical centers found 56% of sites halted new prescriptions entirely, while others switched patients to different medications or placed them on hold.⁸

Some facilities have formal rationing. VA San Diego, for example, implemented “Scarce Resource Allocation Guidance” that restricts new starts of injectable drugs like Zepbound to veterans with a BMI of 40 or higher under approved eligibility tiers. VA San Diego attributes the scarcity to four factors: supply chain issues, limited provider availability, uneven access to MOVE! programs, and budget constraints. Veterans already on a weight-management medication at another VA facility can generally continue treatment when they transfer, regardless of the new facility’s tier restrictions.² Whether other medical centers use similar tiered systems is not clear from national guidance; rationing appears to be a facility-level decision.

What Mounjaro Costs at the VA

Mounjaro falls under copay Tier 3, which covers brand-name prescriptions. For 2026, the VA charges $11 for a 1-to-30-day supply, $22 for a 31-to-60-day supply, and $33 for a 61-to-90-day supply.⁹ Several exemptions reduce or eliminate those charges:

Priority Group 1 veterans, meaning those rated 50% or higher service-connected or unemployable due to service-connected conditions, pay no medication copays at all.⁹
Veterans with service-connected ratings of 40% or less may qualify for free medications if their income falls at or below VA national income limits.⁹
Once you’ve paid $700 in medication copays in a calendar year, the rest of that year’s copays are waived.⁹

Enrollment in MOVE! itself has no copay, regardless of disability rating or priority group.⁹

Filling Mounjaro Outside the VA

You cannot fill Mounjaro at a non-VA pharmacy and have the VA reimburse the cost. VA policy requires non-urgent, regular prescriptions to be filled through VA pharmacies. The VA pays for up to a 14-day supply at retail pharmacies only from the urgent/emergent formulary, and Mounjaro does not fall into that category.¹⁰ When a VA community care provider writes a prescription for a maintenance medication, it has to be submitted to a VA facility pharmacy for fulfillment.¹¹

Veterans transferring weight-management care from a non-VA provider to the VA must supply documentation of their weight when therapy started, and must meet their local facility’s eligibility criteria.²

A Note on CHAMPVA for Family Members

CHAMPVA, which covers eligible dependents and survivors, uses different rules from the VA health system that covers veterans themselves. Effective January 1, 2025, CHAMPVA restricted Mounjaro coverage exclusively to beneficiaries with a type 2 diabetes diagnosis. It is not covered for weight loss, pre-diabetes, or obesity management, and that restriction remains in effect for 2026.¹²¹³

Qualifying CHAMPVA beneficiaries can receive Mounjaro through the Meds by Mail program at no out-of-pocket cost, provided they have no other prescription drug coverage. Because Mounjaro requires refrigeration, it cannot be shipped to a PO box. A provider can send the prescription electronically to “Meds by Mail CHAMPVA” (pharmacy ID 5204437), or an original 90-day prescription can be mailed to Meds by Mail, PO Box 331178, Murfreesboro, TN 37133. Delivery takes up to 21 days.¹⁴
