The core requirement is medical necessity. Humana expects a reasonable likelihood that your condition will improve meaningfully within a predictable timeframe, generally about one month for rehabilitative care, and treatment must follow a physician-approved, individualized plan with specific, measurable goals.2GenHealth AI. What Humana Will Not Pay For
Several categories sit outside coverage regardless of plan type:
- Aquatic therapy and pool-based rehabilitation.
- Sports-specific rehabilitation designed to return you to baseball, golf, running, or dance.
- General conditioning, fitness, flexibility, or weight-loss programs.
- Treatments Humana classifies as experimental or investigational, including dry needling, dry hydrotherapy (aqua massage), virtual reality-based therapy, and the Graston technique.
- Receiving occupational therapy and physical therapy for the same clinical condition, unless the two programs are distinctly different.
- Maintenance therapy after your condition has plateaued or treatment goals have been met.

Costs Under Medicare Advantage
Humana Medicare Advantage plans must cover everything Original Medicare covers, which includes outpatient physical therapy under Part B.3Humana. Copays vary by plan. The Humana Gold Plus H5619-122 HMO, for example, charges a $25 copay for physical therapy at a comprehensive outpatient rehab facility or specialist’s office, and $35 at an outpatient hospital. Costs Under Commercial Plans
Humana commercial plans, whether purchased individually or through an employer, cover medically necessary physical therapy with different mechanics than Medicare Advantage. Visit limits are set at the plan level and typically run 20 to 60 visits per benefit year. Copays usually land between $25 and $50 per visit. On a high-deductible plan, expect to pay the full session cost (often $100 to $250) until you meet your deductible. Referrals and Prior Authorization
HMO plans generally require a referral from your primary care physician before you can see a physical therapist. PPO plans typically don’t, though cost-sharing differs. Some Humana Medicare Advantage HMO plans state outright that no referral is required for covered in-network services.6MedicareAdvantage.com. On prior authorization, Humana removed the preauthorization requirement for outpatient physical therapy on commercial and Medicare Advantage plans starting in late 2017.9Gawenda Seminars. State law can override company policy. Humana’s clinical policy states that “any state mandates for rehabilitative PT or OT take precedence over this medical coverage policy.”1Humana. In-Network vs. Out-of-Network
Staying in network matters. HMO plans generally don’t cover non-emergency care from out-of-network providers at all. PPO plans do, but at a much higher cost; out-of-network coinsurance can reach 40% to 50% of treatment costs, compared with modest in-network copays. Verify a therapist’s network status through Humana’s online provider directory or by calling customer service before your first visit.
Home-Based and Telehealth Physical Therapy
Humana Medicare Advantage covers in-home physical therapy on the same terms as Original Medicare. You need to be under a doctor’s care, require only part-time skilled therapy, and be considered homebound, meaning it’s very difficult to leave home without help from another person or an assistive device like a cane, walker, or wheelchair. A face-to-face visit with a doctor or qualifying provider is required to certify the need. Telehealth coverage for physical therapy exists on a timer. Physical therapists are currently allowed to furnish Medicare telehealth services under a temporary extension running through December 31, 2027. If Humana Denies Your Claim
You have the right to appeal, and the deadline depends on your plan. Medicare Advantage members must request an appeal within 65 days of the denial date. Medicaid members have 60 days, with contact details that vary by state. Commercial plan members generally have 180 days.
Medicare Advantage appeals can be filed online through Humana’s website, by mail to Humana Grievances and Appeals (P.O.
