Does Medicare Cover Benlysta? Infusions, Injections, and Copays

Medicare does cover Benlysta (belimumab), but which part of Medicare pays depends on how the drug is given. Intravenous infusions administered in a doctor’s office or clinic are covered under Medicare Part B. The subcutaneous version — the prefilled syringe or autoinjector patients use at home — is covered under Medicare Part D. Either route almost always requires prior authorization, and out-of-pocket costs can be substantial, though a 2025 spending cap and several assistance programs now limit the damage.

Part B Pays for IV Infusions

Medicare Part B covers drugs administered by a healthcare professional in an outpatient setting, and Benlysta IV fits that description. The infusion must be given incident to a physician’s service and cannot be self-administered.1Healthline. Benlysta Cost Standard Part B cost-sharing applies: once you meet the annual Part B deductible, you pay 20 percent of the Medicare-approved amount.

Benlysta IV is dosed at 10 mg per kilogram of body weight, so total cost varies by patient, and GSK does not publish a single list price for the IV form.2GSK For You. Benlysta Pricing Information As a reference point, the wholesale price for a 400 mg vial runs roughly $2,158 and a 120 mg vial about $653.3Drugs.com. Benlysta Price Guide A typical adult dose uses multiple vials, which puts the 20 percent coinsurance share well into four figures per infusion for many patients.

Medigap Can Cover the Coinsurance

If you have a Medicare Supplement (Medigap) policy, most of that 20 percent is picked up for you. Plans A, B, C, D, F, G, and N cover 100 percent of Part B coinsurance (Plan N has small office-visit and ER copays). Plans K and L cover 50 percent and 75 percent, respectively.4Medicare.gov. Compare Medigap Plan Benefits Medigap pays after the Part B deductible is met, unless your specific policy also covers the deductible.

Part D Pays for the Autoinjector and Prefilled Syringe

Subcutaneous belimumab is on CMS’s Self-Administered Drug exclusion list, which means it cannot be billed under Part B’s “incident to” rules.5CMS. Self-Administered Drug Exclusion List, Article A530216CMS. Self-Administered Drug Exclusion List, Article A53032 Coverage runs through your Medicare Part D prescription drug plan instead.

The list price is $1,284.35 for a single dose and $5,137.41 for a four-pack.2GSK For You. Benlysta Pricing Information Because Benlysta is a biologic specialty drug, Part D plans typically place it on the highest formulary tier, with the steepest cost-sharing. Some plans classify it as non-formulary but still cover it when clinical criteria are met.7Kaiser Permanente Northwest. Benlysta SC Coverage Criteria Tier placement, copay amounts, and step-therapy rules vary by plan, so checking your own plan’s formulary is essential.

The $2,000 Cap and Monthly Payment Option

Starting in 2025, the Inflation Reduction Act capped Part D out-of-pocket spending at $2,000 a year, with the figure indexed for inflation (rising to $2,100 in 2026).8KFF. Explaining the Prescription Drug Provisions in the Inflation Reduction Act9Medicare.gov. What’s the Medicare Prescription Payment Plan Before the law, patients on specialty biologics could face unlimited annual cost-sharing. Now, once you hit the cap, Part D covers the rest of your drug costs for the year at zero cost-sharing.10MedPAC. Report to Congress, March 2025 – Section on Part D Redesign

Hitting that cap in a single month at the pharmacy counter is still a strain. The Medicare Prescription Payment Plan, also launched in 2025, lets Part D enrollees spread their out-of-pocket costs across the calendar year in monthly installments with no interest. Enrollment is voluntary and can be done at any time through your Part D plan.11CMS. Medicare Prescription Payment Plan12PAN Foundation. Understanding the Medicare Prescription Payment Plan

Prior Authorization Is the Norm

Whether billed under Part B or Part D, most Medicare plans require prior authorization before paying for Benlysta. Criteria vary by plan but closely track the FDA labeling, which approves Benlysta for patients aged five and older with active systemic lupus erythematosus (SLE) or active lupus nephritis who are receiving standard therapy. It is not recommended for patients with severe active central nervous system lupus.13FDA. Benlysta Prescribing Information

Representative plan requirements include:

Initial authorizations usually run six to twelve months. Renewal takes documentation that the patient is benefiting, generally shown through disease stability or improvement in the medical record.15Aetna. Benlysta Medicare Part B Coverage Policy Some Part D plans add step therapy, requiring documented failure or intolerance of specific drug classes — NSAIDs, antimalarials, immunomodulators, or anifrolumab — before approving subcutaneous Benlysta.7Kaiser Permanente Northwest. If You Have Medicare Advantage

