Molina Healthcare does cover tirzepatide, but whether your prescription gets paid depends on three things: which brand you’re prescribed, which Molina plan you have, and the state you live in. Mounjaro, prescribed for type 2 diabetes, is generally covered across Molina plans with prior authorization. Why the Brand on the Prescription Changes Everything

Mounjaro for Type 2 Diabetes

Molina formularies list Mounjaro as a covered diabetes medication. The New York Essential Plan formulary effective April 2026 includes “Mounjaro (Tirzepatide)” under diabetes medications with zero cost-sharing for members with a diabetes diagnosis.2Molina Healthcare. Zepbound for Weight Loss

Here the answer splits by plan type.

Medicaid

If you have Molina Medicaid and want Zepbound for weight loss, assume it isn’t covered unless you can confirm otherwise. Federal law lets state Medicaid programs exclude drugs used for weight loss from the Medicaid Drug Rebate Program. As of January 2026, only 13 state Medicaid programs cover GLP-1 drugs for obesity under fee-for-service, and the trend has been shrinking: California, New Hampshire, Pennsylvania, and South Carolina have all dropped coverage.5KFF. Marketplace

Molina’s California Marketplace plan has a published policy that covers Zepbound for morbid obesity, but with strict requirements: a BMI of 40 or higher, or a BMI of 35 or higher with obesity-related conditions such as diabetes, sleep apnea, or hypertension. You also have to complete a three-month formalized weight-management program before authorization and show at least 10% weight loss by week 24 to keep the prescription going.3Molina Healthcare. Medicare

Federal law prohibits Medicare Part D plans from covering weight-loss drugs, and that prohibition still applies to standard Molina Medicare plans. Starting July 2026, however, the federal government launched the Medicare GLP-1 Bridge, a temporary program that sits outside Part D and lets eligible Medicare beneficiaries get Zepbound or Wegovy for weight loss at a fixed $50 monthly copay. Zepbound for Obstructive Sleep Apnea

Zepbound was approved by the FDA in December 2024 for moderate-to-severe obstructive sleep apnea in adults with obesity. This diagnosis opens a different coverage path. Medicaid programs that exclude weight-loss drugs are still required to cover GLP-1 medications prescribed for other FDA-approved indications, and OSA counts.5KFF. No Molina-specific OSA policy for Zepbound appears in available documents. Insurers that have published policies for this indication generally require a sleep study confirming moderate-to-severe OSA (15 or more obstructive respiratory events per hour), a BMI of 30 or higher, and documented lifestyle modification efforts. What Prior Authorization Looks Like

Molina requires prior authorization for tirzepatide regardless of indication. Criteria vary by plan, state, and brand, but the recurring elements include:

  • You have 60 calendar days from the date on the denial letter to file.
