Does Medicare Cover Mycapssa? Part D, Prior Authorization, and Costs

Medicare does cover Mycapssa, but through the Part D prescription drug benefit rather than Part B. Because the drug is an oral capsule you take at home, it is treated as a pharmacy benefit, not a clinic-administered injection.1Kaiser Permanente. Mycapssa Criteria Document Coverage is not automatic. Many Part D plans require prior authorization, some leave the drug off the formulary entirely, and getting approved often takes documentation from an endocrinologist and sometimes a formal appeal.

Part D, Not Part B

Injectable octreotide given in a provider’s office may be billed under Part B.2Counterforce Health. How To Get Mycapssa Covered by Aetna Mycapssa is different. It is an oral specialty drug taken daily, so it runs through Part D, whether that is a standalone prescription drug plan or the drug benefit built into a Medicare Advantage plan.1Kaiser Permanente. Mycapssa Criteria Document If you have Original Medicare without a Part D plan, there is no Medicare pathway for the capsules.

Whether Your Plan Lists It

Not every Part D formulary includes Mycapssa. At least one major Medicare PPO plan reviewed for 2026 did not list it as a covered drug.3Formulary Navigator. AmeriHealth Medicare PPO 2026 Drug List UnitedHealthcare’s clinical pharmacy policy states that “Mycapssa is typically excluded from coverage,” with exceptions possible on a plan-by-plan basis.4UnitedHealthcare. Mycapssa Prior Authorization Criteria When a plan does cover it, the drug sits on a specialty tier with percentage coinsurance rather than a flat copay.

Before anything else, pull up your plan’s current formulary or call member services and ask directly whether Mycapssa is covered, what tier it is on, and what restrictions apply. The answer determines everything that follows.

Prior Authorization

Every major insurer that covers Mycapssa requires prior authorization, and the criteria track the FDA-approved indication closely. To get approved, you generally need to show:

  • A confirmed acromegaly diagnosis, demonstrated through an elevated growth hormone level after an oral glucose tolerance test or elevated age-adjusted IGF-1 levels.
  • Prior successful treatment with injectable octreotide (such as Sandostatin LAR) or lanreotide (such as Somatuline Depot) that you both responded to and tolerated.5Cigna. Somatostatin Analogs Mycapssa Coverage Position Criteria
  • A prescription from or in consultation with an endocrinologist.4UnitedHealthcare. Mycapssa Prior Authorization Criteria
  • In some cases, a clinical reason the patient cannot remain on injectable therapy. UnitedHealthcare asks for this directly.4UnitedHealthcare. Mycapssa Prior Authorization Criteria

Approvals usually run for 12 months. Reauthorization requires evidence that the drug is working, typically an age-normalized IGF-1 level.4UnitedHealthcare. Mycapssa Prior Authorization Criteria Plans may also impose quantity limits. Cigna, for example, caps the standard supply at 56 capsules per 28 days for a 40 mg daily dose, with exceptions allowed up to 80 mg per day.6Cigna. Mycapssa Drug Quantity Management Policy

What You Will Pay

The list price for Mycapssa runs roughly $3,458 for a 28-day supply of 20 mg capsules, and higher doses may require two or more wallets per month.7Drugs.com. Mycapssa Price Guide On a specialty tier, your coinsurance would normally be a punishing percentage of that.

Federal law now caps the damage. The Inflation Reduction Act of 2022 limited annual out-of-pocket spending on Part D drugs to $2,000 in 2025, adjusted to $2,100 for 2026.8GoodRx. Medicare Part D Out-of-Pocket Maximum Your plan may charge a deductible of up to $615 before coverage begins, and once you hit the $2,100 ceiling you enter catastrophic coverage and owe nothing more on covered drugs for the rest of the plan year.9Triage Cancer. 2026 Medicare Part D Quick Guide On a drug that costs over $3,400 a month, you will likely reach the cap at your first or second fill of the year.

The Medicare Prescription Payment Plan, available since January 2026, lets you spread that out-of-pocket amount across the calendar year in monthly installments instead of paying it in one hit at the pharmacy counter. Enrollment is voluntary and free.8GoodRx. Medicare Part D Out-of-Pocket Maximum If you qualify for Medicare Extra Help (the Low-Income Subsidy), you may pay even less, with the Part D deductible and premium eliminated and copays reduced to low fixed amounts that depend on your plan.10GoodRx. If Your Plan Denies Coverage

A denial is a starting point, not an ending. Your first move is to file a coverage exception with your Part D plan: a written request asking the plan to cover the drug despite its formulary status or restrictions. Your prescriber submits a supporting statement explaining why Mycapssa is medically necessary for you. Plans must answer within 72 hours for a standard request, or within 24 hours if you request an expedited decision because waiting could seriously harm your health.11Medicare Interactive. Introduction to Part D Appeals

If the exception is denied, Medicare provides a five-level appeals process. Level 1 is a redetermination by the plan itself, filed within 60 days of the denial, with standard decisions due in 7 days and expedited ones in 72 hours. Level 2 is review by an Independent Review Entity on the same timelines. The higher levels move to an Administrative Law Judge, then the Medicare Appeals Council, and finally federal district court, each with its own dollar thresholds ($200 at levels 3 and 4, $1,960 at level 5 for 2026).11Medicare Interactive. Introduction to Part D Appeals Given Mycapssa’s annual cost, those thresholds are easily cleared.

Keep copies of every document, log phone calls with names and dates, and have your endocrinologist write a letter addressing the specific reasons your plan gave for the denial.12Medicare.gov. Drug Plan Appeals

Help You Can and Cannot Get From the Manufacturer

One boundary catches many patients off guard: the manufacturer’s copay program, Chiesi Total Care, is closed to anyone on a government health plan. Medicare, Medicare Part D, Medicare Advantage, Medigap, Medicaid, TRICARE, and VA beneficiaries are all excluded from financial support.13Chiesi USA. Chiesi Total Care Terms and Conditions Chiesi Total Care does still offer non-financial help to Medicare patients, including assistance with prior authorization paperwork, support during appeals, and coordination with the specialty pharmacy (PANTHERx Rare Pharmacy). The dedicated line is 1-833-346-2277, available Monday through Friday, 8 a.m. to 8 p.m. Eastern.14Chiesi Total Care. Mycapssa Patients and Caregivers

For money, Medicare patients are referred to independent charities. The PAN Foundation operates an acromegaly copay fund open to Medicare, Medicaid, and TRICARE beneficiaries at or below 500% of the federal poverty level, offering an initial grant of $6,800 and up to $13,600 per year when the fund is open. As of mid-2026 the fund is closed, with a wait list for notification when it reopens.15PAN Foundation. Acromegaly Fund With the $2,100 Part D cap now in effect, the gap a foundation would need to fill is smaller than it used to be, though $2,100 is still real money on a fixed income.