Medicare Advantage (Part C) plans deliver Part B and Part D benefits through a private insurer. Since 2019, these plans have been allowed to impose prior authorization and step therapy on physician-administered Part B drugs, Benlysta IV included.19CMS. Medicare Advantage Prior Authorization and Step Therapy for Part B Drugs Whether a specific plan actually does so depends on that plan’s policies. If your plan denies the drug, you have the right to appeal, and clinical arguments — prior response to Benlysta, steroid dependence, ongoing disease complications — have succeeded in overturning denials on external review.20New York DFS. External Appeal Decision, Case 202009-131546

GSK reports that as of late 2025, more than 95 percent of lupus and lupus nephritis patients nationwide have some form of coverage available for both the autoinjector and the IV formulation, though step edits and prior authorizations may apply.21GSK. Benlysta Coverage and Formulary

Help With What Medicare Doesn’t Cover

Even with Medicare paying its share, remaining costs can be heavy. Several programs can help, though Medicare patients aren’t eligible for all of them.

GSK Patient Assistance Program

GSK’s Patient Assistance Program provides Benlysta free of charge to eligible Medicare patients. To qualify, you must have a Medicare prescription drug plan, live in the United States (including Puerto Rico and the U.S. Virgin Islands), and fall within household income limits. For a single person in the 48 contiguous states, the 2026 limit is $63,840 in annual gross income. Patients over the limit can still qualify if eligible medical expenses bring them within range.22GSK Patient Access Programs Foundation. Benlysta Medicare Patient Assistance

Patients who receive Medicare Extra Help, Medicaid, VA benefits, or other government prescription coverage are not eligible for the PAP.22GSK Patient Access Programs Foundation. Independent Charitable Foundations

Several independent foundations offer copay assistance for lupus patients on Medicare. The PAN Foundation’s SLE fund provides an initial grant of $2,100 and up to $4,200 per year for households at or below 500 percent of the Federal Poverty Level, when the fund is open.24PAN Foundation. Systemic Lupus Erythematosus Disease Fund The HealthWell Foundation offers premium and copay assistance for SLE, averaging roughly $4,250 per 12-month period depending on fund availability. The Patient Advocate Foundation Co-Pay Relief program assists with copays, coinsurance, and deductibles when disease-specific funds are open.

These grants open and close as budgets shift. The PAN Foundation’s FundFinder tool tracks more than 200 patient assistance funds across nine organizations and alerts users when funds reopen.24PAN Foundation. Medicare Extra Help (Low Income Subsidy)

Beneficiaries with limited income and resources may qualify for Extra Help, which sharply reduces Part D costs. In 2026, qualifying beneficiaries pay no plan premium or deductible, with copays capped at $5.10 for generics and $12.65 for brand-name drugs. Once total drug spending reaches $2,100, copays drop to zero for the rest of the year. 2026 eligibility requires individual income below $23,940 and resources below $18,090. People receiving full Medicaid, Supplemental Security Income, or help through a Medicare Savings Program qualify automatically.25Medicare.gov. Home Infusion as an Option

Patients who need Benlysta IV but would rather not travel to a clinic can ask about Medicare’s home infusion therapy benefit, in effect since January 1, 2021 under the 21st Century Cures Act. It covers professional nursing services and monitoring when the drug is given intravenously at home using durable medical equipment.26CMS. Home Infusion Therapy

Uptake has been small. A February 2025 CMS monitoring report found just 1,081 beneficiaries received home infusion therapy services in the second quarter of 2024, with 62 providers billing for them. Current rules reimburse only for days a nurse is physically present in the home, which advocates say makes the benefit impractical for many patients. Pending legislation would loosen that requirement, and a 2026 law addressed some qualifying criteria for drugs under the benefit.27NHIA. Fixing the Part B Home Infusion Therapy Benefit The Lupus Foundation of America recommends discussing home infusion with your physician to determine whether it is safe and appropriate for your situation.28Lupus Foundation of America. No Biosimilar on the Horizon

There is no FDA-approved biosimilar for belimumab, and based on the current patent landscape, one is not expected to reach the market until after 2030.29DrugPatentWatch. Belimumab Biologics Information For the foreseeable future, Benlysta is the only belimumab product available, so pricing competition from a biosimilar isn’t a factor in planning your out-of-pocket costs.
