Does Medicare Cover Cruise Ship Claims? The Six-Hour Rule

Medicare does cover cruise ship medical care, but only in a narrow window: the ship must be docked at a U.S. port or no more than six hours from one when you receive treatment, and the shipboard doctor must be legally authorized to practice on the vessel. Step outside that window — on the open ocean, or tied up in a foreign port — and Original Medicare pays nothing. For the rest of the voyage, you are on your own unless you have a Medigap plan with foreign travel benefits, a Medicare Advantage plan that adds international coverage, or a separate travel insurance policy.

The Six-Hour Rule, Explained

Medicare may pay for medically necessary care you receive on a cruise ship when two conditions are both met. The treating physician has to be legally authorized to provide medical services on the ship, and the vessel must be in a U.S. port or within six hours of one at the time of treatment.1Medicare. Medicare Coverage Outside the United States The care does not have to be an emergency. A routine issue that comes up while the ship is still inside that window can also qualify.

Once the ship crosses the six-hour threshold from any U.S. port, Medicare coverage stops.1Medicare. Medicare Coverage Outside the United States On most Caribbean, transatlantic, or Alaska itineraries, that leaves only a small slice of the trip inside the covered zone. Day one leaving Miami, the hours before docking in San Juan, the final approach back to port — those count. The middle of the ocean does not.

When the rule does apply, you still owe your usual Medicare cost sharing. For doctor visits and outpatient care billed under Part B, that means the annual deductible of $283 for 2026 and 20 percent of the Medicare-approved amount after that.2Centers for Medicare & Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles You will also usually need to pay the ship’s medical staff upfront and file for reimbursement yourself.

Why the Rule Is So Narrow

Federal law prohibits Medicare from paying for items or services furnished outside the United States. For this purpose, “United States” means the fifty states, the District of Columbia, Puerto Rico, the U.S. Virgin Islands, Guam, American Samoa, and the Northern Mariana Islands.3Office of the Law Revision Counsel. 42 USC 1395y – Exclusions From Coverage and Medicare as Secondary Payer A cruise ship beyond those boundaries is in international waters, and the exclusion is absolute there except for the port-proximity rule above and a short list of foreign hospital exceptions.

If a Shipboard Emergency Lands You in a Foreign Hospital

Separate from the cruise ship rule, Medicare can pay for inpatient hospital care abroad in a few specific situations. The one most relevant to a cruise passenger: a medical emergency occurs while you are in the United States, and a foreign hospital is closer than the nearest U.S. hospital that can treat you.4GovInfo. Being admitted to a hospital in a distant foreign port of call would not.

What Medicare Will Not Cover Abroad

Two needs cruise travelers often ask about fall outside the rules entirely. Medicare does not pay for dialysis received abroad unless you are admitted to a foreign hospital under one of the inpatient exceptions.1Medicare. Medicare Coverage Outside the United States If you depend on regular treatments, any arrangement with the cruise line or a port facility will be out of pocket.

Medicare Part D does not cover prescription drugs bought outside the United States, including from the ship’s pharmacy or a foreign port. Part D does cover certain travel-related vaccines recommended before international trips, so ask your doctor about immunizations well before you sail.5Medicare. How to Fill the Gap

Medigap Foreign Travel Emergency Benefits

Medigap plans C, D, F, G, M, and N include foreign travel emergency coverage. Plans C and F are only available if you became eligible for Medicare before January 1, 2020.1Medicare. Medicare Coverage Outside the United States The benefit is the same across these plans:

A single medical evacuation from a cruise ship can run through that lifetime cap on its own. The benefit also applies to emergencies only, not routine or ongoing care, and you remain responsible for the 20 percent the plan does not pay.

Medicare Advantage

Medicare Advantage (Part C) plans must follow the same baseline rules as Original Medicare for care outside the country, but individual plans may add international benefits on top.1Medicare. Before you book, call your plan and ask what is covered abroad, whether pre-authorization is required, what you would owe for emergency care, and whether any network rules apply to foreign providers. Get the answers in writing.

Travel Insurance

Medicare itself recommends buying a separate travel insurance policy before going abroad because its own coverage at sea is so limited.1Medicare. Look for emergency medical treatment, emergency medical evacuation (the big-ticket item on a cruise), and pre-existing condition coverage if that applies to you.

Filing a Claim for Covered Shipboard Care

You will usually pay the ship’s medical staff on the spot and seek reimbursement later. Before you disembark, collect:

  • An itemized bill with standard medical codes for each service, procedure, and supply.
  • A clinical summary describing your diagnosis, symptoms, and treatment.
  • The treating doctor’s name, credentials, and documentation of authorization to practice on the vessel.
  • English translations and U.S. dollar conversions if any document is in another language or currency.

It also helps to ask the cruise line for the ship’s navigation log or port schedule showing the vessel’s location when you were treated. That is the single strongest piece of evidence that the six-hour rule applied.

If Your Claim Is Denied

Cruise ship claims draw more denials than typical domestic claims, often because the contractor concludes the ship was outside the six-hour window or that the paperwork is incomplete. Medicare has a five-level appeals process, and most cruise ship disputes resolve at the first two.9Centers for Medicare & Medicaid Services. The first level is a redetermination by your Medicare Administrative Contractor. File within 120 days of the denial notice using the Redetermination Request Form or a written explanation, and include any additional proof of the ship’s location when you received care. A decision generally comes within 60 days.10Medicare.
