OSHA does require fall protection on ladders, but only in one specific situation: fixed ladders that extend more than 24 feet above a lower level must be equipped with either a personal fall arrest system or a ladder safety system. Portable ladders — stepladders, extension ladders, and any other movable type — are not subject to a personal fall protection requirement at any height. Instead, OSHA regulates them through safe-use rules covering placement, climbing technique, and inspection.
That split is the heart of the answer, and it trips up a lot of employers who assume the general fall protection thresholds (four feet in general industry, six feet in construction) apply to ladders the same way they apply to platforms and unprotected edges. Portable Ladders: No Harness Required, But Rules Still Apply
Workers on portable ladders are not required to wear a harness, tie off, or use any personal fall protection equipment, regardless of how high the ladder reaches. The general industry fall protection standard in 29 CFR 1910.28 expressly excludes portable ladders from its scope, and in construction neither the ladder standard nor the fall protection standard mandates personal fall protection on a portable ladder.2Occupational Safety and Health Administration (OSHA). What OSHA does require is safe use. The core rules for portable ladders include:
- Three points of contact while climbing or descending — two hands and a foot, or two feet and a hand.
- A stable, level surface, or the ladder must be secured against displacement.
- For non-self-supporting ladders like extension ladders, a base-to-height ratio of roughly one to four: the base sits about one foot away from the wall for every four feet of working length.3Occupational Safety and Health Administration. 29 CFR 1926.1053 – Ladders
- Side rails that extend at least three feet above any upper landing the ladder provides access to.3Occupational Safety and Health Administration. 29 CFR 1926.1053 – Ladders
- Facing the ladder while climbing, with the body’s center of gravity between the side rails.
The absence of an equipment mandate does not mean the hazard can be ignored. OSHA’s general duty clause still obligates employers to address recognized hazards, and tasks that need both hands at height or otherwise carry unusual risk often call for scaffolding or an aerial lift rather than a ladder in the first place.
Fixed Ladders and the 24-Foot Rule
Fixed ladders are the permanent ones — bolted to buildings, water tanks, silos, communications towers, and similar structures. This is the one ladder category where OSHA does require fall protection equipment, and the trigger is a specific height. Any fixed ladder that extends more than 24 feet above a lower level must have fall protection.4eCFR. 29 CFR Part 1910 Subpart D — Walking-Working Surfaces Below 24 feet, no equipment is mandated, though the ladder still has to meet OSHA’s construction and inspection standards.
What system is required depends on when the ladder was installed:

The Cage Phase-Out and the 2036 Deadline
For decades cages, sometimes called basket guards, were the default way to protect workers climbing tall fixed ladders. OSHA no longer treats them as adequate. The agency’s position is direct: cages and wells are not ladder safety systems.4eCFR. 29 CFR Part 1910 Subpart D — Walking-Working Surfaces A cage may prevent a climber from falling backward away from the ladder, but it will not stop a fall down the ladder itself.
Three dates matter. Since November 19, 2018, new fixed ladders cannot rely on a cage or well for fall protection. Any replaced section triggers an immediate upgrade. And by November 18, 2036, every fixed ladder over 24 feet must have a personal fall arrest system or a ladder safety system, with no grandfathering left for caged ladders.4eCFR. 29 CFR Part 1910 Subpart D — Walking-Working Surfaces Existing cages can stay in place alongside a compliant system as long as they do not interfere with its operation.
What Counts as a Compliant System
OSHA recognizes two approaches for fixed ladders over 24 feet.
Personal Fall Arrest Systems
A personal fall arrest system uses a full-body harness, an anchorage, and a connector such as a lanyard or deceleration device to catch a falling worker and limit the forces on the body. Ladder Safety Systems
A ladder safety system runs a carrier — either a rigid rail or a flexible cable — the length of the ladder. The worker clips into that carrier through a safety sleeve attached to a harness. A slip locks the sleeve onto the carrier and stops the fall. OSHA requires that these systems let climbers use both hands freely, without having to continuously push, pull, or hold any part of the system to keep it engaged.6Occupational Safety and Health Administration. 29 CFR 1910.29 – Fall Protection Systems and Falling Object Protection
Where a compliant system is installed, rest platforms are required at intervals of no more than 150 feet along the ladder. Older ladders still using cages or wells need landing platforms every 50 feet.
Construction Versus General Industry
The 24-foot fixed-ladder rule described above comes from the Walking-Working Surfaces standard, which applies to general industry. The construction standard treats ladders somewhat differently, but the practical answer for the searcher is the same: OSHA does not require personal fall protection on portable ladders in construction either, per the agency’s own interpretation letter.2Occupational Safety and Health Administration (OSHA). Inspection and Removal of Defective Ladders
Ladders must be inspected before first use each work shift and more often if conditions call for it, so that any visible defect capable of injuring a worker is caught early.8GovInfo. 29 CFR 1910.23 – Ladders A defective ladder — broken or missing rungs, cracked rails, corrosion, any structural problem — has to be tagged “Do Not Use” or otherwise clearly marked and pulled from service until it is repaired to its original design condition.3Occupational Safety and Health Administration. 29 CFR 1926.1053 – Ladders For fixed ladders, OSHA also allows blocking access physically, such as covering rungs with plywood.
What Non-Compliance Costs
Ladder violations are among the most common citations OSHA writes. In fiscal year 2024, the construction ladder standard (29 CFR 1926.1053) was OSHA’s third most frequently cited standard, general fall protection (1926.501) was first, and fall protection training (1926.503) was seventh.9Occupational Safety and Health Administration. As of January 2025, a serious violation carries a maximum penalty of $16,550. A willful or repeated violation can reach $165,514 per violation, with annual inflation adjustments to come.10Occupational Safety and Health Administration. OSHA Penalties A single inspection that finds a missing fall arrest system on a fixed ladder, no training records, and a defective ladder still in the rotation can stack multiple citations quickly, and the cost of retrofitting a fixed ladder with a compliant system almost always runs below the price of one willful citation.
