Whether Surest covers a GLP-1 for weight loss depends entirely on your employer. Surest is a UnitedHealthcare plan framework that each employer customizes, and prescription drug coverage, including whether Wegovy, Zepbound, or Saxenda is on your formulary for weight management, is one of those customizable pieces.1Surest. How to Check Your Own Plan
Before anything else, confirm what your plan actually covers. Which GLP-1s Are Covered When the Plan Includes Them
For employers who have opted in, UnitedHealthcare’s weight loss policy effective May 1, 2026 covers three FDA-approved GLP-1 medications, each with its own prior authorization rules.2UHC Provider. Prior Authorization and Notification – Weight Loss
- Wegovy (semaglutide), approved for weight loss, cardiovascular risk reduction in patients with established heart disease, and MASH with moderate to advanced liver fibrosis. Available as injection and, as of February 2026, a tablet. Patients must be at least 12. Initial authorization is 5 months; reauthorization for 12 months requires at least 5% weight loss from baseline.
- Zepbound (tirzepatide), approved for weight loss and moderate to severe obstructive sleep apnea. Patients must be older than 16. Initial authorization is 6 months; reauthorization for 12 months requires at least 5% weight loss and continued lifestyle modification.
- Saxenda (liraglutide), approved for weight loss. Patients must be at least 12. Initial authorization is 4 months; reauthorization for 12 months requires at least 4% weight loss from baseline.
All three share the same core eligibility: the medication must be paired with diet and exercise, and the patient must have a BMI of 30 or higher, or a BMI of 27 or higher with at least one weight-related condition such as hypertension, type 2 diabetes, high cholesterol, or sleep apnea. Prior Authorization and Keeping Your Coverage
Every one of these drugs requires prior authorization. Your prescriber submits the request through the UnitedHealthcare Provider Portal at UHCprovider.com or by calling 877-842-3210.4UHC Provider. Surest Prior Authorization Requirements The submission has to show your BMI, any qualifying comorbidities, and that lifestyle modification is part of your treatment plan.
Reauthorization is not automatic. When the initial 4-to-6-month window ends, your provider has to document that the drug is working. Wegovy and Zepbound require at least 5% weight loss from your starting weight; Saxenda requires 4%. Fall short, and continued coverage can be denied.2UHC Provider. Some Employers Require a Weight Management Program
Clinical criteria may not be the whole story. UnitedHealthcare offers a program called Total Weight Support, which pairs medication with ongoing behavioral coaching. For employers that adopt it, GLP-1 coverage may be conditional on enrolling in the program and attending monthly coaching sessions, with participation built directly into the prior authorization criteria. FedEx, one of the largest Surest employer groups, uses a stricter version. FedEx/Surest members cannot get a GLP-1 prescription for weight loss from their personal doctors. They have to enroll through Calibrate, a clinician-led metabolic health program, complete lab work, be found clinically eligible by a Calibrate clinician, and keep up with bi-weekly video coaching and daily tracking of weight and food. Members who stop participating can be disenrolled and cannot rejoin for 90 days.6OptumRx. Surest uses variable copays rather than deductibles and coinsurance, and shows upfront prices for care in the Surest app.7Surest. Your actual cost depends on how the drug is tiered on your Prescription Drug List, which you can look up in the app or confirm by calling the number on your ID card.

If Your Plan Doesn’t Cover It or Your Request Is Denied
- Ask whether another diagnosis applies. Wegovy now has FDA-approved indications beyond weight loss, including cardiovascular risk reduction and MASH, and Zepbound covers moderate to severe obstructive sleep apnea. - Build thorough documentation up front. Recent BMI, weight history, records of prior diet and exercise efforts, lab results, and a list of any weight-related comorbidities all help.
- Look at manufacturer savings programs. Novo Nordisk and Eli Lilly offer savings cards and copay assistance that can lower out-of-pocket costs, particularly when insurance covers the drug but puts it in a high tier.
