Does Medicare Cover Cancerguard? Costs and 2028 Pathway

Medicare does not cover Cancerguard. The multi-cancer early detection blood test from Exact Sciences is sold only as a self-pay product at $689, is not billed to any insurance, and has not been approved by the FDA, which is a prerequisite for Medicare coverage.1Exact Sciences. Cancerguard MCED Providers Resources A federal law signed in February 2026 sets up a coverage pathway for FDA-approved multi-cancer detection tests beginning January 1, 2028, so Cancerguard could become eligible later if it clears the FDA.2Millermeeks.house.gov. Why Medicare Won’t Pay for It

Cancerguard is a laboratory-developed test that screens for signals of more than 50 cancer types from a single blood draw, analyzing DNA methylation patterns and protein biomarkers.3Exact Sciences. Cancerguard MCED for Providers It is performed by Exact Sciences Laboratories under CLIA certification and CAP accreditation. That regulatory status lets the company sell the test, but it does not make the test eligible for Medicare reimbursement.1Exact Sciences. Cancerguard MCED Providers Resources

The same is true of the competing Galleri test from GRAIL, which is likewise sold without FDA approval and is not covered by Medicare or most private insurers.4American Cancer Society. Multi-Cancer Early Detection Tests No multi-cancer early detection test currently qualifies for Medicare coverage.

What You Pay Out of Pocket

The self-pay price is $689, and Exact Sciences does not bill insurance. The cost is eligible for payment through health savings accounts and flexible spending accounts.5Cancerguard. The bigger financial exposure sits behind the test, not on it. A positive Cancerguard result triggers recommended follow-up imaging such as contrast-enhanced CT or PET-CT, which can run into the thousands of dollars. Insurers may decline to pay for those scans because the test that prompted them is not an established standard of care.7StraightHealthcare. Cancerguard Test To cushion that risk, Exact Sciences runs an imaging reimbursement program that may cover up to $6,000 in non-covered follow-up imaging for eligible patients who test positive.1Exact Sciences. What Cancerguard Still Needs

For Cancerguard to fall under the new benefit, Exact Sciences must obtain FDA premarket approval. As of mid-2026, no multi-cancer detection test has received that approval.10AZBio. Blood-Based Cancer Screenings Medicare Does Cover

Multi-cancer detection tests are the gap. Some single-cancer blood tests are already covered under Medicare Part B:

Most preventive screenings under Part B carry no cost-sharing when the provider accepts assignment, a rule set by the Affordable Care Act.19Center for Medicare Advocacy. If you decide to pay for Cancerguard on your own, you will need a physician’s order and can budget the $689 as an HSA or FSA expense. Just plan for the possibility of uncovered follow-up imaging if the result comes back positive, and check whether the Exact Sciences imaging reimbursement program applies to your situation before you draw blood.
